The chapter that I chose for this blog post was chapter nine about mental health. The article I read was "motivational interviewing and social work practice". The article is about a different technique of changing one's behavior. It takes a focus on clients and their goals. The main principles of this idea are expressing empathy, a focus on a client being self-supportive and development of discrepancy. This has been used to help people with a drinking problem and have even been tried on cases of domestic violence victims. Rather than focusing on making a client make a choice this style of interviewing allows for them to go deeper into topics and explore more options. This style overlapped with 7 different theories. Youth homelessness is a global social issue that is rapidly increasing. Shelton & Bond (2017) divulged how accurate data on the prevalence of youth homelessness in the United States are significantly lacking. It could be due to the challenges of collecting data and variance in definitions of what constitutes homelessness for youth. Often times there are multiple levels of oppression evident amongst vulnerable individuals. Homeless transgendered youth are susceptible to intersectionality. As stated in Competency 6, it is imperative for the ethical profession of Social Work, to be engaged and understanding of the multitude of social issues that impact individuals. Thus, Social Workers are expected to be agents of change, and it is an professional responsibility to challenge unjust policies, systems, and ideologies on the behalf of marginalized, oppressed, and vulnerable populations.
The definition of motivational interviewing (MI) has evolved and been refined since the first publications on its use as a way to deal with behaviour change. The technical therapeutic definition of motivational interviewing is a collaborative, and goal oriented method of communication with giving specific observation to the language of change. It is intended to reinforce an individual’s motivation for and development towards a particular objective by evoking and investigating the individual's own arguments for change (Miller & Rollnick, 2012). What is Motivational Interviewing? Motivational interviewing (MI) is a patient-centered method for enhancing intrinsic motivation to change health behavior by exploring and resolving ambivalence. What will be discussed is how can organizations help the patients change negative behavior to a positive behavioral change, diminishing the lack of motivational behavior. (Miller & Rollnick, 2002) states that we have to help clients overcome their ambivalence or lack of motivation toward changing their behavior in positive way. The Motivational Interviewing film was very informative. I was able to get a clear understanding of what is to be expected by the therapist during a session. The film explained the therapist should engage in reflective listening, develop a growing discrepancy, avoid arguing with clients, roll with resistance and support self-advocacy. Miller believed that this approach was far more effective than traditional methods, where the therapist pushed for change. In contrast, Miller explained that motivational interviewing focused on empowerment and helping clients to become motivationally driven to change. Motivational interviewing is a more direct, client-centered approach, and is best used for clients who are showing a great deal of uncertainty about change. The goal of motivational interviewing in human service work is to help resolve the client’s uncertainty about change, encourage change in the client’s life, and motivate the client into change. This is a short term interviewing skill used to gauge where the client is at in the counseling process and where to go forward from there.

Motivational interviewing is based on a client centered approach to therapy that uses open-ended questions, affirmation, reflective listening and summaries to help the client recognize the pros and cons of change and their reasons for resisting change thereby eliminating their ambivalence about change. Once the client deals with their ambivalence the Miller and Rollick believe that the client will be able to make the necessary changes. In addition, motivational interviewing gets the client to argue for change not the counselor. Furthermore, the client not the counselor is responsible for their progress.
Interviewing and research skills are needed within the social work profession. Effective communication skills are one of the most crucial components of a social worker’s job. Every day, social workers must communicate with clients to gain information, convey critical information and make important decisions (Zeiger, 2017). This interview experience was an opportunity to explore the daily challenges and rewards of a licensed social worker. I was excited for the opportunity to interview a social worker in the gerontology sector as this is a specialty I am considering. This meeting allowed me to explore the educational steps of being a social worker, practices of the agency, the clients who are served, and the challenges the agency has.
The Egan Model of counseling can be integrated into one of the principles of psychiatric nursing called motivational interviewing. Egan Model is used to help the counselor use structured and specific skills to assist the client to move forward. Motivational Interviewing “is a client centered, directive therapeutic style to enhance readiness by helping clients explore and resolve ambivalence” (Hettema, 2005, page 91). These models can easily be integrated into each other; there are many ideas and steps that overlap and complement each other.
People inherently have the power to solve their own problems and come to their own solutions. Clients are expected to play and active role in their own change by being open to expressing their problems,creating goals and ultimately evaluating their progress. Clients often use stories to explore their problems in preparation for deciding which goals they want to set and subsequently accomplish. Each client has specific issues and life experiences which the goal should reflect. Clients are expected to put great effort into discovering a desire that the client has deep convictions about and will commit to putting in the work it takes to change behaviors that are no longer working in their life. Captain Rob Geis agreed to be interviewed and share his experiences with the next generation of Social Workers. Native to Ohio, he graduated High School in 1979, he continued his education at Ohio State University from 1982-1986, completing his Masters in Social Work. For the past 24 years, he has served as a Social Worker in the United States Army. In his current position he is responsible for the Department of Social Work Services. Previous positions have included: two assignments as a Division Social Worker, Medical Inpatient Social Worker, Chief of Social Work, Division Chief of Mental Health, Clinical Director of Army Substance Abuse Program, Combat Operation Stress Control Commander, and General Staff Officer (Geis, 2012). Academic positions have included both, the Army Long Term Health Education and Training (LTHET) as well as the Command and General Staff College.
My motivation to be a social worker initially came up by my aunt. Since childhood, I realized that my aunt, a Macanese, was a deaf. Even though our conversation relied on a pen and pieces of paper, we had tight relationship. Unfortunately, my aunt split from his husband spontaneously due to my uncle’s adultery. After that, aunt lost her interest in everything, even making communication with their children. She persistently felt anxious and got hallucination lately. When I was in high school, my aunt was diagnosed as suffering from depression, a seriously one. For quite a long period, my mother stayed at Macau to accompany with my aunt. At that time, I wrote many letters to her on the grounds that I believed words could deliver positive energy. Subsequently, my letters seemed to cut ice with my aunt. She told me that reading my letters could ease her anxiety and enhanced her to stay positive in a day.
I learnt that the use of words or expressions such as ‘that’s brilliant’ can be misinterpreted by some clients to mean that you agree with what they have done. I often use expressions like that in conversations. I am now aware that it can be misunderstood and would like to guard against it. We watched a Youtube video, two social workers called out to a service user flat for an initial assessment. They were refused entry by a friend because he had bad experience with social workers in the past and his child was taken away.
I used rephrasing as a part of understanding the reason for the client’s visit and repeated the information in a way that we both could understand. The client was able to respond through the questions that I asked her in regards to her situation. I want to get a clear understanding without leaving out valuable information. In the three sessions my intentions were to make sure that what she gave me over the phone matches what she says in the interview. I made sure that when my client was speaking, I took notes on the new information or the information that was not covered in any of the previous sessions. Each session was based upon reflection because I had to make sure progress was being made or not. When my client began to discuss things in a way in which I could not understand, I was able to interpret the information in a way that she and I both understood and agreed upon. The moment an initial contact was made, I started giving my client information on informed consent forms, who I was and the number of years of my social work profession, policies and
