As a nursing home model, Green Houses are obviously providers of long-term care services, including basic nursing and medical services. “Shabizim” also receive additional training and higher salaries in a Green House, compared to a traditional nursing home. Although higher salaries help to attract CNAs, researchers have found that CNAs are more likely to stay with an organization that values them and provides them with additional training (Wiener, Squillace, Anderson, & Khatutsky, 2009). From a healthcare management perspective, the GH model which promotes CNA well-being is of interest because happier, better-paid CNAs are less likely to switch jobs and increase employee turnover rates. Therefore, the Green House Project’s healthcare management philosophy could be emulated to reduce nursing home CNA turnover and improve residents’ quality of care. Evidently, the healthcare management discipline has interests that overlap (and can be used to study) the Green House nursing home model. The Green House Project offers unique insights regarding changes in nursing home management philosophies that can improve both, the health of residents, as well as the job satisfaction of CNAs. Since nursing home budgets, residents’ healthcare and CNA turnover rates, are important factors in nursing home management, the Green House Project is an excellent case study for the field of healthcare management, as …show more content…

Unlike a traditional nursing home with a hospital-like feel, Green Houses are described as “de-institutionalized caregiving facilities. They . . . house 12 or fewer residents. They feature private rooms and full bathrooms, allowing for elder privacy and personality (elders decorate their own rooms). Each house has shared family-style communal spaces, including a hearth, dining area, and full kitchen. (2012).
A sense of privacy is an important characteristic of a home-like environment. As Cristofetti, Gennai, and Rodeschini describe it, “The home has always represented and symbolized the passage from the external world to the internal world, from the public sphere to the private one” (2011). The décor a person fills one’s house with makes it their home, as their personality and choices are intertwined with the space (Cristofetti, Gennai, & Rodeschini, 2011). Upon renovating the quaint little house on the hill with my mom, my own feelings toward the house changed dramatically. Before the project took off, I hesitated to step foot inside the building. The odor and dim lighting made it difficult to envision a successful result, but once we finished I was tempted to move in myself. This is the goal. Taking on this second project, I’d do my best to make the house one I’d love to live in while not allowing myself to implement my personal style preferences. The result is a home both move-in ready and open for visitors.
Long term care facilities are for patients looking for 24 hour care, these are sometimes referred to as nursing homes. Providing safety and quality of life with nursing as well as endless supervision. Long term care facilities are held through profit or non profit organizations. Long-term care facilitates are generally classified by ownership: Proprietary (for profit) meaning owned by individual or corporation and run for profit. Religious, meaning owned and operated by a religious organization, lay/charitable meaning owned and operated by a voluntary, non governmental and non religious body. (non profit). When it comes to operating nursing home facilities, there are many stakeholders involved depending on whether the institution is for-profit, non-profit or government owned. Majority of nursing homes are for-profit organization and they account for almost 70 percent of nursing homes, while non-profit nursing homes account for less than 30 percent and less 6 percent are government owned (Nursing home data Compendium, 2015). Nearly 95.5 percent of nursing homes across the state are dually certified, meaning they have both Medicare and Medicaid certifications (Center for Medicare and Medicaid, 2015). Nursing home funding comes from four different sectors. Nearly fifty percent of their revenue come from Medicaid, followed by Medicare which counts for twenty percent, and the rest of payments come from a mixture of private-long-term care insurance and out of pocket (Yoder, 2012).

It is a well-known fact most Americans seniors would prefer to age in their own homes instead of moving into senior living communities. Working in long-term care can be overwhelming. Imagine you are a new graduate nurse putting your new found knowledge and skills to practice for the first time. Your orientation lasted three days which is standard for nurse home orientation compared to hospital orientation that last approximately six to eight weeks for new grads. The shift has just begun and already you have a new admit, new found pressure ulcer to assess, a possible medication reaction, several new orders to take off and eight patients to document on for varying reasons. Feelings of frustration and confusion take over as you are the only nurse on the unit along with a Certified Medication Technician (CMT) and three Certified Nursing Assistants (CNAs) taking care of 47 patients. Ideal nurse-to-patient ratio continues to be a national issue in both the hospital and long-term care setting (LTC). In the LTC setting there is no official nurse-to-patient ratio; there is a suggested staff-to-patient ratio. This issue not only affects the new licensed nurses but the seasoned nurse as well. Recently, there has been controversial debates as to whether heavy workloads are detrimental to patients. The federal, state, and local government regulates many aspects of healthcare. However, it is the physicians, nurses and other healthcare professional that provide care directly to patients. Consequently, does insufficient staffing, heavy workloads, and unsupportive work environment directly contribute to poor patient satisfaction, nurse burnout, high turnover and job dissatisfaction?
The first step to understand your role as a CNA is to know the difference between a hospital setting and a nursing center. Hospitals provide emergency care, surgeries, and laboratory testing. They care for people of all types of ages and all scenarios. Hospital patients have three types of illness: acute, chronic, or terminal. Many people confuse nursing homes with assisted living facilities, but there are several important differences between them. There is a very thin border, which separates the nursing homes from the assisted living facilities. The primary purpose of both of them is to provide medical care and assistance to the residents. However, there is a difference in the level of care provided in each of them, their eligibility criteria, privacy provided, their cost of living, amenities, social activities, and the coverage by the insurance.

Taking care of the individuals that are getting older takes many different needs. Most of these needs cannot be given from the help of a family. This causes the need of having to put your love one into a home and causing for the worry of how they will be treated. It is important for the family and also the soon to be client to feel at home in their new environment. This has been an issue with the care being provided for each individual, which has lead to the need of making sure individuals have their own health care plan.
These facilities are regulated by the state and federal government and these regulations protects the senior residents. For example it is mandatory for the facilities in Texas to provide mandatory services such as daily living activities like dressing, feeding or help prepare meals and cleaning. Depending on the facility license the staff would have to assist with financial management and certain medical services. Even though the federal government developed guidelines the state can make their own as long as it complies with the federal government. Some organizations may accept private pay while others accepted Medicaid. Regulations are developed to protect residence that from being in an unsafe environment. As a result some assisted living and nursing homes are unable to continue services by having fines or closing for an unknown amount of time. Since each state has different set of regulations I will focus on the state regulations in Texas because it is the state I reside in. The organization in Texas that regulates assisted living and nursing homes is the Department of Aging and Disability services(DADS).
The prolonged shortage of skilled nursing personnel has been a serious concern to the healthcare industry, and this shortage has impacted the quality of care delivery. There are diverse entities involved in the implementation and recommendation of these practice changes. These are led by the Robert Wood Johnson Foundation (RWJF), the Institute of Medicine (IOM), nursing campaign for action initiatives, as well as individual state-based action coalitions.
