This paper is the summary analysis of the health assessment on a family based on the family-focused questions of the following eleven health patterns: 1. “Values, health perception 2.Nutrition 3.Sleep/Rest 4.Elimination 5.Activity/Exercise 6.Cognitive 7.Sensory-Perception 8.Self-Perception 9.Role Relationship 10.Sexuality 11.Coping”.In the course of the interview and assessment, the author identified three wellness nursing diagnoses in the family that also form part of this paper. This author interviewed a family called Josephs. The family consists of a father, mother and two children. The father is 46 years old and the mother 44 years of age. The son is 20 and daughter 17. All family members respect each other’s space. The parents instilled good family values in the children at an early stage and they follow it. The parents follow the customs and traditions of the country they came from whereas both the children prefer to follow the American system. Questions regarding sexuality, they did not want to divulge any information, the father simply stated that they do not have any problem in that area. Both children got a proper sex education from schools and catechism classes. The family’s way of coping life situation is through family support, prayers and short vacations. Their open communication and support for each other, help them to reduce stress. The family’s deep rooted religious faith and seeking divine help at the time of troubles ease their stress and tensions. They do take short family vacations that help them to recharge their energies and get rid of tensions. One positive thing noticed about the family is that they do not use any medication for getting sleep or stress. No one in the family uses alcohol or tobacco except the father who smoke a pack of cigarettes and two pegs of whisky on a daily basis. Research has provided evidence that single-parent households are one of many risk factors that can negatively affect a child’s educational outcome, emotional health and social behavior. Although not all single-parent family households have disadvantages, the focus of his study is to shed more light on the issues and offer solutions. For example, more policies are needed to reinforce fathers to pay child support. This alone will provide mothers with more money to help them better raise their children.

Interdisciplinary care plan meeting between a healthcare staff patient and family members is another example of how patient-centered care(PCC) can be achieved. The patient’s health condition and care should be discussed while still in a health care facility to improve care and to prepare the patient for discharge either to a rehabilitation facility or home with the involvement of the patient and family.
