Mobility
The concept of the week was mobility. Mobility is the ability to be moved easily. The concept was knowing how patient’s move from place to place and knowing how to help and support the patient with nursing knowledge. To know how mobile, the patient is, the nurse needs to perform a mobility assessment of the patient and it is also the nurse responsibility to promote and prevent mobility (Crisp, Taylor, Douglas & Rebeiro, 2013).
Ward 4 – General medicine ha a multidisciplinary team that works with different health professionals. They have healthcare workers that help the nursing staffs while the house surgeons and registrars helps the consultant surgeon of the ward (Taranaki District Base Hospital, [TDBH] 2013).

At the multidisciplinary meeting, the nurse will collect and assess the information provided by the other disciplines and family members stating that the patient is not at her prior level of functioning and then analyze the information to develop a diagnosis of deconditioning. Next, the nurse identifies outcomes for the patient to get stronger, achieve prior level of function, have activities of daily living (ADL’s) met in a safe environment by planning for home health, equipment, and 24/7 supervision through family or placement in a facility. This will be implemented by coordinating delivery of a walker and a 3 in 1 chair prior to discharge to daughter’s home with the home health agency nurse, physical therapist, and aide scheduled to start that day. In a week, the nurse evaluates that outcomes are being met by following up with patient, daughter, and home health agency evaluating that the patient is getting stronger, ADL’s are being met, and will soon be able to return to living independently. To achieve these standards of practice, every nurse should be aware of her own nurse practice act to ensure to be functioning with in the laws of the nurse’s state and to ensure the best outcomes and safety of the patients. In health care, safety does not only pertain to the patient, but to all of the staff as well. Although this is important, it is critical that nurses are safe, since they usually interact with the patients the most. If a nurse does not follow correct safety and health practices, they may cause harm to the patient, which may end up in a lawsuit if the damage is bad enough. If safety measures are followed and nothing wrong happens, this saves the facility money and it could possibly gain money if the patients refer the facility to other potential customers. The consideration of human factors acknowledges the capability or inability to perform a precise task while executing multiple functions at once. Human factors provide an organized method to prevent errors and create exceptional efficiency. Careful attention must be exercised in all levels of care such as the physical, social, and external environment. It is also vital to carefully consider the type of work completed and the quality of performance. Patient falls is one of the commonest events within the healthcare facilities that affect the safety of the patients. Preventing falls among patients requires various methods. Virginia Henderson developed the nursing need theory, which focuses on increasing the patient’s independence to speed up the recovery process (Alligood and Tomey, 2009). This is where my theory begins to connect with hers. Our main duty as nurses is to provide care for the patient while they are unable to care for themselves and facilitate them to be the best individual they can be. For this reason nursing is both a science and an art. I believe placing student nurses in the clinical setting is vital in becoming competent nurses. Every experience the student experiences during their placement has an educative nature therefore, it is important for the students to take some time to reflect on these experiences. A specific situation that stood out to me from my clinical experience was that; I didn’t realize I had ignored the patient’s pain until I was later asked by the nurse if the patient was in any pain.
