The goal of quality improvement (QI) is to improve patient care, processes, outcomes, and manage risk. Ethical consideration to be addressed in accordance with this QI project involve protection of human rights, beneficence, and justice. Interventions related to this QI project are not experimental, on the contrary, the interventions are recognized evidenced-based standard procedures, therefore it was deemed that informed consent was unnecessary. Confidentiality will be maintained as findings of the improvement project will not be publicly divulged. However, risk of identity exposure exists in particularly with data collection. Data collection involving chart audits reveal patient identity, such as name, account number, and date of birth,
however, charts will be assigned a random number during the chart audit process. In addition, scientific validity is questionable due to lack of valid, reliable instruments for measurement collection. Potential harm to the patient includes time lost related to extended observation, and no improvement in health status. Another risk identified, is related to cost. Implementation of the protocol may require additional staffing, time, education, all of which incur cost. However, the protocol is designed to demonstrate safety, therefore the benefit outweighs the risk.
In her paper emerging model of quality, June Larrabee discusses quality as a construct that includes beneficence, value, prudence and justice (Larrabee, 1996). She speaks of quality and value as integral issues that are intertwined with mutually beneficial outcomes. Her model investigates how the well-being of individuals are affected by perceptions of how services are delivered, along with the distribution of resources based on the decisions that are made (Larrabee, 1996). She speaks of the industrial model of quality and how the cornerstone ideas of that model (that the customer always knows what is best for themselves) does not fit the healthcare model (Larrabee, 1996). The Quality and Safety Education for Nurses (QSEN’s) goal is to prepare future nurses with the knowledge, skills, and attitudes (KSAs) that are needed to continuously improve the quality and safety of the healthcare systems within which they work. QSEN focuses on six main competencies; patient-centered care, teamwork and collaboration, evidence-based practice, quality improvement, safety, and informatics. As we have learned in earlier classes these competencies and their KSAs offer a base to help us and other nurses as we continue our education and become RNs. As we will learn in this class these KSAs go hand in hand with health assessment.
In most aspects of life the saying “less is always more” may ring true; however when it comes to providing quality care to patients, less only creates problems which can lead to a decrease in patient’s quality of life as well as nurse’s satisfaction with their jobs. The massive shortage of nurses throughout the United States has gotten attention from some of the most prestigious schools, news media and political leaders. Nurses are being burnt out from their jobs, they are being overworked and overlooked. New nurses are not being properly trained, and old nurses are on their way to retirement. All the while the rate of patient admissions is on the rise. Nurses are reporting lower satisfaction in their job positions and hospital retention rates are at an all-time low, conversely this is affecting all patients’ quality of care. As stated in the article Addressing The Nurse Shortage To Improve The Quality Of Patient Care “According to an Institute of Medicine report, Nurses are the largest group of health care professionals providing direct patient care in hospitals, and the quality of care for hospital patients is strongly linked to the performance of nursing staff”.

In nursing, the patient is often viewed as the main priority of the nursing staff. The nurse works to provide care for the patient based on the patient 's admitting diagnosis. However, the patient must be looked at as a part of the greater system they exist in such as their family or home environment.
