Ayden has a tracheostomy and a feed tube. He has nursing care from 3pm-11pm, and from 11pm-7am. From 7am-3pm, Ayden is in the care of his mother (Porsha), and grandmother (Angel). According to the Director of the nursing care, staff members noted concerns of Ayden not being suction in the home. Porsha and Angel have been trained to care for him, but they are not doing so. They are also not putting him on his oxygen. Ayden was hospitalized in December for an infection in his trachea. On 12-25-15, a staff stated he had not been fed at 11pm. Porsha tried to get the nurse to run two feedings through his tube, so they he could be fed. In July, when Ayden nursing services were starting, Porsha and Angel got mad when a staff could not go to the home

Education regarding unit or group thinking is to be encouraged and reinforced. The fact that newborn Rosarie will be entering the home poses unique challenges that will require all members of the family to work together. Maria, Jamie, and Alice must be educated on the signs and symptoms of respiratory distress in the newborn and interventions that must be initiated when distress occurs. The nurse responsible for this teaching must require both verbalization and return demonstration of skills learned to ensure proper reception of the information. Once skills are developed by the adults within the home, the remaining children should then be educated on the signs and symptoms as well and actively participate in care. Involving the entire family will bring a cohesive thinking, and allow the family to work as a unit. A marriage counseling referral should as be provided to Maria and Jamie in order for them to work out their existing issues improving their likelihood of a successful marriage. Showing compassion and demonstrating caring actions when caring for the patient makes it more likely that the parents will trust the nurse and the information the nurse gives them regarding their child’s condition. This trust is important as it helps the parents feel confident in the decisions they are making about their child’s care. When the parents of an ill child in the NICU have decided to terminate treatment palliative care by the nurse and other healthcare providers comes into play. Palliative care is keeping the child comfortable by treating the symptoms and being there for the parents and child physically, emotionally, and spiritually (Eden & Callister, 2010). The patient and family should be placed in a private room that is dimly lit and has minimal noise. There should be enough room to accommodate other family members as well. With the development of rapids response teams (RRTs), acute care nurses and ancillary departments have a resource available to their disposal when need in uncertain situations. Many times nurses struggle to maintain a patient deteriorating in front of them all the while make a multitude of calls to the physician for orders or concerns. Having a set of “expert” eyes assisting you in these times helps alleviate stress and encourages collaboration amongst staff. (Parker, 2014)
Like the general public, I assumed their only job was to aid doctors and perform routine medical procedures that they are trained to do. The purpose of this interview was to gain insight into how much more nurses do, and I was greatly surprised. Marks shared what it is really like to be a pediatric nurse, and all the other roles they must play in their patients’ lives. There are many who believe that the next shortage will be worse and the demand for nurses will increase. There will be more jobs available especially with the baby boomer nurses retiring. Wood believes that when nurses retire, the next shortage could be even worse than the previous shortage. According to Wood this would lead to an “intellectual drain of institutional and professional nursing knowledge” (Wood, 2011, para 15). Staiger agrees as well that a shortage of nurses is expected again when nurses retire and since the economy will be more stable full-time nurses will go back to being part-time (Huston, 2017). “We can’t turn away from a patient’s pain just because it’s difficult” (chapman, 2015, p. 88). I know the path of least resistance is taking a path of ignorance. Easy, is to ignore or neglect the true pain patients experience in times of crisis. As caregivers I believe we all want to heal others or we wouldn’t be in the field of nursing, but there are only handfuls willing to be present during the healing process because sharing one’s pain hurts. As a surgical nurse, I find being genuinely present takes hard work on my behalf, not only mentally but emotionally. On a unit where patient’s needs and conditions are changing at astonishing rates, being present requires mental strength in order slow down enough to recognize the value presence

A charge nurse working the night shift overhears loud talking coming from a nearby room within the unit. Upon locating the room where the noise is coming from, she recognizes that it is a patient with dementia who is becoming increasingly confused, agitated, and combative. The family member at bedside who is also the primary caregiver is trying to keep the patient in bed, and also appears quite frustrated. The primary nurse is in the room, but seems to be struggling with what to do. The charge nurse instructs the primary nurse to review the patient’s medications, and obtain the one used for agitation. The charge nurse then explains to the family member about using the medication, and suggests they take a break while other alternative methods are attempted. The charge nurse then dims some of the lighting within the room, begins to play relaxing music, and purposefully speaks in a very soft tone to the patient. The charge nurse continues to try to redirect the patient, but also understands that you must not argue the reality with dementia patients. The charge nurse proceeds with light massage to the hands, and feet of the patient, all while ensuring the patient that they are safe. The patient is showing less agitation, and the nurse soon arrives with the proper medication. The patient is calm and resting by the time the family member returns. In summary, the paper focused on caring for an infant with bronchiolitis. The student nurse has realised that it is important to gather information from the parents because they know their baby best. Nursing interventions should encourage family members to participate as well as they are the one who care about their child most. In health care, there are many different approaches throughout the field of nursing. When considering the field of family nursing, there are four different approaches to caring for patients.
