The communicable disease reporting form also known as the DOH-386 form is used by public health agencies to not only control communicable human diseases, but also to detect outbreaks, prevent secondary transmissions, and evaluate the effectiveness of control measures (Smith, 2007). One of the many reasons why its mandatory to complete and send the DOH- 389 form to the local department of health is because if there is an increase of a communicable disease in a certain area the department could get more government funding to fight the diseases. Furthermore, reporting communicable diseases to the local health department can help detect outbreaks and prevent secondary transmission. This is especially true when there is an outbreak of illnesses, such as Encephalitis, smallpox, and rabies. These illnesses are warrant prompt and need to be reported immediately (2017). Reporting these diseases through the DOH-389 form can not only help control communicable diseases, but also detect outbreaks, prevent secondary transmissions, and evaluate the effectiveness of control measures (Smith, 2007). This is especially important when there is an outbreak of illnesses such as Rabies, Smallpox, and Encephalitis these diseases can be dangerous and need to be reported immediately. It is the responsibility of physicians, nurses, laboratory directors, infection and control practitioners, health care facilities, state institutions, and schools to report communicable diseases (Communicable Disease Reporting Requirements, 2017). The state has placed local agencies like the CDC and the Local Department of Health to help the public battle these illnesses. Filling out the DOH-389 form can help save
Many states and colonies across the globe issued detailed sets of directives to their residents on what exactly they should do if they come into contact with the illness. One such example is the directive issued by T.W.H. Holmes, the Secretary of the Victoria Board of Public Health in Australia. The directive details the symptoms, complications, treatment, and prevention of the disease. Something very common during the outbreak of any pandemic is the use of quarantines to separate the sick and the healthy. In fact, that is the first order for prevention of disease in T.W.H. Holme...

Direct legal issues that result in inadequate infection-control practices, medical costs of healthcare-associated infections and the amount of deaths that have occurred due to these preventable infections are the main targeted issues that will focus on this project. Reporting requirements of HAIs vary from state to state, provider, facility, frequency, and type of infection. Due to this, there is inconsistency in the methods of data collected, risk management, data validation, and the requirement or reporting HAIs. The legal requirements and statutes that mandate disclosure of errors need to be addressed in order to reduce and prevent HAIs. In addition, the discussion of legal duties and responsibilities of the care providers, facilities and patients are discussed.
It is mandated to report suspected or confirmed communicable disease under the New York State sanitary code (10NYCRR2.10, 2.14). Measles should be reported immediately to New York local health department where the patient resides. Reporting can be done by phone call or by reporting measles form PD-16. For example, in schools the school nurse will notify about the measles outbreak to communicable disease Liaison and then the Liaison will forward case to epidemiologist after blood testing and confirmation of measles disease.
In1994, there is another outbreak in Motaba River Valley. Unfortunately, an African monkey carrying that biological agent (pathogen must exist in order for the disease to occur) has brought into the United States. Unfortunately, the film shows two outbreaks that occurred in Boston and California. Outbreak portrays communicable (infectious) disease in the Cedar Creek. Communicable diseases are diseases that can be transmitted from one person to another through biological agents or their products. This biological agent had all three characteristics: infectivity (Ability to invade a host, survive & multiply) , pathogenicity (experience to produce clinical disease) and virulence (ability to cause serious illness...

The summary of CDC Reportable Diseases contains the statistics of the notifiable illnesses and conditions. "A notifiable infectious disease or condition is one for which regular, frequent, and timely information regarding individual cases is considered necessary for the prevention and control of the disease or condition" (www.cdc.gov). Reportable diseases and conditions are reported to public health authorities by health care providers. Protection of public health is ensured by proper identification and follow up cases. "Case notification of nationally notifiable infectious diseases and conditions helps public health authorities monitor the effect of these diseases and conditions, measure the disease and condition trends, assess the effectiveness
It is essential that the advanced practice nurse provide accurate information and thoroughly answer patients’ questions about vaccines and their safety. A strong knowledge base, and proper educational techniques are essential for the role of the advanced practice nurse. According to the CDC (2017a), healthcare providers play a vital role in maintaining elimination of measles in the United States. Healthcare providers should maintain a high awareness of measles and suspect measles in persons who have a febrile rash and clinically compatible symptoms (e.g., cough, coryza (symptoms of a cold), or conjunctivitis) and those who recently have traveled abroad or have had contact with travelers. Identification and awareness, as well as proper reporting to the local health department are key preventative strategies for the advanced practice nurse, and should be utilize in the care of all patients. Education, and available resources are the key to prevention of the measles virus, and should be the central focus of care for the advanced practice nurse caring for any at-risk
Everybody wants a long lasting healthy relationship with their partner. Every relationship teaches couples something and helps them develop as individuals. According to the survey on (magazine.foxnews.com/love/expert-survey-reveals-number-one-reason-couples-divorce), 25-50 percent of divorces could have been avoided with couples' therapy. In Australia in the last ten years more then 137 known people have died and many more fallen very ill from contagious and infectious disease. Diseases such as diphtheria; tetanus, pertussis, poliomyelitis, measles, mumps rubella and Haemophilus influenzae, This is a great tragedy considering all these diseases are easily preventable by immunisation.
The Centers for Disease Control and Prevention (CDC) studied the illness and death rates before and after widespread implementation of national vaccine recommendations (in place before 2005) for 13 vaccine-preventable diseases; diphtheria, pertussis, tetanus, poliomyelitis, measles, mumps, rubella (including congenital rubella syndrome), invasive Haemophilus influenzae type b (Hib), acute hepatitis B, hepatitis A, varicella (chickenpox), Streptococcus pneumoniae and smallpox.
Many diseases get a lot of exposure and people know them well. One, however, gets looked over but it affects, “1% to 3% of people.” Geographic
