Advanced Practice Nurses (APNs) need to see culturally competent patient care as a necessary set of skills that they (APNs) need to attain in order to deliver effective patient-centered care. Patient-centered care (PCC) is a healthcare model focused on actively involving the patient in all aspects of planning, implementation and monitoring of care. It integrates respect for the patient’s needs, values and beliefs into the health care process. Important aspects of PCC are collaborative care, Family-centered care, and comfort. PCC allows the patient to have autonomy and a more collaborative role in making decisions regarding their treatment.
A cultural assessment interview is very important when taking care of patients or their families who may be from a different culture than the nurse’s. In order to be able to better take care of a patient, we first need to know their own interpretation of disease and illness within their cultural context, values, and beliefs. In the book Dancing Skeletons: Life and Death in West Africa by Katherine Dettwyler, the issue of culture and healthcare are greatly prevalent. Katherine Dettwyler herself goes to West Africa as an anthropologist and her horizons are broadened when during her research she comes in contact with how much culture has an impact on healthcare and everyday life.
Cultural Competence is important for many reasons. First, it can help develop culturally sensitive practices which can in turn help reduce barriers that affect treatment in health care settings. Second, it can help build understanding, which is critical in competence, in order wards knowing whom the person recognizes as a health care professional and whom they views as traditional healer, can aid the development of trust and improve the individual’s investment and participation in treatment. Providing culturally competent care is a vital responsibility of a nurse’s role in healthcare. “Culturally competent care means conveying acceptance of the patient’s health beliefs while sharing information, encouraging self-efficiency, and strengthening the patients coping resources” (Giddens, 2013). Competence is achieved through and ongoing process of understanding another culture and learning to accept and respect the differences.
Today, many Americans face the struggle of the daily hustle and bustle, and at times can experience this pressure to rush even in their medical appointments. Conversely, the introduction of “patient-centered care” has been pushed immensely, to ensure that patients and families feel they get the medical attention they are seeking and paying for. Unlike years past, patient centered care places the focus on the patient, as opposed to the physician.1 The Institute of Medicine (IOM) separates patient centered care into eight dimensions, including respect, emotional support, coordination of care, involvement of the family, physical comfort, continuity and transition and access to care.2
Cultural competence can be defined as using the ability of one’s awareness, attitude, knowledge and skill to effectively interact with a patient’s many cultural differences. Madeline Leininger, a pioneer on transcultural nursing describes it this way; “a formal area of study and practice focused on comparative human-care differences and similarities of the beliefs, values and patterned lifeways of cultures to provide culturally congruent, meaningful, and beneficial health care to people” (Barker, 2009, p. 498). The importance of cultural diversity in healthcare allows for the delivery of appropriate cultural autonomy. Showing respect for others will lead to trust between nurse and patient which in turn improves healing and health.

Each culture has their own set of beliefs and values that are shared among groups of people which influences personality, language, lifestyles, house hold, level modesty, social standings, foods, health treatment and identity. Culture affects how people view health and illness; dictating when, where and what type of medical treatment they will receive and who will be their care provider.
Someone once shared a story with me, a story about a little girl, and I would like to share it with you. In this story we will call the little girl Cindy. Cindy was 8 years old, an extremely bright young girl who loved school and loved to dance. Anyone who knew Cindy knew that her favorite thing about school was show and tell on Fridays. One day her friend, who we will call Allison, brought in her favorite pink glitter hat.
As a nurse strive to provide culturally sensitive care, they must recognize how their client's and their perceptions are similiar as well as different. Characteristics can be as diverse as ethnic background, language spoken, gender status, physical appearance, race, and religion to name a few. Migration from various countries is creating a diverse population with different cultures and languages within the United States. Due to these cultural differences and lack of knowledge, disparities are increasing. Studies have shown that both language barriers and lack of cultural customs can hinder the services provided to the patient by the healthcare worker (Renzaho, Romios, Crock, & Sonderlund, 2013). This study provided a positive outcome when communication and cultural mutual understanding took place and patients had a more positive health outcome.
