Patient safety is a necessary and critical component of quality healthcare. However, medical errors inevitably occur and pose a detriment to patient care. Despite efforts to reduce or eliminate errors in medicine, adverse events attributable to medical errors continue to threaten patient safety. Efforts to improve quality and safety in healthcare have largely ignored the patient’s insight and perspective. The patient’s role in promoting safety, however, is no longer passive but rather a powerful driving force for quality improvement strategies. Patient contributions to the patient safety movement have improved the culture of safe care and lowered the incidence of adverse event rates and outcomes. Hence, patient engagement is a favorable strategy for error reduction and safety measures. With this in mind, this paper will discuss the effect of patient safety issues on quality improvement programs. This paper will also discuss why a focus on patient safety has developed. The concept of EHRs supports improving the safety of healthcare and streamlining its delivery. An EHR is a digital medical record of a patient’s paper chart and allows for continuity of care and collaboration amongst a patient’s continuum of healthcare. According to Manca (2017), EHR technology allows for improved data collection, shared patient information, and treatment history. The shared EHR contains relevant documentation from multiple medical specialties, offering a comprehensive and detailed representation of a patient’s health status. For example, regardless of the setting, providers have specialized, limited interactions with patients that result in differing, fragmented views of the patient’s state of health (Office of the National Coordinator for Health Information Technology [ONC], 2017). EHRs can minimize fragmentation by gathering, arranging, and consolidating a patient’s case history. Additionally, EHRs can diminish the rate of transcription errors. The Institute of Medicine (IOM) reported in 1999 that between 44,000 and 98,000 people die each year in the United States due to a preventable medical error. A report written by the National Quality Forum (NQF) found that over a decade after the IOM report the prevalence of medical errors remains very high (2010). In fact a study done by the Hearst Corporation found that the number of deaths due to medical error and post surgical infections has increased since the IOM first highlighted the problem and recommended actions to reduce the number of events (Dyess, 2009).

Unfortunately, the quality of health care in America is flawed. Information technology (IT) offers the potential to address the industry’s most pressing dilemmas: care fragmentation, medical errors, and rising costs. The leading example of this is the electronic health record (EHR). An EHR, as explained by HealthIT.gov (n.d.), is a digital version of a patient’s paper chart. It includes, but is not limited to, medical history, diagnoses, medications, and treatment plans. The EHR, then, serves as a resource that aids clinicians in decision-making by providing comprehensive patient information.
Patient safety is the basis of quality health care in the hospital. Works applied to patient safety and practices that have not prevented hazard have focused on negative outcomes of care, such as mortality and morbidity. Healthcare employees are important to the surveillance and coordination that will reduce such adverse effects.
The main purpose of EHRs is to mainly exchange health information electronically to help improve quality and safety for patients. Four pros of EHRs is to provide accurate and recent information of the patients, allow for quick access to the patient records, share the health information securely, and make patient records and notes legible. These four points are important and necessary because the goal overall is to improve public health. Patient information should always be updated and current. Health professionals need to easily have access to patient records to either update them or verify the information. Also, health professionals can now avoid any discrepancies with electronic records verses when records were completely on paper.
The purpose of this paper is to identify a quality safety issue. I will summarize the impact that this issue has on health care delivery. In addition, I will identify quality improvement strategies. Finally, I will share a plan to effectively implement this quality improvement strategy.

The goal of patient safety is to prevent harm to patients Mitchell (n.d.). Patient safety in any health system is critical not only for the credibility of the system, but for patient trust and satisfaction as well. Adverse outcomes are defined as any injury or harm resulting from medical care (Watcher, 2008). Adverse outcomes can result in death and disability and cost the health system dearly. Bernard and Encinosa (2004) reported that in the U.S. it costs twice as much to care for patients that experienced adverse outcomes. The Institute of Medicine (IOM) (2000) reported that adverse outcomes cost the U.S. more than 16 billion dollars or 6% of total inpatient costs. Therefore, adverse events are costly both in terms of human life and fiscal resources.
Medical errors can happen in the healthcare system such as hospitals, outpatient clinics, operating rooms, doctor’s offices, pharmacies, patients’ homes and anywhere in the healthcare system where patients are being treated. These errors consist of diagnostic, treatment, medicine, surgical, equipment calibration, and lab report error. Furthermore, communication problems between doctors and patients, miscommunication among healthcare staff and complex health care systems are playing important role in medical errors. We need to look for a solution which starts changes from physicians, nurses, pharmacists, patients, hospitals, and government agencies. In this paper I will discuss how does the problem of medical errors affect our healthcare delivery system? Also how can these medical errors be prevented and reduced?
It is right of a patient to be safe at health care organization. Patient comes to the hospital for the treatment not to get another disease.
