Introduction
Mr David King is a 71-year-old male who lives at home with his wife Mary. David is retired but remains active. He has no serious past medical history, but his parents died from ‘heart issues’ and remembered that his dad had experienced ‘blood pressure problems’. He self-administers analgesic for his headaches that he has been experiencing in recent months.
David was admitted to the Accident and Emergency department with his wife. As told, David was found slumped upstairs in the shower, his right side of his body was faccid, his face and mouth had drooped and he also had difficulty in communicating with slurred speech. David was unable to raise his arms and was incontinent of urine.
Conversed in this study include nursing assessment tools, identifying health problems, goals, interventions and rationales and key issues after discharged for the individual.

Glasco Scale
A Glasco Scale is a neurological assessment performed by a nurse. This assessment is to obtain specific data in relation to the function of central nervous system for a client in danger of deterioration. (2010, Tollefson, J) There are many aspects of the assessment, including an assessment of level of consciousness, the orientation of the client, motor response, pupillary activity, muscle strength and tone and vital signs. Thus, the importance of this case is to demonstrate the nursing skills regarding to an individual and their health situation. Allowing the nurse to enhance the quality of care and ensuring safety at all times is achieved for the individual and overall performance.
