Reflection is an active process whereby we gain an understanding of how our professional experiences have contributed to our professional development. As a pragmatic learner, I understand the value of reflection as I consider previous experiences to determine the best course of action. From my earlier clinical placements and from feedback on my assessment forms I have identified some areas where my attitude and professional outlooks have matured and changed. My confidence, competence, knowledge, ability to work in a team, my interpersonal skills and attitudes towards patients and their families have developed. Over my four years of clinical placement I have been given greater responsibilities from the radiation therapists leading to more positive experiences.
While on placement in Cork University Hospital in 2nd year, I found that I had difficulties communicating with the patients because of our different accents. I was not confident, therefore when giving appropriate side effect management advice or supporting the patients. My low level of self-confidence arose because of the lack of effective communication and my limited skill level at the time. To develop and improve my confidence and communication skills I became proactive and requested assistance from my clinical assessors. I, also, incorporated all feedback from my assessment forms into my learning objectives. Through reflection, practice, repetition and use of a mentor, I have significantly improved my skills and competency and have shown a willingness to participate in the clinical environment. Through reflection and critical analysis I have been able to identify my strengths and weakness during clinical placements over the years. It has allowed me to begin a process of self-learning that will assist me in my professional and personal development. From my research project and this reflective assignment I have recognised the importance and value of this lifelong learning tool in health care practice as it will help provide a high quality of care and improve professionals’ confidence, decision making and actions.
I will be using the Gibbs, G. (1988) model of reflection to reflect upon for this essay, as this six stage cycle will guide me through the process of description to the action plan, as I find myself to be an active/reflective learner and feel that I can relate to, and learn something that is of value to my practice, and future career and lifelong learning skills through this model of reflection. Throughout this reflective account I will refer to the patient as Mr X, in order to respect confidentiality and maintain his anonymity (NMC) (2008), and local trust policies and guidelines (2009).
Nurses are able to reflect upon their past experiences of work and build and improve this ensuring their level of competence and skills is in line with NMC guidelines. Improving the quality of care provided to patients is an ongoing process and requires practitioners to contently reflect and improve their practice. (Howatson-Jones, 2013) One way in which reflection can improve the quality of care is through the use of professional supervision, as stated by (Daly, Speedy and Jackson, 2014) a focus for supervision should be enhancing a nurse’s skills and ability to reflect on practice. It should reflect on the standard of care provided and highlight areas for improvement such as further training. This, in turn, leads to a greater self-awareness of practitioners’ own abilities. The process is not about finding faults, but to improve and learn to ensure the quality of care is high and professional for all patients. Reflection has its importance in clinical practice; we always seek to be successful and that can be achieved by learning every day of our life through experiences we encounter. In that way we can reconsider and rethink our previous knowledge and add new learning to our knowledge base so as to inform our practice. Learning new skills does not stop upon qualifying; this should become second nature to thinking professionals as they continue their professional development throughout their careers (Jasper, 2006). According to Rolfe et al. (2001), reflection does not merely add to our knowledge, it also challenges the concepts and theories by which we try to make sense of that knowledge. Acquiring knowledge through reflection is modern way of learning from practice that can be traced back at least to the 1930s and the work of John Dewey, an American philosopher and educator who was the instigator of what might be called ''discovery learning'' or learning from experience. He claimed that we learn by doing and that appreciating what results from what we do leads to a process of developing knowledge, the nature and importance of which then we must seek to interpret (Rolfe et al., 2001).
The purpose of this essay is to reflect and critically study an incident from a clinical setting whilst using a model of reflection. This will allow me to analyse and make sense of the incident and draw conclusions relating to personal learning outcomes. The significance of critical analysis and critical incidents will briefly be discussed followed by the process of reflection using the chosen model. The incident will then be described and analysed and the people involved introduced. (The names of the people involved have been changed to protect their privacy) and then I will examine issues raised in light of the recent literature relating to the incident. My essay will include a discussion of communication, interpersonal skills used in the incident, and finally evidence based practice. I will conclude with explaining what I have learned from the experience and how it will change my future actions.
