The patient is an 86-year-old female who was found on the floor by her son. He left the home for approximately an hour and when he came back he found his mother on the floor. She was confused the day prior to admission. She relates that she had insomnia for the past 2 days she was to refill her Ambien came back and took her Ambien she was actually going down the stairs she felt dizzy and fell down. She has a history of vertigo and has been on Meclizine for that and there is also questionable history of loss of consciousness. She has a long-standing medical history of hypertension and dyslipidemia, as well as dementia. Initially on presentation her BP was 132/72, pulse of 60, respirations of 14, she is afebrile and oxygenating well on

Hitchcock uses many techniques that act as signatures on his films, enabling the viewer to possess an understanding of any Hitchcock film before watching it. His most famous signature is his cameo appearance in each of his films, but Hitchcock also uses more technical signatures like doubling, visual contrast, and strategically placed music to create suspense.
Alfred Hitchcock’s Vertigo is a thrilling film filled with mystery and suspense. However, Hitchcock left many unsolved issues at the end of this film. In contrast, when comparing Vertigo to more recent films of similar genre’, mysteries are usually always solved and thoroughly explained by the end of the film. Ironically, Hitchcock’s failure to explain everything to the audience in Vertigo is one of the film’s best attributes. This lack of knowledge allows the viewer to use their own imagination and speculate as to what might or might not have become of certain characters.

The story takes place in San Francisco in the 1950's. The film opens on a high building, where officer Scottie and his partner are in pursuit of a suspect. Scottie's partner's life is on the line and only he can save him. Unfortunately, he has vertigo, a fear of heights. Scottie is unable to assist his partner who unfortunately falls to his death.
J.P., a 58 year old female, presents to the Emergency Room on March 18th. She has a past medical history of cervical cancer, atheroembolism of the left lower extremity, fistula of the vagina, peripheral vascular disease, neuropathy, glaucoma, GERD, depression, hypertension, chronic kidney disease, and sickle cell anemia. She complains of right lower extremity pain accompanied by fatigue, a decreased appetite, increased work of breathing, burning urination, and decreased urine output for three days. Upon admission, a complete physical assessment was performed along with a blood and metabolic panel. The assessment revealed many positive and negative findings.

At the time Vertigo was produced, Hollywood studios were still very much in control of film production and of actor’s contracts. I will be examining the following characteristics of the film Vertigo: 1)individual characters who act as casual agents, the main characters in Vertigo, 2)desire to reach to goals, 3)conflicts, 4)appointments, 5)deadlines, 6)James Stewart’s focus shifts and 7)Kim Novak’s characters drives the action in the film. Most of the film is viewed in the 3rd person, except for the reaction shots (point of view shot) which are seen through the eyes of the main character.(1st person) The film has a strong closure and uses continuity editing(180 degree rule). The stylistic (technical) film form of Vertigo makes the film much more enjoyable. The stylistic film form includes camera movements, editing, sound, mise-en-scene and props.

She has a very complicated medical history of mitral valve prolapse, uterine fibromas having hysterectomy, a question of Ménière's disease, anxiety, hypertension, asthma, CVA in 1994 with mild right-sided residual weakness, urethral stenosis, recurrent UTIs, pulmonary embolism, and idiopathic afib she did presyncopal developed Equinox, a history of diverticulosis, and diverticulitis. The patient is admitted inpatient. It is to be noted initially there is a question of a syncopal episode. Troponins are negative. She is dehydrated with urinary specific gravity of 10:30, and she has positive nitrates and leukocytes. She was initially placed in the emergency
75 year old man with a history of diabetes, coronary artery disease, and COPD requiring nasal oxygen is brought to the clinic by his daughter. She reports that the patient has poor memory and has been getting worse over the past 2 years. She states that after he makes a statement or asks a question, he will repeat the statement or question again 15 or 20 minutes later. He can no longer manage his personal finances. The patient reports that his mood is good, and that he gets up occasionally to urinate throughout the night, but is able to get back to sleep quickly once he is done..
Though complex and brilliantly written for its time, the plot of Alfred Hitchcock’s film, Vertigo, is only half of the genius behind it. Alfred Hitchcock’s unique presence as an auteur is truly what sets his films apart. There is symmetry to his shots that give the film an artistic feel, as if each frame were a painting. Many times, within this symmetry, Hitchcock places the characters in the center of the frame; or if not centered, then balanced by whatever else is adding density to the shot. For example, as Madeline sits and looks at the painting in the museum, there is a balance within the frame. To counter-act her position to the right of the painting, Hitchcock puts a chair and another painting on the left side, which is visually pleasing to the eye of the audience. The use of red and green not only adds a visual effect as well, but later serves as a clue that Madeline is not actually dead, when the women who looks like her is wearing a green dress.
My colleague and I received an emergency call to reports of a female on the ground. Once on scene an intoxicated male stated that his wife is under investigation for “passing out episodes”. She was lying supine on the kitchen floor and did not respond to A.V.P.U. I measured and inserted a nasopharyngeal airway which was initially accepted by my patient. She then regained consciousness and stated, “Oh it’s happened again has it?” I removed the airway and asked my colleague to complete base line observations and ECG which were all within the normal range. During history taking my patient stated that she did not wish to travel to hospital. However each time my patient stood up she collapsed and we would have to intervene to protect her safety and dignity, whilst also trying to ascertain what was going on. During the unresponsive episodes we returned the patient to the stretcher where she spontaneously recovered and refused hospital treatment. I completed my patient report form to reflect the patient's decision and highlighted my concerns. The patient’s intoxicated husband then carried his wife back into the house.

However, I decided to recheck the pulse manually, founding that it was irregular (78 bpm). The patient stated that she felt better after rest. Immediately after the incident I made a decision to explore carefully the medical chart of Mrs. D., along with her nursing care plan.
