When time passed after midnight because of GCS the child couldn’t fall in to sleep and mum feeling distressed, then mum asked the nurse (diploma) who is looking after the kid to skip hourly observations. The nurse (diploma) checked with the doctor on call for neurosurgery and he explained to the nurse to advice mum the importance of hourly observations. However similar situation with different nurse (Baccalaureate degree), when parents asked to skip the hourly observations she has explained them in detail consequences for not doing observation and parents realized the importance to continue observations for their child. The difference is diploma nurse did not try explaining to the parents again, but BSN nurse with her knowledge explained and taught the parents about the seriousness of head injury observations. A student’s educational record is supposed to remain private between the student, parents and teacher. A teacher cannot release information without consent and if they do release the information without consent, the teacher could be fired. The Family Educational Right to Privacy Act restricts a teacher’s ability to share a student’s educational record without permission. It is unethical and potentially harmful to the student for a teacher to share a student’s grade with the class. Releasing educational records without permission is not the only unethical thing a teacher can do and I will go into further depth on this later in my writing.

Health or nursing assessments are continuous cycles of data collection and making inferences. There are steps to be taken to ensure adequate collection of objective and subjective data, these steps need to be taken in order otherwise an inaccurate assessment may occur. These steps are: assessment, diagnosis, planning, implementing and evaluation (Lewis, Foley, Weber, & Kelley, 2011, p. 2-5).
