Some interventions that are used to increase communication skills are provide effective interprofessional communication are the SBAR, team huddles, and multidisciplinary rounds using daily goal sheets. To briefly describe each, the SBAR is a standardized means for communication in patient situations. It asks these four questions, “What is going on with the patient,” “What is the clinical background or context”, “What do I think the problem is”, and “What do I think needs to be done for the patient”(Dingley, 2008). This is used by all health care disciplines, hereby ideally eliminating hierarchy between the physician and the other disciplines. Team huddles are quick pow wows that set the flow of the rest of the day. In the article, it states that these team huddles result inlet interruptions during the rest of the day and immediate clarification of issues (Dingley, 2008). They have specific guidelines to make sure that they run as effectively as possible as well. Care coordination refers to the coordination between and among professional teams that serve valuable roles involved in providing care to clients. Different disciplines of health care professionals include nursing, medicine, case management, pharmacy, nutrition, social work, and allied health professionals, such as speech therapists and physical therapists. They are found in all health care delivery systems and are extremely effective when the focus is strictly on the needs of the client. Interprofessional teams are valuable because each health care professional has specialized knowledge and skills so that health care plans are determined with patients’ best interests in mind. With the communication of ideas amongst the disciplines, their roles consequently complement one another in an age of exponentially growing information. This team process of care coordination can improve quality of care, enhance client satisfaction, and reduce hospital cost by decreasing length of stay; care coordination ensures the best possible outcomes (Koch, 2014, p. 436).
The problem of poor communication stems from an environment of high stress levels. After a consulting company scrutinized processes throughout the hospital related to care coordination and patient flow, the evidence was clear. We set up a meeting with Robin Burse, the Nurse Supervisor of the med/surg floor, to gather information on the current process of the facility regarding patient transfers. Then, we created a survey to better analyze the current handoff process utilized by nurses on the floor as well as what constitutes pertinent information needed during patient handoffs and transfers (Boat & Spaeth, 2013, p. 2) (James, Quirke & Mcbride, 2013, p. 299). According to Robin there was no standardized tool or policy on the transfer of patient information when sending or receiving patients between units. There could be possible barriers to communication and collaboration that could affect the entire team’s success. Personal values and expectation, personality differences, culture and ethnicity, and gender could affect how some individuals respond according to a patient’s care or needs(O’Daniel, 2008).
