The practise of Chiropractics Originated from 1895 in America, when Daniel David Palmer- the founder of chiropractics, performed the first procedure on a deaf janitor. Chiropractics is a fundamental system based upon the ideology that abnormal bodily functions correlate with the maladjustment of the nervous system and a displacement of the vertebrae hence the treatment and prevention is intended towards the neuromuscular system. The chiropractic profession is based upon the treatment of the structures of the body, in particular those of the spinal column. Adherents of this practise are guided through the belief that the body is self healing and self regulating however interferences towards the spinal cord and nervous system- chiropractic subluxations, need to be located, analysed and fixated. The primary approach of chiropractics is to manipulate and in some sense adjust the spine and other bodily structures without the use of surgery or drugs although varying chiropractors may utilise radiology and diet/lifestyle functions to further harmonize the body .An example of this is the correlation between lifestyle factors such as smoking and the result of lower back pain. Similarly, this can be seen through the cause of lower back pain due to obesity .
Osteopathy is another variation of alternative medicine founded in the 19th century which practises a holistic approach of healing the musculoskeletal system. My most defining experience with osteopathic medicine was shadowing a family physician, Dr. Truong. I was impressed by his holistic, patient-centered care and his hands-on manipulative skills. Not only did he provide the medical treatment to his patients, but he cared for them mentally and spiritually. For example, he asked his patients about their life goals at their initial visit, and he reminded and encouraged them to work towards their goals during their future visits. He also promoted healthy lifestyle, such as eating low sugar, high vegetable diet and doing exercises regularly. As a result, one of his patients lost 20 pounds by following his advice. Additionally, Dr. Truong used OMT and acupuncture to help his patients relieve their pain. One of his patients had serious knee pain that could not be treated by others. He found a spot on her thigh and performed OMT, and the pain never came back again. He also taught me the four tenets of osteopathic medicine. With my knowledge of traditional Chinese medicine, which shares some similarities with osteopathic medicine, I quickly comprehended its philosophy.

OA is a musculoskeletal disease that causes chronic joint pain and reduced physical functioning (Laba, brien, Fransen, & jan, 2013). Osteoarthritis (OA) is a non-inflammatory disorder of synovial joints that results in loss of hyaline cartilage and remodeling of surrounding bone. OA is the single most common joint disease, with an estimated prevalence of 60% in men and 70% in women later in life after the age of 65 years, affecting an estimated 40 million people in the United States (Goodman & Fuller, 2009). Women are more commonly affected after the age of 55, almost everyone has some symptoms by the age of 70 (Tan, Zahara, Colburn & Hawkins, 2013, p.78). Osteoarthritis can be described radiological, clinical, or subjective. It commonly affects hands, hips, knees, shoulders, and the spine (Walker, 2011, p. 14). Osteoarthritis is an intrinsic defect in the joint cartilage. Caused by slowly progressive deterioration of articular cartilage that is accompanied by degenerative bony changes, including thickening of the subchondral bone cyst, and formation of large bony protrusions (osteophytes) at the joint margins. Osteoarthritis usually occurs in the large weight bearing joints such as the knees and hips, as well as some of the smaller joints in the hands and feet (Ciccone, 2007,p. 229). ). Osteoporosis has three types of categories the first category is type 1 which occurs in women after menopause and results from declining levels of estrogen and other sex hormones in the body, this could also occur in men due to low levels of the sex hormone testosterone. Type 2 is called Senile Osteoporosis, which occurs in elderly men as well as elderly women because of decreased bone formation due to aging. Besides their other medical studies, osteopathic students get 200 hours of training in "osteopathic manipulative medicine," a hands-on technique for diagnosis and healing. Limited motion in the lower ribs, for instance, can cause pain in the stomach that seems a lot like irritable bowel syndrome. Identifying the muscle strain in the ribs through manipulation, and then treating it, can relieve the stomach distress. An osteopath learns to apply specific amounts of pressure on a body part, attempting to relax it or stimulate it. While such an approach might have raised eyebrows in the profession a decade or two ago, these days almost no one--except perhaps the crustiest old M.D.'s--dismisses it as New Age nonsense. Manipulative medicine is based on the not terribly heretical idea that structures in the body influence function, and that a problem in the structure of one body part can cause problems in the function of other parts.

‘A Collaborative Approach Between Chiropractic and Dentistry to Address Temporomandibular Dysfunction’ by Rubis, Rubis and Winchester found in the Journal of Chiropractic Medicine is a case report that discusses the comanagement of a patient with TMD. In this case, a patient presented with typical symptomology of TMD: headaches; jaw, neck and shoulder pain along with clicking in the TMJ. Treatment by the chiropractor included: pelvic adjustments to correct their pelvic unleveling, thoracic and cervical spine adjustments to address the patient’s shoulder and neck pain. The patient also received manual therapy of their left TMJ where the chiropractor gapped the joint then proceeded to adjust the mandible to reposition the disc as well as myofascial release of the lateral pterygoid muscle. Comanagement by the dentist included a repositioning and night time splint for anterior repositioning of the mandible. After being treated three times per week for six weeks, the patient reported they no longer experience pain or clicking of their TMJ as well as no headaches or neck pain. This case report illustrated that dental and chiropractic comanagement was beneficial for short term treatment and pain resolution for this
Medical study is a combination of clinical experience and scientific research, which requires proof and evidence. These two components can help physiotherapists with diagnosis, provide treatments for patients and making clinical decision. However, what are the ways for individuals to testify the effectiveness of these methods and treatments? Is there scientific evidence proving the information is correct and up to date? How helpful and appropriate are these methods and treatments to the patients? Hence evidence-based practice is necessary. It has a strong impact in physiotherapy, to ensure researches are more focused and relevant to physiotherapists and as a guiding principle to practice and treatment of patients.
