Outpatient care is the setting the I have the most experience in working in. However, I was still able to learn something new during the panel. I learned about the type of patients that I expect to see in this setting mostly is back pain, shoulder pain, and knee pain. In the next 5 years I see myself working in an outpatient setting. I do not think I will like fast pace evniroment of acute care. I;m also worried about the physical demands of a SNF . Outpaitent care requires the least amount of lifting , compared to toher physical therpay settings. The slower pace evniroment of outpatient care,is the type of evironemnt I enjoy working in the most based on previous experience. The shoulder have been my favorite joint to work on in the time

As a medical / surgical RN, I provided care for the elderly, the infirm, the mentally challenged, the young, and the psychologically disturbed. The wide variety of patients exposed me to the effects of life style choices, health care choices, and the resulting impacts to the patient as well as to the family of the patient. This experience has fully matured my view of the awesome responsibility that we, as health care professionals, have been charged with, and it has furthered my desire to obtain the skills necessary to provide more advanced care for my patients. In addition to exposure, maturity and experience, my career as a medical / surgical RN has also sharpened my critical thinking abilities and provided insight on observing signs and symptoms that a patient may be unware of. Furthermore, as a charge nurse I learned the importance of collaborating with other health care professionals in order to provide the highest level of care available. Long-Term Acute Care Hospital, specializes in treatment for special needs such as patients with concurrent acute illnesses, recovering from acute illness due to multisystem failures, requires direct care by multiple specialists, ventilator services (weaning or decannulation), needing of daily physician visits or have complex respiratory-relevant diagnoses. Working in long-term care can be overwhelming. Imagine you are a new graduate nurse putting your new found knowledge and skills to practice for the first time. Your orientation lasted three days which is standard for nurse home orientation compared to hospital orientation that last approximately six to eight weeks for new grads. The shift has just begun and already you have a new admit, new found pressure ulcer to assess, a possible medication reaction, several new orders to take off and eight patients to document on for varying reasons. Feelings of frustration and confusion take over as you are the only nurse on the unit along with a Certified Medication Technician (CMT) and three Certified Nursing Assistants (CNAs) taking care of 47 patients. Ideal nurse-to-patient ratio continues to be a national issue in both the hospital and long-term care setting (LTC). In the LTC setting there is no official nurse-to-patient ratio; there is a suggested staff-to-patient ratio. This issue not only affects the new licensed nurses but the seasoned nurse as well. Recently, there has been controversial debates as to whether heavy workloads are detrimental to patients. The federal, state, and local government regulates many aspects of healthcare. When I was working as a bedside nurse in the Emergency Department, in one of my duties I was not satisfied with the treatment plan made by a resident doctor for XYZ patient. He entered intravenous KCL (potassium chloride) for the patient. The purpose of that medication and its dose for that patient was not clear to me. I assessed patient history and came to know that a middle aged patient came with the complaint of loose bowel movements, vomiting, and generalized weakness. His GCS (Glasgow comma scale) was 15/15, looked pale but was vitally stable. Over the course of time that myself and the other members of my group spent at Medshare’s National Headquarters and Southeast Region Distribution Center, I gained a valuable experience and broadened my horizons with regards to the international medical aid that the non-profit Medshare provides around the world. The staff and crew that serve at Medshare’s Southeast Region Distribution Center, all have different backgrounds and stories as to how they originally connected to the organization. Some members of the crew included individuals who went to university and received undergraduate degrees in the disciplines of Pre-Med, International Business, and Human Services, while other older members of staff included doctors
The theory used for this case study was effective, because it was able to get P to recognise that change need to occur so the support that she received would be effective. Reflecting on action the outcome of the assessment and intervention that was applied to P’s case I used supervision and to discuss concerns I had regarding this family with my PS. Supervision helped me establish my concerns and areas I required further support in application to theory in evidence based practice ‘blind self’.

I am working as a staff nurse at one of the Integrated Management System (IMS) accredited hospital in Sarawak since 2014. I qualified as a Registered Nurse with a Diploma in Nursing in year 2007. My first year I have been working in a multi-disciplinary ward. We cover a range of specialties including minor and major surgeries. The health care team in my ward consists of 1 Unit Manager, 20 staff nurses and 15 care assistants.
My experience in mental health clinical was very different from any other clinical I had before. In a mental health clinical setting, I am not only treating client’s mental illnesses, I am also treating their medical problems such as COPD, diabetes, chronic renal failure, etc. Therefore, it is important to prepare for the unexpected events. In this mental health clinical, I learned that the importance of checking on my clients and making sure that they are doing fine by performing a quick head-to toes assessment at the beginning of my shift. I had also learned that client’s mental health illness had a huge impact on their current medical illness.
Although students were not allowed in the recovery unit, I was able to talk to one of the recovery nurses. I learned that a nurse’s duty of care includes monitoring the patient’s vital signs and level of consciousness, and maintaining airway patency. Assessing pain and the effectiveness of pain management is also necessary. Once patients are transferred to the surgical ward, the goal is to assist in the recovery process, as well as providing referral details and education on care required when the patient returns home (Hamlin, 2010).
For the past five plus years I have held the position of Nurse Manager of an inpatient Medical and a Rehabilitation unit. Part of my duties include operational oversight of both departments. I have had the good fortune of working alongside of and reporting to some excellent role models and mentors in operation management.
