The family nurse practitioner student is currently completing her third clinical rotation at Family Docs physician’s office that is located in Pembroke Pines, Florida. The focus of the rotation is chronic disease management. Her clinical preceptor is a medical physician, Dr. Jeanne-Elyse Cedeno. The student has been learning from Dr. Cedeno for eight weeks. In comparison to the student’s first and second clinical rotations, the student has seen more patients with complex health issues at this site. The office has some issues with the staff. These issues have decreased work efficiency. The office has two medical assistants. The medical assistants do not know how to take an EKG or blood pressure manually. In many physician’s offices, medical assistants are trained to take an EKG and the physicians or nurse practitioners can interpret the results with patients. It takes time for the family nurse student and preceptor to complete an EKG while there are four or five patients waiting to be seen by the providers. Furthermore, not all patients’ vital signs are completed before the student and the preceptor see a patient. When the lab room containing the digital blood pressure machine is busy, the vital signs are not completed on time. The student discusses the issues with the preceptor who has been aware of the problems. She does not train the medical assistants because one of the assistants does not want to learn, and sometimes she has an attitude when the preceptor tells her to do her job. The other assistant is new to the office. In addition to the staff issues, the electronic medical record system does not integrate with the lab results automatically. When the staff is busy and is unable to manually upload a patient’s lab result, the providers have to log into the lab’s official website to review the lab results. This is time consuming. For objective one, the student had one patient who just started phentermine to lose weight. The patient is generally young and healthy. She is a nurse who works in a hospital at night. She did not eat regularly during work. When she did not work, she liked to eat all the time. The patient requested to take phentermine for weight loss. Phentermine is an amphetamine-like prescription medication to suppress appetite. It helps weight loss by decreasing hunger and makes a patient feel full. The side effects of Phentermine are headache, tachycardia, and restlessness. The student educated the patient about the side effects before the preceptor prescribed Phentermine. When the patient had the first follow-up visit for blood work after taking the medication for a month, she lost 18 pounds. The blood work is to check the patient’s kidney and liver functions, electrolytes, and the functions of blood cells. The patient denies any symptoms of the side effects. Her vital signs were stable. The patient looked energized and happy. For objective two, the student is able to perform either head-to-toe physical exams or problem-focused exams. The student is also able to narrow down differential diagnoses to help the diagnoses based on objective findings and subjective findings.
