Although I believe I am competent in the above specified areas, I am still not confident that I will get sufficient support in the hospital environment in terms of knowledge sharing, moral support and being given constructive feedback. Constructive criticism increases confidence in the work role and reduces stress in an individual (Doody, Tuohy & Deasy, 2012). In my previous professional placements, I have been able to demonstrate my competency to work in the healthcare environment. I have received positive feedback from placement educators, buddy nurses, and patients. Despite being competent in a range of the above areas, NGNs may still face transition shock.
As a medical / surgical RN, I provided care for the elderly, the infirm, the mentally challenged, the young, and the psychologically disturbed. The wide variety of patients exposed me to the effects of life style choices, health care choices, and the resulting impacts to the patient as well as to the family of the patient. This experience has fully matured my view of the awesome responsibility that we, as health care professionals, have been charged with, and it has furthered my desire to obtain the skills necessary to provide more advanced care for my patients. In addition to exposure, maturity and experience, my career as a medical / surgical RN has also sharpened my critical thinking abilities and provided insight on observing signs and symptoms that a patient may be unware of. Furthermore, as a charge nurse I learned the importance of collaborating with other health care professionals in order to provide the highest level of care available. There is a shortage of all health care professions throughout the United States. One shortage in particular that society should be very concerned about is the shortage of Registered Nurses. Registered Nurses make up the single largest healthcare profession in the United States. A registered nurse is a vital healthcare professional that has earned a two or four year degree and has the upper-most responsibility in providing direct patient care and staff management in a hospital or other treatment facilities (Registered Nurse (RN) Degree and Career Overview., 2009). This shortage issue is imperative because RN's affect everyone sometime in their lifetime. Nurses serve groups, families and individuals to foster health and prevent disease.
Calls to outpatient areas such as radiology, rehab, and the hospital lobby are also on the rise, with family members, visitors, and employees being added, besides the inpatients, to the list of eligible Code Rescue calls for the ICU nurse to respond to. With Code Rescues involving a Stroke Alert, the ICU nurse must accompany the patient to the CT Scan area for a STAT CT of the brain, which takes the nurse away from their assigned patients for an even longer period of time based on the status of that patient. When a nurses take their break, another nurse is required to monitor those patients as well as take care of their own patient assignment. The attention given to the other patients is not considered to be extensive, basically “keeping an eye” on them until their nurse returns. Long-Term Acute Care Hospital, specializes in treatment for special needs such as patients with concurrent acute illnesses, recovering from acute illness due to multisystem failures, requires direct care by multiple specialists, ventilator services (weaning or decannulation), needing of daily physician visits or have complex respiratory-relevant diagnoses. Which in terms focuses on patients who have complex medical problems and the rehabilitation needs that one may require special or intensive treatment over an extended period of time. A chronic nursing shortage has been a reoccurring problem that impacts the future of our health care system and, even more so, the future quality of long-term care in this country. Currently, there are several federal and state initiatives, organizations, and agencies dedicated to solutions of the shortage. However, we continue to lack the necessary number of RNs needed to deliver quality care to a population with growing health care demands.

Already, this experience has facilitated numerous valuable encounters during clinical while setting me apart over other candidates who may be less comfortable in the care setting. Indeed, even as a nursing student, my comfort working with patients and members of the care team has allowed me to materially and positively affect patient care on numerous occasions. Additionally, my first degree in Chemistry gave me the tools and analytical nuance to pursue systematic research. Combined with my nursing education and passion for nursing research, these proficiencies uniquely prepare me to be at the forefront of evidence-based family practice nursing. Further, the fast track program will make pursuing a doctoral degree more logistically feasible for me.
Each day we are faced with making decisions regarding the plan of care and discharge of a patient based on the number of days an insurance company allows to treat the patient. Most times the days allowed are less than what is required to assist the patient back to their prior level of function and ability to safely return home. This causes an internal struggle for the provider and can lead to easily accepting what the insurance company allows even though it is not always best for the patient. Typically, we follow the rule of always doing what is right, which could mean that we keep the patient on the unit longer than the insurance will provide payment.

I joined the unit council and I’ve participated in the clinical ladder program at my facility. I believe all of these accomplishments will aid in progressing to the next level. I strive to have several experiences under my belt and I would like to be viewed as a hot commodity within the nursing field so in addition to hospital setting nursing I work part-time at a prison and a nursing home. The extra money helps pay for school and I plan to continue my education ever further. In addition to basic life support I am also certified in advance cardiovascular life support. I believe that I have some qualities of a proficient level nurse already. When caring for my patients I always go with my instinct. There have been numerous times that I’ve assessed patients and I couldn’t put my finger on what was wrong but I utilized my resources for a second opinion. I would have my charge nurse or department based educator step in to assess and give me their opinion as well.