This essay will explore the authors meaning of reflection, how various reflective styles can benefit the practitioners within their settings, and how important reflection is within Early Years Practice. According to Appleby (2010, p.10), reflection is a generic term which describes various behaviours involving feelings, and thinking about thoughts and experiences, in order to examine issues and develop, new understandings and insights. The ability to become reflective in practice has become a necessary skill for health professionals. This is to ensure that health professionals are continuing with their daily learning and improving their practice. Reflective practice plays a big part in healthcare today and is becoming increasingly noticed.
Definitions of reflection vary depending on the discipline of the author. Having reviewed the literature, Bulman (2008, p.2), a nurse, defines it as “reviewing an experience from practice so that it may be described, analysed, evaluated and used to inform and change future practice”. It is a personal process requiring honesty, openness, self-awareness, courage and a willingness to act on criticism. It acknowledges that feelings and emotions influence actions. Critical reflection involves in depth examination and questioning of personal, social, historical, cultural and political assumptions and perspectives that are embedded in actions. It is an active process enabling one to make sense of events, situations and actions that occur in the workplace (Oelofsen, 2012). It transforms a situation in which there is confusion and doubt to one that is clear and coherent (Dewey, 2010).
It may have been something you did differently that had a positive result and you may conclude you would do it again upon reflection. The most important part of reflection is making sense of situations and events that occur so that our decision making and any actions we take lead to good, effective practice. Nurses have to keep a record of their continued professional development. At annual reviews, nurses are able to present evidence of their development through a portfolio which should contain reflective accounts of their practice. These reflective accounts will help them identify strengths and weaknesses, highlight their performance, improve their skills and highlight any areas that could be developed.
A key technique current and nurses in learning can use is reflection based on past experiences. This method of critical thinking enables the user to have an approach of systematic analysis for present and future scenarios with the intent of improvement. In terms of knowing, reflection has appeared to be a crucial part of learning and understanding. (Bonis, 2009).
This essay is going to reflect upon the nursing skills I developed during a period of placement simulations, placing emphasis on oral care, communication with a non-engaging patient and bed bath. It will outline the fundamental aspects of clinical nursing skills that I have begun to acquire. This will also highlight the learning processes which took place and how it helped me to enhance my knowledge, and ethical values in order to deliver quality and safety of care. Using a variety of sources from current literature, I will use a reflective model to discuss how I have achieved the necessary level of learning outcome. By utilising this model I hope to demonstrate my knowledge and understanding in relation to these skills as well as identifying areas with scope for learning.
In this instance, I will reflect on how the incident aided my learning. I will also look at how effectively I educated the patient.

Reflection encourages the student to acknowledge and act upon their strengths and weaknesses in their ability to make clinical decisions (3). As the student reflects it allows them to focus their attention on areas where they have both succeeded and struggled in the clinical setting and apply direct relation to how much knowledge they have gained from the experience (3). Furthermore, whilst it also also allows the student the ability to pose questions, seek clarification of specific events, find meaning and discuss matters of concern and interest with their preceptors in their reflective writing (3). Nonetheless, it is not until the student has achieved the additional knowledge that they will fully understand the limitation of their own practice (3).
Help improved my clinical skills and professional development, made me proactive and thus have learnt that reflection is an important tool for student nurse to improve on their practice.
A reflection log is, in essence, a personalised learning resource. This reflection log will showcase the thinking and learning skills I have developed from module (IS6136 - Professional Practice Skills), which is underpinned by a peer learning system. As part of this peer system, I was encouraged to discuss and share my thoughts, as well as to develop my reflection log in a collaborative way. The reflection log helped me to extend and further my understanding of the material presented. Section 2.0 of this log will discuss five major takeaways from this module, in light of the learning expectation set by the lecturer. Section 3.0 will conclude the reflection log.
