After reviewing the situation and analyzing, this situation will and has affected my practice as a student nurse integrating into my professional self. Considering my analysis of the key issue, I have realized stress management and a lack of clinical knowledge, affecting judgment and clinical decision-making. My assumptions in the beginning of this situation when I first experience the patient in distress were to immediately call rapid response because they had called earlier on the day. With the knowledge I have gained, it’s important to stay calm in situations to be able to think clearly, analyze the situation appropriately to focus on the patient’s needs. I learned that stress is caused by how an individual perceives it. I perceived this situation as stressful because of my lack of experience and self-efficacy. My preceptor responded to the situation in a calm manner because she was mentally prepared and had a plan of actions if he became symptomatic. I learned that if I better prepare myself in review possible situations that occur and have interventions ready, it would decrease the stress and shock. I really saw how standing there being scared of the patient dying did nothing for my professional growth and impacted the patients safety. It also wasted time, mean while I could have been doing other tasks such as reviewing the medical directives to see if there were further instructions indicated. In the future, I will remind myself of this situation and remember letting emotions get in the way clouds clinical decision making and judgment. The role of nurse practitioner in the Canadian healthcare system is relatively new compared to the traditional roles of doctors and registered nurses, and as with any new role, there are people who oppose the changes and others who appreciate them. In the conflict presented in New Graduate Nurse Orientation, we see that there is a new hire to the floor. Recently graduating from nursing school Helen really wants to fit in with the fellow nurses on the unit. Transitioning from Registered Nurse (RN) to Nurse Practitioner (NP) can bring many challenges. This transition begins when a person starts graduate education and doesn’t end until a few years after gaining the position of NP. Having a proper support system, a job that is interesting, and a keen sense of self-awareness are all important factors to a successful transition. It was intimidating and a bit scary, but instinctively I tried to help the patient and his family in any manner I could. As the day progressed, I had less anxiety when administering medications to the patient, and I felt more at ease with checking on the patient and his family to ensure they had no unmet needs. Nobody is perfect. We all make mistakes. Some of the best lessons in life are learned from making a mistake. But in the healthcare world making mistakes means losing lives. This has started to happen so frequently there has been a term coined – Failure to Rescue or FTR. Failure to rescue is a situation in which a patient was starting to deteriorate and it wasn’t noticed or it wasn’t properly addressed and the patient dies. The idea is that doctors or nurses could’ve had the opportunity to save the life of the patient but because of a variety of reasons, didn’t. This paper discusses the concept of FTR, describes ways to prevent it from happening; especially in relation to strokes or cerebrovascular accidents, and discusses the nursing implications involved in all of these factors.
It was a quiet and pleasant Saturday afternoon when I was doing my rotation at the surgical medical unit at Holy Cross Hospital. It’s time to get blood sugar levels from MM, a COPD patient. The purpose of this essay is to reflect and critically study an incident from a clinical setting whilst using a model of reflection. This will allow me to analyse and make sense of the incident and draw conclusions relating to personal learning outcomes. The significance of critical analysis and critical incidents will briefly be discussed followed by the process of reflection using the chosen model. The incident will then be described and analysed and the people involved introduced. (The names of the people involved have been changed to protect their privacy) and then I will examine issues raised in light of the recent literature relating to the incident. My clinical week was emotional and physically draining this week. I enjoyed being the lead on Thursday because it gave me the opportunity to stop and observe. The nurses and the CNAs were very stressed out, and I clearly saw the effect on the patients. For instance, one of the CNAs asked me to help her with an occupied bed change. I was excited. However, she kept passing a bunch of comments of how hard nursing is and how she did not want to be old. I did not acknowledge any of her comments. During the incident, I was shocked and never realize that this incident would ever happen to me. As all know, the incidence of patient falls will be the huge thing in every health care centre. I am worried if Madam Y experienced any complications, I might not be able to forgive myself. This critical incident made me feel sad, guilty and disappointed in myself. After this incident, I started to blame myself for the fall and this affected my nursing practice until the end of my shift. Before my shift started, I did my research about my new client for week three clinical. I thought I was well prepared for the clinical, I knew the client’s mental and medical conditions but I was more focused on the client’s mental health issues and not the medical illnesses. When the nurse informed me that client W was experiencing shortness of breath due to his COPD, I was a bit shock because I was not expecting that to happen.
Certainly, I began with the goal getting of his vital sign and giving him a bed bath. Fortunately I had practice how to take a vital sign and giving a bed bath before my first clinical at the university skill lab and I had developed this confidence before my clinical tour. The nursing field is one of the hardest fields because it develops a lot of stress for the personnel in this field. That is why becoming a student nurse is too demanding and stressful for the students. The students have to give a lot of them to become a good nurse. While the becoming nurses are trying the best in the school, they start facing a lot of stress because this field is demanding a lot of their time for preparing for the class and their studies. As student of nursing the stress levels start increasing since we know that we are in the program. That is why we need to start managing the stress in different ways. This way can be by setting priorities, asking for help, exercising, focusing on now, changing your perception and between other ones that are important for a great success of becoming a nurse(book). After graduating, and we are an official nurse there are risks that make nurses to be in constant stress.
