This form of writing varies in definition, but nursing defines it as keeping medical records that contain important facts regarding a patient and the development of his or her illness (The Free Dictionary). It is constantly changing with the times converting from paper to a paperless system, which also requires nurse’s advancement in technology. Another key purpose for charting is in case legal actions were to occur, a proper medical chart would make a difference in the entire case. (Charting and Documentation Guide for Nursing Students). APA is used in scientific and social-scientific based disciplines that include nursing. The task of documentation is vital to nursing practice. Many times, however, this documentation is repeated in different areas of a patient’s chart. DiPietro et al. (2008) reported that 40% of the written documentation done by nurses was on personal paper at the patient’s bedside. This had to be copied into the formal patient record at a later time, resulting in double documentation. The reason nurses are forced to use this method of documentation instead of transcribing assessments directly into the chart is that this vital record of the patient’s information is often not readily available. Because several disciplines of the healthcare team require the chart throughout the day, there is no guarantee as to when the nurse may actually have access to it. Additionally, in almost all hospitals that utilize paper charting, the chart must travel with the patient when he or she leaves the floor for testing or procedures. This creates another roadblock to all members of the healthcare tea...

Nurses today need to have excellent communication skills. Receiving information from patients is the most important aspect of nursing. If nurses are unable to communicate with their patient then they would not be able to have a relationship with them. Communication is where two or more people interact with each other, by using sounds and words, this is verbal communication. Some individuals have different behaviors where they try to express themselves to others without using spoken words, this is non verbal communication. Once nurses have the proper communication tools, techniques and evidence based practice, when communicating with their patient, they would get the necessary information needed from the patient to properly diagnosis them.
