The nursing discipline embodies a whole range of skills and abilities that are aimed at maximizing one’s wellness by minimizing harm. As one of the most trusted professions, we literally are some’s last hope and last chance to thrive in life; however, in some cases we may be the last person they see on earth. Many individuals dream of slipping away in a peaceful death, but many others leave this world abruptly at unexpected times. I feel that is a crucial part to pay attention to individuals during their most critical and even for some their last moments and that is why I have peaked an interest in the critical care field. Participant 4 stated, “I think just basically being there for the family as well…I think even just a cup of tea can go a long way with any family (McCallum & McConigley, 2013). Another theory that intertwines with Watson’s is Barbara Dossey’s Theory of Integral Nursing. Dossey articulates, “Healing is not predictable, it is not synonymous with curing but the potential for healing is always present even until one’s last breath,” (Parker and Smith, 2015, p. 212). Dossey believes that integral nursing is a comprehensive way to organize different situations in fours perspectives (nurse, health, person and environment) of reality with the nurse as an instrument in the healing process by bringing his or her whole self into a relationship with another whole self. In the HDU, the RN’s interacted with each patient while providing high quality care to create a healing environment for the patient and family even when their prognosis was otherwise. Patient 3 specified that “We still have to provide care...and make the family feel that they are comfortable and looked after” (McCallum & McConigley, 2013). During my career as a registered nurse I have had the privilege of caring for my patients at the bedside and meeting their needs holistically. Additionally, the safety of my patients is one of the most important aspects of my current role. The experience of advocating for my patients during my nursing career has taught me to place my patient’s health and wellbeing first. The second most important aspect of nursing that I have learned during my career is how to meet my patient’s needs as a whole, not just physically but also emotionally and psychologically applying the holistic approach to each patient. Every person’s needs must be recognized, respected, and filled if he or she must attain wholeness. The environment must attuned to that wholeness for healing to occur. Healing must be total or holistic if health must be restored or maintained. And a nurse-patient relationship is the very foundation of nursing (Conway et al 2011; Johnson, 2011). The Theory recognizes a person’s needs above all. It sets up the conducive environment to healing. It addresses and works on the restoration and maintenance of total health rather than only specific parts or aspect of the patient’s body or personality. And these are possible only through a positive healing relationship between the patient and the nurse (Conway et al, Johnson).
Recovery and professional caring both are integrated in everyone’s career as a nurse. As nurses we need to aid individuals in the recovery process, as well as promoting a professional and caring environment for them to strive in. Jean Watsons Theory of Human Caring and the Repper and Perkins recovery model both inter-relate in recovering from an illness. In all three of the recovery models components that are inter-related, authenticity is needed to make the connections with the patients, especially with the model being based on individual adaption and preference (Bennet et al., 2014, p. 39). This is more of a contemporary way of thinking as it more relates to change and growth of the individual (Collier, 2010, p. 17). To be able to aid a patient to make a full recovery, the patient must have the resources and support, with good intentions and authenticity from those who are enabling them, such as nurses.
In the Intensive Care Unit (ICU), patients are being monitored very closely while their vital signs, their neurological status, and their physical status are being managed with strong medications, lifesaving machines, and the clinical knowledge and skills of trained ICU nurses. Outside of the ICU, it is essential for staff nurses to identify the patient that is clinically deteriorating and in need of urgent intervention.
Watson first published her theory of caring in 1979 in a book titled, Nursing: Human Science and Caring. Watson and other researchers have built upon this theory and caring theory should continually be evolving as the delivery of patient care evolves. This theory focuses on care between the nurse and the patient. This interaction is defined as setting mutual tasks, how a spiritual force may help the interaction and when caring in the moment of true healing may occur. “We can’t turn away from a patient’s pain just because it’s difficult” (chapman, 2015, p. 88). I know the path of least resistance is taking a path of ignorance. Easy, is to ignore or neglect the true pain patients experience in times of crisis. As caregivers I believe we all want to heal others or we wouldn’t be in the field of nursing, but there are only handfuls willing to be present during the healing process because sharing one’s pain hurts. As a surgical nurse, I find being genuinely present takes hard work on my behalf, not only mentally but emotionally. Jean Watson is a well-respected American nursing theorist who created the Theory on Human Caring. Watson’s concept on caring for a human being is simple, yet has much depth and meaning, and holds strong for nurses to work with compassion, wisdom, love, and caring. The Theory on Human Caring is necessary for every nurse, as it is our job to care for others in a genuine and sensitive way. The theory is extensive; its core foundation is based on nine concepts all interrelated and primarily focused on a nurse giving a patient care with compassion, wisdom, love, and caring (Watson, J., 1999). The nine essential aspects consist of: values, faith-hope, sensitivity, trust, feelings, decision-making, teaching-learning, environment, and human needs. Watson also created the Caritas Process consists of ten different ways of giving care:
A week before Christmas in 2013, my stepfather suddenly lost consciousness. His body stiffened and he began to violently convulse – he was having a seizure. Later in the hospital, the doctors informed us that my stepdad had suffered a hemorrhagic stroke. The anguish that I felt that night was eased by a nurse who talked to my family about my stepfather’s condition and assured us that the hospital would do everything that they can to assist him with recovery. The next few weeks were filled with uncertainty as my stepdad laid comatose, no knowledge of when or even if he would wake up. During that time, my interactions with the nurses always made me feel safe and comforted Unfortunately, he passed away in January 2014. Though this was a traumatic and life altering experience, what I remember most are the nurses that provided care to my stepfather. They went above and beyond their outlined job duties to care for my loved one and to make my family feel secure even in such a difficult time.

