Physical resources that help to alleviate DVT or embolism like the Intermittent Pneumatic Compression is very effective. Hydro lift and other mechanical lifts makes it much safer for client and workers alike if used properly. The use of computer is excellent in gathering information and data. According to the American Health Information Management Association, Health information is the data related to a person’s medical history, including symptoms, diagnoses, procedures, and outcomes. Health information records include patient histories, lab results, x-rays, clinical information, and notes. The data can be analyzed to see how a patient’s health might have changed. I took interest in Health Information Management when it was brought to my attention by a doctor. He told me that is a very interesting field and it is in high demand as they have more jobs than people to fill them. I went home, researched it and now here I am making my entry into the field.

Documentation is a form of communication that provides information about the patient and confirms that care was provided to that patient. Some reasons why nurses document is for communication and continuity of care of the patient and by that it means clear, complete and accurate documentation in a health record ensures that all those involved in a client’s care, including the client, have access to information upon which to plan and evaluate their interventions. Next, quality improvement/assurance and risk management through chart audits and performance reviews documentation is used to evaluate quality of services and appropriateness of care.
