On 5/2/17 a separate ISP meeting was completed (because of PFA) with Melissa Carson. Caseworker, Kenesha Grant and Ondria Moore, spoke with mother regarding why DHR is involved. According to Mrs. Carson, the reason for DHR involvement is due to her Ex-husband-Philip’s domestic incident that exposed their children (Penelope, Judea and Jianna) to a safety threat. Mrs. Carson provided worker copy of the children Birth cert, SSN, PFA and custody hearing document. According to her, Mr. Miller and he were married in 2006 and divorce in 2016 Mrs. Carson, told worker Mr. Miller has always been abusive physically, emotionally and verbally towards her and his current wife Lena Miller. She said that Lena Miller is the person is force to take care of
As the EAI team was discussing Molly’s case, one of the ED Residents made a few telephone calls. Molly’s PCP reported that during her last visit about 2 weeks ago, Mollie was alert and able to respond to questions appropriately. He confirmed that Mollie’s daughter and son in law have experienced psychiatric problems, adding that the son in law has expressed anger regarding Mollie’s living arrangements. The home health care agency was contacted. The RN and aide both report they have never met the son in law and have had very limited contact with Mollie’s daughter. When contacted by telephone, the daughter provided no explanation for Mollie’s extensive bruises noted on admission to the hospital. The daughter stated that Mollie did not fall, but in fact lowered herself to the floor in an effort to draw

Alameda has had a hard life as a young girl growing up, both of her parents were alcoholics. Alameda was a 16 year old minor who had a baby and dropped out of school, and then was unable to care for the infant. A case manager by the name of Barbara LaRosa was assigned to Alameda case. Barbara took on Alameda as her client and made a visit to her parents’ home, while making the visit she found Alameda dad incompetent, and could not get any information from him to help with his daughter well-being.
Social agency and the court authorizing the placement, and caregivers are responsible for the continuing monitoring to ensure that the child in placement receives adequate care and supervision (Downs, Moore and McFadden, 2009, p.275). Services for children in foster care are a teamwork effort of the different parties involved (Downs, Moore and McFadden, 2009). Unfortunately in Antowne’s situation the agency and the court system failed him because although he was removed from his mother, the abuse and neglect continued. The systems involved did not provide the safety net Antwone needed.
Waters explain what she does on a micro level it fits the description of what a unified model of advocacy entails. From when a child is removed from their homes and placed into the care of the state, specific tasks have been allocated to her and her unit. As the social worker or case worker read the reports provided an understanding is developed. And if an understanding isn’t developed through reports, the next step of visiting the child, requesting documents from psychiatrist, medical doctors and/or psychologist allows one to gain further insight. A case plan is then created, which states what services the parents and the children must complete and is presented in court to a judge. Many parents are faced with this issue across the United States, but in Katy’s case, she has inflicted with a life changing event a terminal illness that prevents her from working and paying for child care services. Katy received a letter from her employer stating that they will no longer reimburse for child care services because of her not working. Denial of child care has contributed to the exacerbation of Katy’s symptoms and poor health outcome. Therefore, I am requesting a meeting held during this week with Katy and the Department of Social Services (DSS). The discussion will be focused on denial of child care payments and agreement for a Fair Hearing to discuss Katy’s inability to work and her limitations.
I will be evaluating the case of Angela and Adam. Angela is a white 17 year old female and Adam is her son who is 11 months old (Broderick, P., & Blewitt, P., 2015). According to Broderick, P., & Blewitt, P., (2015) Angela and her baby live with her mother, Sarah, in a small rental house in a semirural community in the Midwest. Adam’s father, Wayne, is estranged from the family due to Sarah refusing to allow him in the house however, Angela continues to see him without her mother’s permission which is very upsetting for Sarah. The video was about a interview was with Sue Dowling, who currently works with the Social Service Department in the Arlington Massachusetts area, and is an ongoing assessment social worker. Dowling noted in the interview that there are three phases in the Department of Social Services. The first phase is an intake or investigations. The second phase is the assessment. The reporting party (RP) reported foster mother Lori Steele located at 1048 Newhill Street, Glendora CA 91740 observed foster child Chateau Lane-Jones had been sending nude photos and video to men on "Facebook" and "Snapchat. Some of the replies from the mew indicated they were going to see Chateau. The replies included pictures of men. On 10/20/16 Chateau provided the name of her school and the foster parent's address. This information was provided by Chateau's foster sisters. The RP was not aware if the pictures the men submitted were nude or not. According to the RP the foster mother has been proactive in protecting
On 3/16/17, Hennepin Child Protection accepted a report alleged physical abuse, domestic violence of Destiny Jennings by half-brother, Montez Jennings. Per reporter Destiny and Ms. Jennings came home and Montez began threatening Ms. Jennings states he was going to “knock that smile off your face” and "you're really gonna get it if you call the police. Per reporter Montez was throwing food at Ms. Jennings and approached her then Destiny stepped in and Montez pushed Destiny away. Per reporter Ms. Jennings had to go outside to call police as Montez threatened to knock the phone away if she tried calling police. Per reporter Ms. Jennings went to the garage because she was afraid for her safety.
When the agency is screening incidence in Erie County they determine which caseworkers are best for the job. When OCY has information on a recurring family, the agency in their best effort try to send caseworkers who the client may be more comfortable to engage. The client may be a formerly physically abused partner, who may not want to talk with a caseworker being the same gender as their attacker. OCY makes it a priority to hire caseworkers with some ethnic backgrounds, or who have worked with various people. The agency wants the client to be more cooperative, and open to the agency if they see a caseworker like
Mr. McCoy’s testimony revealed that his first issue is regarding the custodial parent (CP), Sabrina Durant Hawthorne. He stated the CP has not had his daughter, Daisha Durant, since she was three years old. He stated he mailed the court order that stated that the child support was to stop to the DHR. He stated he received a garnishment from his bank account and did not understand why. He stated that he called Child Support and was told that he owed the CP $3,000.00; he did not understand why he owed the CP money when the case was closed. He also stated that his aunt, Shirley McKee, has had the child since 2000, the child is 19 years old now and the child support case was closed.
On 08/27/15, Denise came to school visibly upset. During the day, Denise disclosed to her counselor that on 08/22/15 her mother slapped her in the face after they argued about wanting to visit her father. The reporter did not see any visible marks or bruises on Denise’s body but the child told her this isn’t the first time her mother has hit her. Denise told the reporter each day after school, her mother makes her strip naked and she searches her body, room, and back pack. There is an alarm system and motion sensors all over the home and a camera in Denise’s room. The child was adopted by Ms. Flowers at the age of 4 and has been in the home since. Denise basic needs are being met at this time.