Nurses are both blessed and cursed to be with patients from the very first moments of life until their final breath. With those last breaths, each patient leaves someone behind. How do nurses handle the loss and grief that comes along with patients dying? How do they help the families and loved ones of deceased patients? Each person, no matter their background, must grieve the death of a loved one, but there is no right way to grieve and no two people will have the same reaction to death. It is the duty of nurses to respect the wishes and grieving process of each and every culture; of each and every individual (Verosky, 2006). This paper will address J. William Worden’s four tasks of mourning as well as the nursing implications involved – both when taking care of patients’ families and when coping with the loss of patients themselves.
“Persons intentionally choose to become nurses to help patients meet their health needs,” even when the patient is actively dying. (Wu & Volker, 2012) Hospice nursing and palliative care nursing are both considered end of life care. However, hospice nursing is typically given to patients with a terminal illness and who have less than six months to live. Palliative care is typically given to patients with a life threatening illness, and is used to increase the patient’s quality of life. Choosing a nursing career in either hospice or palliative care can be extremely difficult, but will provide an opportunity for great personal growth. At times, an end-of-life caregiver may feel responsible for their patient’s death, or they may feel isolated due to a lack of support. Nurses new to this field should “feel that their unit acknowledges death as a difficult event and that discussion of death is acceptable in the workplace.” (Lewis, 2013) The most rewarding, and also most difficult, part of being a hospice or palliative care nurse is the ability to be a part of your patient and their family’s life, including their loss, grief, and death. (Wu & Volker, 2012)
The societal taboo associated with death and dying is only worsened when death becomes imminent for an infant or child. Pediatric death and dying is a seldom discussed and often evaded topic in healthcare. This topic, although somber and challenging, is relevant for those nurses who encounter pediatric death and dying first hand. The following discussion will define death and dying in a pediatric population, identify the role of the bedside nurse in support of the dying child and parents of child, the bedside nurse’s role in an interdisciplinary team on a floor where death is a common occurrence, and promotion of nursing self-care to combat compassion fatigue and burnout.
After graduating from Messiah College, I began my nursing career in the intensive care unit. Looking back to my experiences I have seen growth in both my clinical practice and nursing knowledge. On the unit, I have put into practice my critical thinking skills when caring for critically ill patients or dealing with patients who are on the verge of coding or in respiratory distress. My leadership roles on the unit involve my participation in multidisciplinary rounds, being a council representative, a preceptor, and charge nurse. Along with my work on the unit I have continued to enrich my nursing knowledge by staying up to date with evidence-based practice nursing journals.
One must have the full desire to help and care for people in order to have an enjoyable career as a nurse. Nurses are known for the care and compassion shown to the patients and this is comforting for both patients and their families. Caring for a patient can raise their spirits and can inspire a better outlook on their situation. As a nurse, I want to care for all my patients with the hope of improving their health and outlook on their life. I chose Jean Watson because she understands that caring is the core of nursing and believes caring promotes a better health than only performing medical care. She was the first grand theorist that focused on the aspect of caring for the patient while also caring for yourself as the nurse. Watson’s theory
In other words, knowing the person behind the disease and hearing their stories, makes the provision of care more personal. One of the residents was a teacher, another was a soldier, and there was once a politician; all worked for the betterment of others. Similarly, it is our task as nurses to affect a difference in the patient’s and their loved one’s lives. Nurses provide comfort and alleviate suffering, not just dealing with the physiological and psychological pain, but also, more importantly, the emotional torment. Although there is no treatment to stop or reverse the progression of their debilitating disease, knowing that the care I rendered provided relief for the patient, even only for that moment, accorded a bit of gratification.
