Gene Therapy for Cystic Fibrosis
Modern molecular genetics has given hopes and heartaches to thousands of people around the world. These people are looking towards gene therapy for an answer to their questions. To some people such as NIH director Harold Varmus the answer is a better understanding of basic genetic research and to others the answer is a cure, a hope, that their lethal disease will someday be cured. This essay touches on the background of gene therapy for Cystic Fibrosis (CF), current social and ethical issues facing gene therapy for CF, and some thoughts on the importance of this controversial subject.
Gene therapy is the application of the technique where the defect-causing "bad" genes are replaced by correct "good" genes. The idea of gene therapy is to treat the disease by correcting the "bad" DNA (Deoxyribonucleic acid) rather than the current me thod of providing drugs, or proteins not produced by the defective gene. Gene therapy addresses the problem first hand by directly working with the genetic information causing the disease. From the book Shaping Genes, Dr. Darryl Macer says "It is like f ixing a hole in the bucket, rather than trying to mop up the leaking water." There are two kinds of gene therapy, somatic cell gene therapy and germline gene therapy.
Somatic cell gene therapy is where genes can be put into specific cells and areas of the body which are affected by the disease. All cellular DNA in our body is essentially the same because it was replicated from the same zygote (fertilized egg). Cel ls differentiate into their respective tissues depending upon which part of the total genome is used.
Germline gene therapy is where the correct "good" gene is inserted into the germline in place of the defective "bad" gene, and when reproduction occurs the gene will be passed on to the progeny. Inserting the "good" gene into the very early embryo sta ges of development allows for both germline and somatic cells to be corrected. Government has limited the research to only somatic cell gene therapy such as performed in Cystic Fibrosis research.
Cystic fibrosis is one of the most common lethal mutations in humans. The autosomal recessive allele is carried by 1/20 Caucasians, 1/400 couples will have children with the disease, and ¼ children will be afflicted. If untreated, 95% of affected ch ildren will die before age five (Bell, 1996).
Cystic Fibrosis (CF) Pathophysiology: Cystic fibrosis is a genetic disease of the secretory glands that affects the respiratory and digestive system. It mainly affects the lungs, pancreas, liver, intestines, sinuses, and reproductive organs. Cystic fibrosis affects the cells that produce sweat, mucus, and digestive fluids. Mucus becomes thick and sticky, causing build-up in the lungs and blocking airways, making it easier for bacteria to develop. This prompts repeated lung infections and can cause severe lung damage after some time.
Being diagnosed with a chronic illness is a life-altering event. During this time, life is not only difficult for the patient, but also for their loved ones. Families must learn to cope together and to work out the best options for the patient and the rest of the family. Although it may not be fair at times, things may need to be centered on or around the patient no matter what the circumstance. (Abbott, 2003) Sacrifices may have to be made during difficult times. Many factors are involved when dealing with chronic illnesses. Coping with chronic illnesses alter many different emotions for the patients and the loved ones. Many changes occur that are very different and difficult to get used to. (Abbott, 2003) It is not easy for someone to sympathize with you when they haven’t been in the situation themselves. No matter how many books they read or people they talk to, they cannot come close to understanding.

In 1758 a Swedish botanist named Carolus Linnaeus established the classification system still in use for various forms of life. He listed four categories that he labeled as "varieties" of the human species. To each he attributed inherited biological as well as learned cultural characteristics. He described Homo European as light-skinned, blond, and governed by laws; Homo American was copper-colored and was regulated by customs; Homo Asiatic was sooty and dark-eyed and governed by opinions; Homo African was black and indolent and governed by impulse. We can in retrospect recognize the ethnocentric assumptions involved in these descriptions, which imply a descending order of prestige. Most striking is the labeling of the four varieties as governed by laws, customs, opinions, and impulse, with Europeans on the top and Africans at the bottom. In fact, different populations within all four varieties would have had all four forms of behavior. (8).

It was not until the 18th century when the word race began to enter languages and vocabularies, and this idea of a difference between peoples was prodded further into existence through the work of Carolus Linnaeus. Linnaeus composed a list of subspecies of human beings based on racial differences. There were several other scientists, such Georges Cuvier and Charles Darwin, as who created subspecies of man. Social Darwinism, alluded to the concept that eventually one greater subspecies of man would prevail and be the most elite of all of human kind. These lists often categorized the order of species with the white, European man at the top of the list and the darker skinned, African man at the bottom. Through the selected readings it becomes clear that race is not only a social construct but also a value that changes depending on the region in which one inhabits. Despite the lack of scientific support for race as a biological phenomenon, race still results in misfortune for many minorities. This present throughout everyday life in terms of job opportunities, education, and life experiences.
...The most profound conclusion on the concept of race is the argument that the term is not a biologically innate fixture. Despite the discredited nature of the concept of ‘race’, the idea stills “exerts a powerful influence in everyday language and ideology”. Maya Angelou illustrates the destructive nature of prejudice, and the effects that it may have on society. Her childhood took place primarily in the 1930’s, a time also known as the Depression Era where racism dictated the lives of many African-Americans that Harper Lee details in her novel To Kill a Mockingbird. Martin Luther King Jr., an advocate of black rights during the depression era, details in his essay “An Experiment in Love” how Christian love, or “agape” had sparked the African-American nonviolence movement. The novel is told through the perspective of a 6-year old girl, Scout and centralizes on two innocent figures, Arthur Radley, a social outcast, and Tom Robinson, a black man accused of rape, who are persecuted due to the prejudiced view of the town. As she matures and learns about the town, she begins to develop ideas and pass judgment on individuals that are in the town. Atticus Finch, Scout’s father, takes up the court trial to defend Tom Robinson, who is wrongly accused and sentenced of the rape of a white woman and eventually runs, only to be shot by the prison guards. Despite the open communication present in Harper Lee’s To Kill a Mockingbird, there is deficiency of love, as the author illustrates that societal prejudice is inevitable and leads to discrimination.
