1. What might be some of the challenges at each phase of an interview? Mental health assessment is the Interview-based data-gathering procedure used by mental health nurses in establishing baseline records of the patient, in which the actions of the health care team would be based upon (Hungerford, C., Hodgson, D., Clancy, R., Monisse-Redman, M., Bostwick, R., & Jones, T., 2015). According to Evans and Brown (2012), there are 3 phases of the therapeutic relationship- orientation, working, and termination. It stated that the challenges encountered in the first phase of the interview are ensuring self-awareness of nurses, nurse-consumer agreements regarding each of their responsibilities, observing confidentiality, and self-disclosure. Moreover, the issues that may be stumbled upon the working phase are transference of consumers, countertransference of nurses, testing behaviours of the consumers, and promoting independence to patients by preventing themselves in giving unnecessary advice. In the terminal phase of the interview, the consumer must feel the sympathy of the nurse, the assurance of continuity of care, and positive reinforcements should be ensured. 2. They indicated that the use of self is a crucial part of being a mental health nurse for it is the foundation of every effective nursing intervention. Self-consciousness is a nurse’s duty and responsibility for it is the leeway for an effective and less-prejudiced way of interacting to their patients (Happell, Corwin, Roper, Foster, and McMaster, 2008).
