People who live with chronic illness like stroke often experience difficulties in their everyday life. Chang & Johnson (2014) describe stroke as a debilitating condition and tremendous personal and financial burden for not the patient and their loved ones, but the community as whole. Providing optimal care to people suffering a disability and chronic condition they are key principles of practice where nurses together with another healthcare team must implemented. These principles include collaborative decision making, empowerment and diversity(Findley,2014).

The assessment task shows us that the patient can become vulnerable and have personality problems when affected by a stroke. As health care workers we have to consider empowering patients so that they gain control of their well- being (Berns, 2016). Patient empowerment is a process designed to facilitate self-directed behaviour change whereby patient choose realistic goals, personally meaningful that involved deeply embedded aspects of patient life (Anderson & Funnell,2010). The goal is to increase patient capacity to make autonomous informed decision regarding their condition (Anderson & Funnell,2010). When nurses collaborate with interdisciplinary health care team, patient, families, carers and community they work together to deliver a comprehensive quality of care (Bell,2014). By building up therapeutic relationship with patient and use of good communication skills brings trust and some positive health outcomes (Doherty,2016).
