Abstract
For my second week of my 12-hour clinical, I was in the operating room and I witnessed several surgeries and how the circulation nurse does his/her duties along with how the operating room has certain finesse to it. From when the nurse gets the patient to when the patient gets on the operating table to the anesthesiologist quickly putting the patient to sleep and then the certified surgical technicians and the circulation nurse preps the patient for surgery then how the surgeon and their team conduct the surgery. There are many checks prior to the actual surgery itself; whether the patient has any allergies to any medication, if they have any metal in or on their body that needs to be removed or that the team needs to be aware of, if they know who they are, what surgery they are receiving, and by which doctor. Seeing how quickly and efficiently the anesthesiologist can put the patient to sleep and then intubate them was amazing and you can see the skill that has been mastered. There really are many checks that are done like the surgical instruments that are counted by the surgical techs and then checked off with the nurse to make sure that everything is accounted for and not left inside the patient or lost. Every time the surgeon needed another needle or sponges it was documented every time. The nurse documented the time that the surgeon came into the room, when the first time the knife met skin, when the patient was closed up, and when the patient the room and went to the PACU. The teams that I witnessed really did work very well together and you could see the comradery that they have developed and it was like a whole other family that you get to
This essay describes how the anaesthetic machine and airway management equipment are prepared in operating theatres and discusses how they are ensured safe for use. It evaluates the Association of Anaesthetists of Great Britain and Ireland (AAGBI) guidelines related to safe practice and the preparation of the ET tubes, laryngeal masks, guedels, Naso pharyngeal airways and the laryngoscope. The function of the anaesthetic workstation is to deliver a mixture of anaesthetic agents and gases safely to the patient during the induction process and throughout surgery. In addition, it also provides ventilation to support breathing and monitors the patient’s vital signs to minimise the anaesthetic risks to the patient whilst in the care of health professionals. The pre-use check is vital to patient safety as an inadequate check of the anaesthetic machine or airway management equipment can and does lead to significant harm of the patient including mortality (Medicines and Healthcare Products Regulatory Agency (MHRA), 2008 and Magee, 2012).
So that there no exposed to bacteria or sickness, because there around opened bodies all the time. After they have cleaned the operating room up, they will keep posted on the patient, clean their wounds, clean their rooms, with new pillows new bedsheets, and keeping the patient healthy. The surgical technologist will keep the surgeon in check just in case of emergencies, bleed outs, and in case of the patient failing.

Originally, it was at our hospital that the 1996 well known “Willy King” incident, about the amputation of the “wrong” leg occurred. Safety is a primary concern in the health care environment, but there are still many preventable errors that occur. In fact, a study from ProPublica in 2013 found that between 210,000 and 440,000 patients each year suffer preventable harm in the hospital (Allen, 2013). I followed the RN nurse who was to assist and prep the operating room (OR). She first went into the clean utility room, where she picked up essentials for the surgery. When everything was gathered and prepared, we had to sit and wait for the patient who had arrived late. The RN would check the computer constantly to see if the patient was on file. After the clock hit 9, which was the time for the surgery, the RN nurse decided to go help put the patient on file quicker. When we arrived at the patients room, there was a nurse making the patient fill out papers. The RN nurse took over the papers while the other nurse completed the documents on the computer. While watching all the questions being asked, and the time it took to fill out the paper work, I realized that the paper work process is not easy.
Patient safety one of the driving forces of healthcare. Patient safety is defined as, “ the absence of preventable harm to a patient during the process of healthcare or as the prevention of errors and adverse events caused by the provision of healthcare rather than the patient’s underlying disease process. (Kangasniemi, Vaismoradi, Jasper, &Turunen, 2013)”. It was just as important in the past as it is day. Our healthcare field continues to strive to make improvement toward safer care for patients across the country.
Introduction: The operating theatre is a particular area in the hospital. This special care environment with developed techniques generates several ranges of risks for the patient including the occurrence of infection. Inappropriate staff behaviors and practices can lead to cross contamination in the OR and subsequent surgical site infection (SSI). SSI is a major public health problem.
Keeping patients safe is essential in today’s health care system, but patient safety events that violate that safety are increasing each year. It was only recently, that the focus on patient safety was reinforced by a report prepared by Institute of medicine (IOM) entitled ” To err is human, building a safer health system”(Wakefield & Iliffe,2002).This report found that approx-imately 44,000 to 98,000 deaths occur each year due to medical errors and that the majority was preventable. Deaths due to medical errors exceed deaths due to many other causes such as like HIV infections, breast cancer and even traffic accidents (Wakefield & Iliffe, 2002). After this IOM reports, President Clinton established quality interagency coordination task force with the help of government agencies. These government agencies are responsible for making health pol-icies regarding patient safety to which every HCO must follow (Schulman & Kim, 2000).
Time out was done by the anesthesiologist, the circulating nurse, the surgeon, and the scrub tech all pausing before the surgery and verifying the patient’s name and date of birth, the procedure being done, the site and location on the body in which the procedure was being done, and documented the count of all the equipment the scrub nurse had before surgery to compare to after surgery. 5. The patient’s privacy was protected and respected throughout the whole surgical procedure. The staff was very professional and I felt I learned a lot from them during my OR experience. 6. A sponge count is when the scrub nurse counts the sponges that are unused before the surgery she relays this to the circulating nurse and it is documented. After the surgery the count is redone to make sure that there are no sponges left in the patient. 7. The circulating nurse documents the information and signs the chart in the operating room. From pre-op to the operating room the nurses in pre-op gave off report to the circulating nurse by SBAR. From the operating room to PACU the anesthesiologist went with the patient and handed off the patient’s condition and information to the nurse in there. 8. There were no ethical or legal issues that were raised during my observation in the whole surgical process. 9. I went to the operating room on March 23, 2016 for the Wilkes Community College Nursing Class of 2017 for observation. Another student and I were assigned to this unit from 7:30am-2:00pm. When we got their we changed into the operating room scrubs, placed a bonnet on our heads and placed booties over our shoes. I got to observe three different surgeries, two laparoscopic shoulder surgeries and one ankle surgery. While cleaning the surgical room for the next surgery, I got to communicate with the nurses and surgical team they explained the flow and equipment that was used in the operating room.
Although students were not allowed in the recovery unit, I was able to talk to one of the recovery nurses. I learned that a nurse’s duty of care includes monitoring the patient’s vital signs and level of consciousness, and maintaining airway patency. Assessing pain and the effectiveness of pain management is also necessary. Once patients are transferred to the surgical ward, the goal is to assist in the recovery process, as well as providing referral details and education on care required when the patient returns home (Hamlin, 2010).
