Nicholas T was caught in the bathroom by staff with a syringe trying to insert it in his arm. The syringe was taken from the client was given to the nurse. Both client got verbally aggressive with staff ,causing a scene on the unit , refusing to follow directive and threating staff and other clients. It took staff 20 min to remove the clients off the unit to the lobby. The Delray police was called aid in quelling the angry couples. The clients receive all their belongings upon leaving.
As the EAI team was discussing Molly’s case, one of the ED Residents made a few telephone calls. Molly’s PCP reported that during her last visit about 2 weeks ago, Mollie was alert and able to respond to questions appropriately. He confirmed that Mollie’s daughter and son in law have experienced psychiatric problems, adding that the son in law has expressed anger regarding Mollie’s living arrangements. The home health care agency was contacted. The RN and aide both report they have never met the son in law and have had very limited contact with Mollie’s daughter. When contacted by telephone, the daughter provided no explanation for Mollie’s extensive bruises noted on admission to the hospital. Mrs. Hylton is a 45 year old female who presented to the ED via LEO under IVC by her therapist, Melanie, from ADS. Per documentation Mrs. Hylton denies suicidal ideation and homicidal ideation to nursing staff and MCM before the evaluation. She also contracted for safety with MCM. Dr. Horton requested a mental health assessment on Mrs. Hylton. Before the assessment Ms. Melanie and her supervisor Melissa were contacted. Ms. Melissa reports Mrs. Hylton verbally contracted for safety, however left before ADS could type up terms of verbal agreement. Melissa reports afterwards she was not aware of Mrs. Hylton symptoms of psychosis when speaking with her until being informed by Melanie of findings after conversation with Mrs. Hylton. Melanie upon
Tijanee M became self injurious, without warning she went to her room and placed a blanket over the door jamming the door for entry. Staff immediately attempted to open the door and was unable. Tijanee verbalized continuously she is going to kill herself tonight. The staff was able to gain entry to the Tijanee’s room with the use of the screwdriver. Tijanee was sitting on the floor with a sock tied her neck. Staff was able to provide emotional support and retrieve the sock from Tijanee’s neck. Tijanne began to pick at her recent sores until they were bleeding. Staff used kind gestures and hurdle help to calm Tijanee down. Tijanee was able to exit her room willingly and nursing was called for assessment.

On 06/11/18 at 11:28 AM ET, MM Andrew Aiken reported a non-associate felt SSG Emily Clarke had accused her of shoplifting so she threatened her at 3:45 PM GMT on 6/11/18. Clarke felt the non-associate was trying to place an accessory bag in to her own shopping bag. Clarke told the security guard over the headset that she felt that he was need in the backroom. The daughter of the non-associate overheard what Clarke had said over the headset and told the non-associate. The non-associate, who was upset, that Clarke had accused her of shoplifting, went over to Clarke, and began to curse and yell at Clarke. Upon my arrival, I spoke with Natalie Stovall who stated that her and her boyfriend had gotten into an altercation but he left the residence prior to our arrival. Natalie stated that she did not wish to pursue any charges regarding the assault. I then had dispatch run Natalie’s driver licenses to make certain she was clear from any warrants. Dispatch advised that Natalie had two warrants out of Dallas County.
Shaniah Hutson entered the 1031’s side of the building and entered into the office where both staffs were located. Staff asked Shaniah if she wanted to take her medications this morning. Shaniah replied, “No, I have an eye appointment today.” Staff N.E. responded to Shaniah explaining that the eye doctors should be closed, and we are short staff.
