3. The major responsibilities of the Office of Inspector General have in relations to coded health care data is fraud and abuse. The Office of Inspector General is also the largest health care program that watches over Medicaid and Medicare; they investigate the pharmaceutical companies and medical supply companies to make sure they are legit.
4. I’m not too clear on this question.
5. I compared three hospitals; our hospital here in high point NC: High Point Regional Health System, a hospital in a neighboring town in Thomasville NC: Novant Health Thomasville Medical Center and one of the top teaching hospitals in NC: Duke University Hospital. From what I see, Thomasville NC: Novant Health Thomasville is the better for us because High Point Regional Health System is over crowed and under staffed. Duke University Hospital is a great hospital for specialist. My mother goes to Duke to see a liver specialist and my dad use to go for cancer treatments.

What should an organization do? How does one create an environment that promotes data quality? What are some of the problems associated with very poor quality-data?
An organization should retrain their nurses and staff in the importance of quality- data. Patient safety is the most important aspect in a health care facility; if data error is made then the result can be serious, case in point with the second example of this scenario. Some problem with poor quality-data: miss communication, incorrect patient records, causing danger to the patient or even death.

Medical errors can happen in the healthcare system such as hospitals, outpatient clinics, operating rooms, doctor’s offices, pharmacies, patients’ homes and anywhere in the healthcare system where patients are being treated. These errors consist of diagnostic, treatment, medicine, surgical, equipment calibration, and lab report error. Furthermore, communication problems between doctors and patients, miscommunication among healthcare staff and complex health care systems are playing important role in medical errors. We need to look for a solution which starts changes from physicians, nurses, pharmacists, patients, hospitals, and government agencies. In this paper I will discuss how does the problem of medical errors affect our healthcare delivery system? Also how can these medical errors be prevented and reduced?
