The intake form or assessment form is designed to offer the provider with a multidimensional perspective about the new client that helps the provider to gain basic information about the Client’s current life context and presenting problem. The information could include general information about the client, health and mental health history as well as family and socio-economic history, and substance use or legal history. The intake form discussed in this assignment is used by Agor Behavioral Health Services, Inc.

Although this intake form is designed to provide an initial assessment and well-rounded description of the client’s past experiences and present struggles, it can be said that the form contains too many questions. The multitude of questions and checklists could be overwhelming for the client. In addition it could take a long time to complete and might determine the client to have second thoughts about receiving services from the provider.
c. Are the questions reading level appropriate for clients? Please explain the answer.
The majority of questions are reading level appropriate for clients. The questions are short and clear and they provide the choice of either multiple choice, check mark, or fill in the blank. Some terms however, such the ones from the symptom checklist, might be unfamiliar to some clients or they might have difficulties rating

The agency’s website had informational brochures in both English and Spanish which emphasizes the fact that that the agency servers Spanish speaking clients. However, the intake form posted online was available only in English. This indicated the need for intake forms available in the language appropriate for the population that the agency serves, including Spanish and Portuguese.
e.Is there enough space to write what is needed in each answer space? Why or why not?
This intake form provides enough space to write what is needed in most of the answer space. For instance, there is enough space to write the medication name and dosage and all the other sections that require a short answer. However, the sections that require more elaborate explanations, such as circumstances of emancipation from home, special circumstance in childhood, or cultural issues that contribute to current problem would benefit from having more writing space.

Formulation of Problem/Needs: The client 's presenting problems are caused by her mother’s emotional verbal abuse. In spite of all, her emotional problems Ana maintains a positive outlook towards her future. Ana demonstrates self-determination as she clearly expresses her current issues. She struggles with overeating because she feels unloved and worthless. Ana is seeking services to overcome the resentment she feels towards her mother. She is requesting help to manage her coping skills and reduce her feelings of depression. According to Ana these feelings started at a young age. Ana’s current challenges are learning to cope with her mother’s verbal abuse. Anna will arrange monthly meetings with her social worker to talk about what methods she’s used to coping with her depression. The Addition Severity Index is a well-known and widely used tool for use in treating alcoholics and other addicts. It is an approximately 45 to 60 minute long interview comprised of questions about the patient’s life. The interview covers eight subscales focusing on many different parts of a person’s life which helps to provide a comprehensive understanding of their life. The severity is scored on a ten point scale ranging from no problem or treatment indicated to extreme problem, treatment absolutely necessary. The scale helps the interviewer determine the seriousness of a client’s problem and to plan an effective course of treatment. The ASI can also be found in a self-administered paper-and-pencil form and an interactive CD-ROM multimedia version for the computer (Maleka, 2004). This test has been found to be reliable by most but some others do not agree. It is difficult to say whether or not the test is a reliable and valid measure of treatment due to the complexity of the questions. Once a client’s psychosocial needs are identified it is easier to find treatment suitable for that client. There are some problems with the test such as it is not properly designed to cover such a wide population (Maleka, 2004). Other problems include irrelevant questions for alcoholics and other drug users, difficulty remembering relevant information, and lying and exaggerating information for the best interest of the patient (Maleka, 2004). Use of the ASI can be found to be particularly problematic when used with the homeless or double-diagnosis patients. The only limitation to the study is that it failed to mention any costs related to obtaining the services. And it did not mention any demographic variables with respect to race, average age, and levels of income and mental health status (at intake) of the population they served. According to Cummings (2009) at intake, clients suffered a greatly with their activities of daily living and were recently hospitalized for psychiatric issues. It would be unfair to assume that the current study took into account for any of these issues. The study was limited in revealing this pertinent information. Greater knowledge of the patient during intake may provide for greater accuracy in determining the correct agency to direct the prospective client. Other than that, the program appears very thorough in their service delivery.

The intake interview assists in establishing and diagnosing any problems the client may have. The therapist may then explain to the client what to expect during the interview, including the time duration. A good assessment/ or intake will focus on the individual situation, strength and coping mechanism. The intake form is for the client, it gives the therapist more information and an idea of who you are. The intake process that is considered of a series of questions and consent form that the client has to sign and agree to. Olivia as the client, was feeling overwhelmed due to having responsibilities in many separate aspects of her life. Including but not limited to her school, family, friends and new fitness goals. As the counsellor I aimed to help the client come up with a strategy that would not only incorporate all of her responsibilities but allow time to take for herself as well. Doing so, will greatly reduce Olivia’s stress. In order to implement a plan I used multiple techniques to acquire information and insight about the client, as well as her present situation. Throughout the phases of counselling I used the problem solving process, this will help the client manage and save her time. I also exhibited exploring and probing to gain insight about client. Various types of questions were also used including, open-ended questions, close-ended questions and coping questions. I also used the technique of empowering the client to make a change. I accomplished this by highlighting the client’s resources she already obtains.
