Role of the Neonatal Intensive Care Nurse The role of the nurse in the Neonatal Intensive Care Unit, or NICU, setting is to provide multiple levels of care for infants who experience problems early on after birth or who are born prematurely and require additional medical attention. Many children in the NICU have many different specialized needs. These needs may include, but are not limited to, specialized feedings, intravenous care, oxygenation issues, temperature issues and issues with respiration. The NICU nurse performs the many duties a neonate requires multiple times throughout his or her shift. These duties are executed utilizing a type of care referred to as cluster care. This is defined as specific scheduled hands on times completing a majority of the care the neonate requires at one time so that there are periods of rest in between. The child requires these periods of rest to promote healing, growth and wellness. The NICU nurse not only has to care for a severely sick or premature child but he or she also has many responsibilities. Some of these include educating the parents or primary caregivers on the advanced care that their infant may need, handling psychosocial issues, legal factors, ethical dilemmas and socio-cultural factors. Due to these factors the NICU nurse must also have competent interpersonal skills, knowledge on current legislature and understanding of many different cultures. The proficiency of the skills the NICU nurses possess is very admirable and their knowledge seemed endless. The care that the NICU nurses had for all of their patients was especially heartwarming. They seemed to care so much for each one of their patients and strived to provide the best care possible. The different machines and technological advances I witnessed were some of the most interesting things I have seen to date since I have been in school. I do not know if I could personally do what they do because I had a very difficult time not thinking and worrying about the tiny patients I observed throughout the day once I left the hospital. I left the NICU that day feeling extremely grateful for the health that God has bestowed upon my son and family.

Neonatal nursing is a field of nursing designed especially for both newborns and infants up to 28 days old. The term neonatal comes from neo, "new", and natal, "pertaining to birth or origin”. Neonatal nurses are a vital part of the neonatal care team. These are trained professionals who concentrate on ensuring that the newborn infants under their care are able to survive whatever potential life threatening event they encounter. They treat infants that are born with a variety of life threatening issues that include instances of prematurity, congenital birth defects, surgery related problems, cardiac malformations, severe burns, or acute infection. Neonatal care in hospitals was always done by the nursing staff but it did not officially become a specialized medical field until well into 1960s. This was due to the numerous advancements in both medical care training and related technology that allowed for the improved treatment and survival rate of premature babies. According to the March of Dimes, one of every thirteen babies born in the United States annually suffers from low birth weight. This is a leading cause in 65% of infant deaths. Therefore, nurses play a very important role in providing round the clock care for these infants, those born with birth defects or other life threatening illness. In addition, these nurses also tend to healthy babies while their mothers recover from the birthing process. At Cook Children’s Hospital, NICU parents are not only seen as the parents of the infants, but they also incorporate them as part of the team. Parents are highly encouraged to spend as much time as they possibly can with their premature infants, to have physical contact with them by giving them kangaroo time, which allows parents to have skin-to-skin contact with their infant, as soon as the infant reaches the stage in which he or she is a suitable candidate to be in physical contact with their parents. Siblings who are over 3 are allowed to visit their siblings at the NICU at specific times of the day, and child life specialists help siblings understand what is going on with their baby brother or sister who is in the NICU. Ironically, conflict can provide nurses with an excellent opportunity for developing compassion that will lead nurses unto a place of meeting in which there is a deep respect for differences and equally intentional openness to the possibility of connection. Healthcare practitioners are confronted in a daily basis with the practical manifestation of these issues. In particular, nurses are more confronted by cultural issues than the other healthcare providers because nurses spend majority of their time with patients. However, some nurses are reluctant to confront and discuss the cultural issues because of lack of knowledge in dealing with patients of diverse cultures (Tjale & Villiers, 2004).

Transcultural nursing requires us to care for our patients by providing culturally sensitive care to a broad spectrum of patients. The purpose of this post is to describe cultural baggage, ethnocentrism, cultural imposition, prejudice, discrimination, and cultural congruence. I will also give an example of each term to help you understand the terminology related to nursing care. I will define cultural self-assessment and explain why it is valuable for nurses to understand what their own self-assessment means. Finally, I will describe the five steps to delivering culturally congruent nursing care and how I have applied these concepts to my nursing practice.
As nurses entering the medical field understanding the culture of our patients is crucial to proper care. Each culture has their own set of beliefs and values that are shared among groups of people which influences personality, language, lifestyles, house hold, level modesty, social standings, foods, health treatment and identity. Culture affects how people view health and illness; dictating when, where and what type of medical treatment they will receive and who will be their care provider.
This week’s clinical experience has been unlike any other. I went onto the unit knowing that I needed to be more independent and found myself to be both scared and intimidated. However, having the patients I did made my first mother baby clinical an exciting experience. I was able to create connections between what I saw on the unit and the theory we learned in lectures. In addition, I was able to see tricks other nurses on the unit have when providing care, and where others went wrong. Being aware of this enabled me to see the areas of mother baby nursing I understood and areas I need to further research to become a better nurse.
As a nurse strive to provide culturally sensitive care, they must recognize how their client's and their perceptions are similiar as well as different. Nurse enhance their ability to provide client-centered care by reflecting on how their beliefs and values impact the nurse-patient relationship. To provide appropriate patient care, the nurse must understand her/his culture and that of the nurse profession. Migration from various countries is creating a diverse population with different cultures and languages within the United States. Due to these cultural differences and lack of knowledge, disparities are increasing. Studies have shown that both language barriers and lack of cultural customs can hinder the services provided to the patient by the healthcare worker (Renzaho, Romios, Crock, & Sonderlund, 2013). This study provided a positive outcome when communication and cultural mutual understanding took place and patients had a more positive health outcome. It is very important that nurses are diversified in various cultures in order to better care for our patients. According to Mareno and Hart (2014), cultural competency has become one of the core values being taught in nursing programs. Their study showed that the perceived level of cultural awareness and skills among the nurses provided was low. Awareness and knowledge levels increased with higher education.
