How to Pull a Tooth I have worked in the dental field for over five years now. During this time, I’ve assisted in several different types of dental procedures. But I have a favorite. My favorite procedure to do is to extract teeth. If you’re like most people, you’re probably cringing at the thought of having a tooth pulled. But believe me, having a tooth pulled in a dentist office is a lot different than your mom doing it with a piece of floss and a doorknob. There are a lot more steps and instruments needed in a dental office to make a terrible experience more comfortable. Going to the dentist is generally uncomfortable for anyone. The first thing you have to do is calm the patient. If the patient is tense, the patient’s nerves could be very tight. Tightened nerves restrict the flow of the anesthetic and make it harder to numb the tooth. The patient tends to not cooperate as well if they are not fully numb. The tenser the patient is, the harder it is to pull a tooth. Explain the procedure to them. Not knowing what to expect is generally the scariest part for most people. Once they have an understanding of what will happen, they should relax a little more. Once the patient is calm, you are ready to numb the tooth. Most dentist offices use a topical anesthetic. Topical anesthetic is like prescription strength Orajel. It helps numb the injection site before getting the actual injection. The topical is placed around the tooth and toward the back of the jaw on the same arch as the tooth being pulled. Topical only takes about a minute to set in. At this point, you are ready to administer the anesthetic through injection. When the injection is done, make sure to rinse the patient’s mouth out thoroughly. Sometimes the patient will swallow a little bit of the anesthetic. This will cause the patients throat to tickle or tingle. This is perfectly normal and may only last a few minutes. The anesthetic should take no longer than three minutes to set in. Before starting to pull the tooth, make sure the patient is completely numb. Their lip should tingle and feel fat. Test the area around the tooth. Take an instrument and push into the gums around the tooth. The Patient should not feel this. If the patient passes these tests, move on. If they are still feeling the pokes, the doctor will give them more anesthetic. The patient is numb and we are ready to begin to use the elevators. In dental terminology, elevators are something completely different than an elevator you would take to the 10th floor. In dental, elevators are instruments that elevate and loosen the roots of a tooth from the jawbone. Elevators look like Philips head screw drivers that’s tip has been rounded out with the sides curved in. Take the elevators and apply pressure on each side of the tooth.
You should be positioned and pressing down between the roots of the tooth and jawbone. You should be able to feel the tooth starting to lift and loosen. When the tooth is loose enough to wiggle back and forth, you can proceed to pull it out with forceps. Forceps are instruments that look like pliers. They have serrated edges that help grip the tooth. There are many different kinds of forceps that are made for specific teeth. Some forceps are meant for upper teeth, some for lower. Some forceps are meant for molars, some for anterior teeth. Once you have the appropriate forceps, hold them with a firm grip and grab ahold of the tooth. Move the forceps back and forth, and from side to side until the tooth wiggles out. Remove the forceps and extracted tooth from the patient’s mouth. Using your thumb and first finger, lightly pinch the jawbone where the tooth once was. This will help to close the hole during the healing process. Also, you should also be able to feel for any tooth or bone fragments that may have broken off during the extraction. Suction any access saliva and blood in the mouth. Take a few pieces of folded up gauze and place them over the extraction site. It will help control the bleeding and help form a blood clot to start
healing.

For some reason even though I've taken a hiatus from assisting, I always come out with a very nice picture. It makes me feel really good knowing that I still got it! The dentist has an overall positive attitude towards patients. every now and then the patient will act very anxious or aggressive and the dentist will take notes and warn me of ways that we handle certain situations sometime she could come off as being a little bit blunt

Growing up I heard many stories from my peers of their experiences at the dentist’s office. It is upsetting to say that most of my visits were vividly terrifying. As a child, the grinding, scratching, and high-pitched twangs of the instruments digging around in my mouth would leave my ears ringing and teeth vibrating every single time. The smell of fluoride and disinfectants permanently infected the air, sending chills through my body the minute I walked in. I quickly learned that latex posses an extremely unappetizing aftertaste.
