Presenting Problem: Shaine is a 17 y/o male who has a chronic and persistent danger to self by demonstrating self-mutilation, with unmanageable behaviors (i.e. anger outburst/ aggression) which he has presented these behaviors greater than 6 months. The symptoms began 2 years ago according to mother and have recently escalated significantly. He has anger outburst, several x/week since 2014, that range from yelling and verbal abuse to aggressive behaviors (i.e. shoving, hitting holes in wall/door, suicidal threats, cutting self and sister with knife). He does not response to parent redirections. He claims not remember the details of his actions an feels no control over them. He isolates frequently in his room and has no friends other than one “girlfriend” he met online with whom he has had no face to face contact. Substance Use Hx: The pt denies drinking alcohol or use of any drugs. He is a nonsmoker. Medical Hx: Pt has a past medial history of ear surgery. He also had two fracture injuries …show more content…
to his right middle and left middle finger. He does not have any NKA or NKDA. He also suffered from a a head injury that required staples. Family composition/ Current Supports He was born and raised in Alaska.
Shaine resides with his biological mother, stepfather, and step sister. He has three sisters and two brothers. His parents got divorced when he was 5 years old. Inside the home the pt does not respond to interventions, discipline or redirection. Medication does not seem to be effective (mother reports he taken it regularly). Per report the mother recent behaviors would include 12/11 the pt was physically and aggressive towards her and the family, where he became belligerent toward her and assaulted her. The pt grabbed a knife and stated he was going to kill himself, than cut himself with the knife (not requiring stitches). On the 12/12 stepfather told him “stop” which cause him become aggressive by throwing an astray at him, which mother was able to calm him down. Yet when she left, he was in a verbal altercation with stepsister that transistion to physical altercation that York Co. sheriffs came out to de-escalate the
situation. Educational He is a 12th grade student at Tabb, his grades are D’s and F’s. In school the pt does not have an IEP or other formal plan. He has been suspended for maladaptive behaviors in school. Police have been called to x2 incidents but not arrest or charges have been made yet. Psychiatric Treatment Hx: The pt received inpatient at Mary View for a week on 4/28/13. He was hospitalized a week at Poplar Springs in 2013, and recently at Riverside Behavioral for a week in November 2015. He received counseling and medication management from Colonial Behavioral Health 05/2013-04/2015 x1 week. He received in home in the summer of 2014 x3 week by an unknown provider. He is currently receiving counseling and medication management at Rocklanding Psychological Group from 04/2015 to present. Shaine has not responded to the medications or treatment modalities that have been attempted. He does not respond to redirection or discipline form parents, and his parents cannot provide the amount of supervision and redirection needed to help Shaine develop consistent coping skills and adptive thinking.

Kathy Nicolo is a White female in her middle thirties living in California. She was recently referred to treatment after finding two people dead after they committed suicide. Kathy called the police and upon their arrival she was visibly distraught and was referred for counseling services. While this event precipitated her connection with services, things were not going well for Kathy for about the past year. She spends a lot of her time sleeping and often does not wake up until the late morning or afternoon. Her affect is typically flat or down and she does not find pleasure in any activities. Jaleel White is a 12 year old boy who is in the Foster Care System. Jaleel went to 3 different homes after his mother was investigated for abuse and neglect. He’s currently with the Price’s and their foster daughter Laura. For an announced visit Jaleel was clean and wearing appropriate clothing. He told everything clearly without hesitation.
