Emory University Hospital is a teaching facility that embodies an “organizational culture that encourages critical thinking and acknowledges the inevitability of change” (Rubenfeld & Scheffer, 2015). By embracing a culture of change, Emory strives to fulfill its mission of “serving humanity by improving health.” This mission is being fostered, on my unit in particular, by the implementation of the evidence-based practice of an Accountable Care Unit (ACU). This transformational care model empowers nurses as leaders by giving them a voice and platform to advocate on their patient’s behalf. On my unit, these nurse leaders implement an ACU by offering and collecting information through their interaction with an interdisciplinary team, the patient,
and fellow nurses (Salmela, Eriksson, & Fagerstrom, 2011). Nurses also lead through coaching and mentoring new graduate nurses in a Preceptorship Program. Preceptors refers to individuals who “guide, motivate, sets the norms and standards and delegate power and authority” for the sole purpose of cultivating a new breed of change agents (Salmel et al., 2011, p.430). Lastly, our nurse leaders give support through teamwork and offer advice, which is usually for the purpose of advocating for themselves or the patient (Salmela et al., 2011). Since Emory is an organization whose culture supports change and the implementation of new ideas and practices, I feel my evidence-based plan would be welcomed because it aims to improve the care our patient population receives. To ensure my plan was successful, I would first seek approval from my unit director and then I would set up a meeting with my unit’s charge nurses and medical director. The support of these three entities is the only way to ensure physicians and nurses implement the change process I am introducing.
Hardwiring Excellence gives a general map for creating a culture focused on service, leadership, accountability, and employee and patient satisfaction. While Studer provides firm foundations and ideas, at points the reader is left wanting more in-depth explanation. Overall, Studer’s text emphasizes strategies to capitalize on a hospital’s most positive aspects, and how to motivate employees to use these strategies.
The Ottawa Model of Research Use (OMRU) offers a “comprehensive, interdisciplinary framework of elements that affect the process of health-care knowledge transfer, and is derived from theories of change, from the literature, and from a process of reflection” (Graham & Logan, 2004, p. 93). It promotes research use, and could be used by policymakers and researchers (Logan & Graham, 1998). The OMRU is an example of a planned change theory, which helps “administrators control factors that will influence the likelihood of changes occurring at the organizational level and how these changes occur” (Graham & Logan, 2004, p. 2).
When performing evidence based practice research, the Iowa Model uses a team or individual approach to assist nurses in the journey to quality care. The Iowa Model begins by offering a process of selecting a proper clinical topic, which is often a recurring problematic issue (Polit & Beck, 2012). This topic is formulated as a question to improve a technique or procedure. Once the researcher determines that an ample amount of reported investigation exists on the desired question, information may be gathered and presented for approval (Polit & Beck, 2012). The research may lead to a gradual change in nursing practice.
Vincent nurses, provides a framework for professional nursing practice guided this research. As the business of healthcare is about taking care of people, the model starts with the patient, and their family, as the central focus. Surrounding the patient, are the concepts of mind, body and spirit, which cause us to think holistically regarding the care provided. Finally, the core values/ faith based practices, guide us in managing our patients in a way that is consist with our culture/ values, supportive of our professional growth, encourages the use of best practices, that result in better outcomes, and makes us productive in a way the encourages giving back outside the hospital as well (Stone, 2011).
The nursing profession consist of different workflow designs to provide the appropriate care to our patient population. “Nursing models of care are developed to identify and describe nursing care” (Finkelman, 2016). While providing care for our patients it is important to render the appropriate care delivery model according to the type of patients you are serving within the community. The American Nurses Association (ANA) developed a foundation for which all nurses are expected to perform their basic duties in order to meet the needs of the society we serve. The ANA “has long been instrumental in the development of three foundational documents for professional nursing; its code of ethics, its scope and standards of practice, ands statement of social policy.” (ANA, 2010, p. 87) The ANA defined nursing as “the protection, promotion, and optimization of health and abilities, prevention of illness and injury, alleviation of suffering through the diagnosis and treatment of human response, and advocacy in the care of individuals, families, communities, and populations” and used to create the scope and standards of nursing practice. In closing, as the healthcare climate consistently changes across the United States, it is imperative that registered nurses understand the demand for superior knowledge and lifelong commitment to nursing. As nursing theory and practice expand, our role as leaders is to place a high priority on our commitment to educate and guide nurses, thus improving the lives of their patients through the support of transformational leadership. The Institute of Medicine’s recent report, The Future of Nursing – Leading Change, Advancing Health, specifically calls for nurses to advance and embrace leadership roles on all levels and fronts – from collaborative improvement to positive identification of areas facing challenges within the healthcare setting.
A key characteristic of Advanced Practice Nurses (APN) is to develop and refine the ability to generate and synthesize research and use this information to influence positive changes to everyday nursing practice (Burns & Quatrara, 2013). APNs are well placed as clinical leaders to facilitate for frontline nurses the links between practice and research. Stevens (2013) reminds us that evidence-base practice can result in improved care, positive patient outcomes and enhanced nursing satisfaction. Support of a decentralized organizational philosophy can transform organizations, staff, and patients because it affects the culture, improves staff outlook, promotes personal involvement and encourages staff to reach higher levels of quality care. In my organization, nursing leaders should strive to involve all patient care units and staff in shared governance, educate unit leadership council chairs, and build on the positive aspects by empowering, motivating and developing staff members. These actions will increase creativity, responsibility, intellectual stimulation, and well-being.
As nurses we need to continually expand our skill levels, gain new knowledge and expand our technological and scientific practice potentials. Nurses need to be willing to commit time to research, seeking out evidence based practices and become change agents within their communities, organizations, local, state and federal governments. I believe it is a nurse’s responsibility to inform and educate patients. The role of the nurse is diverse and complex, and is not only concerned with the patient’s body, but also their mind and soul. Nurses are not simply trained to perform tasks, but are trained to be professionals who utilize evidence-based practice with a compassionate and empathetic approach to care. These tasks carry a heavy weight and responsibility, and require that we treat all patients with honor, dignity, and respect.
