“Always remember to be an internist, my dear.” This was an invaluable piece of feedback I received on a rheumatology rotation in my first year of internal medicine residency. Truly, I have never learned so much about medicine as I did during this first rotation in rheumatology. I became fascinated by the subtle presentations and cases that posed a diagnostic challenge to multiple subspecialties. I have been inspired by rheumatology as a field where multisystem disease is encountered on a daily basis and rheumatologists must use both general medicine and subspecialty skillsets to diagnose and treat patients. During my residency, I had the opportunity to work with the Rheumatology Rehabilitation team at the University of Alberta (U of A). I met passionate mentors in physical and occupational therapy dedicated to helping arthritis patients preserve and optimize joint function. There was one man with rheumatoid arthritis attending the program who said the most important learning point he wanted me to take away was how much his rheumatologist and the rehab team had changed his life. Prior to starting biologic therapy, he was completely debilitated in his day-to-day life and unable to perform simple activities such as picking up his young children. He now found himself not only with disease control, but also quality of life. I value the multidisciplinary approach in rheumatology and emphasis on treating the “whole” patient, rather than a single organ
However, I am not going to spend a long time describing the nitty-gritty of this because there is an elephant in the room. Both of these writings are on a terrible chronic disease affecting millions of people worldwide. What’s worse is that millions and millions more do not even know that this disease exists. I remember when I sprained my ankle while playing baseball, it was so bad that I needed crutches for two weeks and had to keep my foot wrapped for multiple weeks after. The incident took me out for the rest of the season, where my little league team got very close to going into the postseason but fell short. Rheumatoid arthritis is a chronic inflammatory and an autoimmune disease that occurs when the immune system mistakenly attacks the body’s tissue (Rheumatoid arthritis, 2017). This disease affects the entire body, which is called a systemic (means entire body) disease. Arthritis is derived from the word part arthr-, which means “joint,” and -itis, which means “inflammation,” so altogether it means “inflammation of the joints.” It creates inflammation that causes the tissue that lines the inside of joints (synovium) to thicken. About 1.5 million people in the U.S. are affected. It affects all races, but it affects three times as many women than men (What is Rheumatoid Arthritis, n.d.). Overtime, rheumatoid arthritis causes painful swelling that can potentially result in bone erosion or joint deformity, which leads up to physical disabilities. RA can affect more than just your joints, but can spread to body systems, skin, eyes, lungs, heart, blood vessels, e.t.c (Rheumatoid arthritis, 2017).
Ever since I was a little kid, I had always loved going to my father’s office and getting an adjustment. No matter if it hurt or not. On job shadow days, I would go to his office and always ask him what he was doing when he adjust his patients, but what he said was “You are to young to understand.” As I grew up, my father has told me about what he really does and my interest in being a chiropractor has grown dramatically.
The rehabilitation staff welcomed me into their facility and allowed me the opportunity to expand my overall knowledge of injury rehabilitation and patient care. The therapists made sure that I knew exactly what illness was occurring in each patient physically and how their proposed course of treatment would affect the patient in the long run. The best part about the entire experience was the interaction with patients, this allowed me the opportunity to receive first-hand experience in patient care and bedside manner. Prior to this observation opportunity, I failed to realize just how rewarding this profession could be. Being able to work with the patients is a gift and I do believe that this enriching opportunity was highly effective in providing a realistic professional experience.

Rheumatoid Arthritis is when the joints are chronically inflamed, which happens because it is an autoimmune disease which means that the immune system attacks the body tissues. Although Rheumatoid Arthritis mainly affects the joints, it can also affect other organs.

My journey to pursue an education in physical therapy actually originates from my early childhood. I endured a very impoverished upbringing and my family’s socioeconomic status was below the poverty line. At a very young age, money became the central issue of my life. I found myself worrying about mortgages, health bills, and grocery money. As I grew older, I vowed to myself that I would not endure my family’s financial burdens into my adulthood. I knew the best way to accomplish this was by obtaining a college degree and having a professional career. However, I was unsure what career I wanted to pursue. They motivated me into taking the first step into my professional career in where I achieved my bachelor’s degree in Fitness & Human Performance. It was there that I was given the opportunity to do my internship at Proactive Physical Therapy (PPT) and see hands on what to expect in a PT setting. Rheumatoid arthritis is an autoimmune disease that attacks the joints and connective tissue. Autoimmune diseases are illnesses that occur when the bodys tissue is mistakenly attacked by its own immune system. The immunes system is primarly organized to look for infections and destroy them, patients with rheumatoid arthritis have antibodies in their blood which target their own tissue which leads to inflammation. RA is basically a chronic syndrome that is characterized by inflammation of the peripheral joints, but it may also involve the lungs, the heart, the blood vessels and eyes.
I have been exposed to the medical field my whole life. My Mom was a nurse and I would love to listen to the stories she told about her time in the hospital. If anybody asked me at the age of five what I wanted to be when I grew up, I would say “a nurse like my mom.” A few years later I decided I wanted to be a doctor from the few times I got to watch the television show “ER”. The beginning of my interest in physical therapy was when I met my fourth grade pen pal, Elmo, at a nursing home. Elmo had muscular dystrophy and only retained control of his right arm and pinky which allowed him to manouver his wheelchair. During one visit where we got to spend the whole day with our pals, the physical therapist at the nursing home came to see Elmo. I was allowed to stay and I remember thinking it was so cool that there was one person to help Elmo retain the little muscle tone and mobility he had left. In sixth grade, my class went to the hospital to sing Christmas caroles and I remember the coolest place being the physical therapy room. I thought it was amazing how patients were learning to walk again or throw a ball.

My passion and admiration for the healthcare field began during my teenage years, when I was diagnosed with rheumatoid arthritis—a chronic, autoimmune disease affecting various joints throughout my body. As I went through years of treatment, my physician and other medical professionals were instrumental in helping me to maintain a positive attitude after my diagnosis. Their care was the impetus for my future goals, as I saw the genuine goodness that they felt from helping others. As a result of all the medical help and emotional support that I received from these medical providers throughout my youth, I have chosen to pursue a career in the service of others through the public health field.
The internship that I acquired during the summer of 2010 at the Greenwood Sports and Industrial Rehabilitation Center (GSIRC), allowed me to gain an immeasurable amount of experience during my tenure at this facility. Within this period, the exposure to new concepts within the physical therapy industry allowed me to observe and work hands on with patients of all ages and disabilities that exposed me to various treatment plans and programs to restore client’s mobility. The essential Physical Education and Exercise Science (PEES) courses taken at Lander University were beneficial to my academic knowledgebase, which prepared me and developed me for the opportunity within the physical therapy discipline. Working as an intern taught me the responsibility of time management and working within a lean schedule that allow me to further develop my skill set. This reflection paper will illustrate my classroom experience, internship experience, and learning experience.
