Regimen of Continuing Treatment: Includes a course of prescribed medication (e.g. antibiotic) or therapy requiring special equipment to resolve or alleviate the health condition. A regimen of continuing treatment that includes the taking of over-the-counter medications such as aspirin, antihistamines, or salves; or bed rest, drinking fluids, exercise, and other similar activities that can be initiated without a visit to a health care provider, is not, by itself, sufficient to constitute a regimen of continuing treatment.

An illness or injury that causes more than three (3) consecutive calendar days of incapacity and involves at least one in-person treatment by a health care provider and results in a regimen of continuing treatment under a health care provider’s supervision,
D1: I have decided to look at a 6 year old going through bereavement. Bereavement means to lose an individual very close to you. When children go through bereavement they are most likely to feel sad and upset about the person’s death. Children at a young age may not understand when a family member dies. Children may not understand bereavement. For example a 6 year old’s father been in a car crash and has died from that incident. Death is unpredictable and children can’t be prepared for a death of a family member as no one knows when someone is going to die or not. Unfortunately every child can experience bereavement even when a pet dies. It is important that we are aware that effects on the child so we can support them in the aftermath.
Chronics are those patients who have a condition which is untreatable, ’machines with flaws which cannot be repaired’ (17) and can only be controlled with medical methods. They will spend the rest of their lives inside the ward of the hospital. Patients who are seen as being likely to recover from their illness, and will return to society.
The purpose of Application of a Motor Learning Treatment for Speech Sound Disorders in Small Groups was to evaluate the effectiveness of motor-learning based therapy, also called Concurrent Treatment, within groups of up to four elementary public school students with disordered articulation, normal language, and normal hearing. The authors of this paper recognized that while many studies have been done to determine the efficacy of students in individualized therapy settings, few studies had been done to look at therapy within small groups. Therefore, the researchers tested twenty-eight 6-9 year old children within a small group using Concurrent Treatment. The children were able to acquire their targeted speech sounds within 40 30-minute sessions (20 hours over 20 weeks).
Case conceptualization and treatment planning is used by therapist to assist in determining a client’s diagnosis, goals, and treatment plan that is most effective in determining the issues surrounding the clients diagnosis. It is crucial that the client’s treatment plan is specific to the individual, is relational and appropriate to the needs of the client.
This helps with reducing 30-day readmission rates, and the overall health of the patient. The appointments are set up for the patient already so that they don’t forget to make them when they are home. The nurses and the case managers educate on the importance of follow-up care. This way the patient is not readmitted for the same disease again. At the same time the case manager is promoting health maintenance of the patient. One of the most important things that the case manager can do is to make sure the patient understands their plan of care. The case manager must be able to properly communicate with the patient by any means deemed
What does ‘care’ mean? Care is the provision of what is necessary for your health, welfare and protection of someone or something. However when you talk about ‘care’ in a care practice the term changes and becomes more about enabling people to meet all their needs which would refer to their social, physical, emotional, cognitive and cultural needs. The individual is central to the meaning of care in this context.
2.3 Explain how the health and social care practitioner own values, beliefs and experiences can influence delivery of care.
Changes in the current health care system can help prevent unsuccessful transitions of care. In order to move away from the “silos” of care, many institutions are starting to trend towards primary patient centered and interdisciplinary care. Having a team in charge of the care for a patient will allow more effective treatments and more communication between the different providers. While this is only within an inpatient setting and not necessarily transitions of care, the variety of clinicians involved in the care of a patient allows more information to be transmitted across different setting. The Society of Hospital Medicine developed Project BOOST to address issues with care transitions and to standardize a method for transition of care. Project
Morgan read over each patient assessment in their chart, as well as rounding on each patient daily to gather her own assessment. With all of the data, she came up with diagnosis that was required from her. Morgan stated the nursing diagnosis she most frequently uses is risk for falls. Goals are then set depending on individual needs. By collaborating with the interdisciplinary team in a therapeutic way, interventions are implemented to meet each patient’s needs. Evaluations are performed daily by case managers through interdisciplinary rounding and the goals that were made are assessed and any changed to the plan of care are made. Case managers will follow up with outside facilities that patients transfer to after a hospital admission to evaluate their progress. If a patient is readmitted to the hospital within 30 days of discharge, a reevaluation is
Chronic illness or chronic condition is a long lasting or persistent disease or condition in a person’s health. A chronic illness can affect anyone’s life at any time. A person’s life can be affected greatly in many ways by a chronic illness. A person may be affected physically, their independence may be taken away, they may feel tired and/or in pain due to a chronic illness they may be suffering from. Financial problems may occur due to the high medical costs treating the chronic illness, someone could lose their, job, their appearance may be affected, and a person may experience confusion. If a chronic illness unexpectedly approaches someone’s life, a person may feel overwhelmingly confused. This confusion may come, because they don’t understand why they now have this illness. This confusion is primarily seen in children. Since chronic illness affects lives so greatly, a person may start to feel anger, stress, or anxiety because of the interruption of the chronic illness in a person’s life.
A health care directive is a legal document, based on personal values and beliefs, providing people with a sense of control and independence for their medical care. The purpose of a health care directive is to allow personal wishes on various treatments options to be met, when the individual is presented incapable. Since it is a legal document, health care professionals and family members have to abide with individual’s health care directive. Personally I value autonomy, independence, family, dignity, and selflessness. These core personal values allow me to reflect, fully comprehend and accept the consequences to the health care decisions I make for myself. In conclusion, the sections of the health care directive
A long-term health condition can also be referred to as a ‘chronic illness’ and it can be used to define recurring conditions such as asthma, arthritis, and diabetes.
The uncertain nature of chronic illness takes many forms, but all are long-term and cannot be cured. The nature of chronic illness raises hesitation. It can disturb anyone, irrespective of demographics or traditions. It fluctuates lives and generates various inquiries for the patient. Chronic illness few clear features involve: long-lasting; can be managed but not cured; impacts quality of life; and contribute to stress. Chronic illnesses can be enigmatic. They often take considerable time to identify, they are imperceptible and often carry a stigma because there is little sympathetic or social support. Many patients receive inconsistent diagnoses at first and treatments deviate on an individual level. Nevertheless, some circumstances require
When I first seek out for therapy, it was interesting and scary. I made my first appointment and I was anxious and upset during my visit. I did a 20 minutes interview over the phone before my first initial office visit. My intake worker that assess my caseload allowed me to know her briefly, and let me know she would not be my therapist, but allowed me to accept the rules, polices, and therapist assigned to me. As the article stated, “explain what therapy is, how it works, and answer questions about what the client can expect from therapy in general and from therapy with you.”
My Understanding of the Human Behaviour Theory: BEHAVIOURAL APPROACH Human conduct, how we act and how we react are all related to the theory of behaviourism, or the behaviourist approach. Behaviourism as a theory was first founded by John B. Watson and B.F Skinner who saw that all human behaviour is influenced and can be replaced by new, sometimes more appropriate behaviours. John B. Watson knew that by observing obvious behaviours, the reason for certain actions could be identified. This is where the term classical conditioning comes from, meaning of which is when specific influences create or bring on a certain response. B.F Skinner was the man who developed the theory of operant conditioning.
