After reevaluating my proposed problem and research questions, I was able to focus on what type of research design and strategy would be best for my Capstone Project Proposal. For review, my proposed problem is on sleep disturbances associated with cardiac surgery patients. My proposed strategy is that by increasing nursing education on sleep there will be a reduction in sleep disturbances for cardiac surgery patients. Keele (2011) describes and differentiates quantitative and qualitative research. After reading over the material for both quantitative and qualitative research designs several times, I have decided on what research method is more appropriate for my Capstone Project Proposal. I will be using the qualitative research method for my problem solving approach. The qualitative design has the following characteristics: subjective data, utilizes inductive reasoning, and develops theory (Keele, 2011, p. 36). The cardiothoracic nurses’ baseline knowledge on sleep would need to be assess by survey. Cardiac surgery patients’ baseline view of their quality of sleep after surgery would need to be collected by Hospital Consumer Assessment of Health Plans Survey (HCAHPS) and interviews. The HCAHPS is one instrument the hospital uses for patient feedback. The patients have the ability to complete a survey after their stay within the hospital. Two of the topics addressed by the HCAHPS is hospital environment and night time quietness. The next step would be implementation of sleep educational classes and another assessment of nurses’ knowledge on sleep. The cardiothoracic nurses’ would then utilize their education on sleep and apply it to their nursing care. Based on the preceding ontological and epistemological assumptions a researcher then makes methodological assumptions. Having decided on exactly what is to be studied the researcher then decides how the research can best be managed thus formulating a plan of action. Considerations include whether the research used should be primary or secondary. Whether one will test an existing hypothesis or whether one will construct a theory after having collated evidence. Finally one can draw conclusions as to which strategy to implement with the ultimate goal of producing the type of knowledge that is required. This then results in the type of method or methods of research to be used.
Hospitalized patients are often hooked up to monitoring devices such as heart monitors, which monitor the electric activity of the heart, or connected to a physiological monitor so their vital signs are constantly being measured. These monitors are intended to continuously assess the patients’ status, and alarm if the patients’ status drops below what is considered normal. The increased use of monitoring devices has created a new phenomenon known as alarm fatigue. According to the ECRI institute (2011), “alarm fatigue occurs when the sheer number of alarms overwhelms staff and they become desensitized to the alarms resulting in delayed alarm response and missed alarms-often resulting in patient harm or even death.” Alarm fatigue has become a major problem within the nursing community and has already had a negative impact on patient safety. Due to the adverse effects alarm fatigue is having on quality patient care, there has been a call to action to find solutions that may deter alarm fatigue. Evidence-based practices involving quality improvement initiatives have been put into effect. The problem has also gained national attention from such institutions as the Food and Drug Administration (FDA) and The Joint Commission (TJC).
This article has shown me that new strategies are needed to reduce sleep disturbances, improve sleep quality, and support the need for supplemental daytime sleep in hospitalized individuals. These strategies include monitoring patients’ sleep and assess quality of sleep and duration, resolving the problems of sleep disturbance, recognizing that nighttime noise, light, and other factors potentially interfere with patient sleep, minimize lighting in shared patient rooms and turn off lights earlier at night, frequently assess for pain and administer prescribed pain medications to minimize sleep disruption. This article taught me more about sleep cycles and disturbances in hospitalized patients. As a future nurse, I have to accurately assess the patients’ personal characteristics and health education needs, and share this knowledge with my classmates.
As student nurses, I have many goals that would like to achieve upon completion of 150 hours of my capstone course. My plan is to use this opportunity to demonstrate competency skills, and knowledge gained through previous quarters. I have listed seven learning goals that I wanted to achieve during the capstone experience. On my first day of capstone, I have gone over each one with my preceptor and requested from her to provide me with feedbacks, so I can use them as stepping tool to work upon. These goals ranged from critical thinking decision making, culture, growth & development, legal and ethical to clinical competence safety/holistic care, evidence-based practice, and professionalism lifelong learning.

Based on the problems associated with the current scheduling system and the research in the previous chapter, it is important that Central DeWitt High School consider the feelings and preferences of the students when creating the their schedules. Therefore, I am proposing that the administration and staff members consider allowing students to indicate who they would prefer to have as their teacher. This may require teachers to teach different courses, which would allow the administration to have more flexibility when creating the master schedule. This would not guarantee that each student would have each teacher that they have requested, but that their input is being considered when creating the master schedule. In the proposed plan, it will be important to educate the teachers’, students, parents, the counselors, and the administration. The plan will need to include how to collect the information, implement the plan, what will happen when a request could not be honored, which courses will need to be taught by more than one teacher and educating the staff, students, parents, the school board,
As nurses frequently interact with the patients, they are the ones exploring evidence-based practice to identify ways to modify the hospital environment and use more nonpharmacological methods to promote sleep to help the body repair itself (Robinson et al., 2005). Florence Nightingale noted patients require proper nutrition, sleep, quiet, and “unnecessary noise harms the patient” (Robinson et al., 2005, p. 263). Imagine how outcomes might improve if we allowed patients to sleep. My MindMap explores the processes and outcomes obtained through the implementation of an ICU sleep protocol. Instituting the sleep protocol in an ICU with various admitting physicians and patient diagnosis will allow the data to be analyzed objectively and in aggregate (Dlugacz, 2006). I believe that the implementation of a sleep protocol will enable patients to sleep in the ICU. The Inputs for this study will include the development of a sleep protocol, education to ICU staff regarding alarm management, the ability to measure sleep through the use of polysomnography and finally the use of a noise monitor to alert staff when the unit is becoming too noisy.
Sleep deprivation is one risk factor for ICU delirium. Sleep deprivation is common for patients in the ICU. The average length of sleep for a patient in the ICU is only 2 hours a day. The sleep quality of patients in the ICU is less than 6% REM. Invasive ventilation along with drugs both disrupt sleep. There is a lack of daylight for most patients in the ICU. The lack of daylight disrupts circadian rhythm and contributes to delirium for critically ill patients. In addition, melatonin production is decreased.
