I am a Canadian of Irish and Italian descent. I have been exposed to both authentic Italian and Irish cultures and desire to visit both places one day. I am a female and come from a large family. Prior to entering the nursing program, I attended St. Michael’s Catholic elementary school and JL Crowe high school in Trail. My upbringing has immensely impacted the person that I have become today. I find comfort and security in knowing that love and forgiveness, regardless of my mistakes, is never withheld. Not believing in something greater than myself soon leaves me in the darkness and despair of the unknown and I feel like the meaning and purpose of my life is robbed from me. My religion also impacts my nursing practice. I have faith that God hears our cries and prayers. I believe that he does answer them, although it may not always be in the way that we want, expect or understand. If a patient of mine is struggling I can turn to God for assistance in caring for one of his children. If they happen to disclose their beliefs and are open to talking about them, I can walk beside them on their journey with life and death. It reflects trust, intimacy, and responsibility, which are elements essential to any nursing relationship. It is the core of nursing. In the article, “Dimensions of Caring: A Qualitative Analysis of Nurses’ Stories”, an analysis was done on over two hundred stories submitted by nurses around the world, illustrating the point that the practice of nursing encompasses much more than just technical skills. In one of the stories, a nurse stood by the side of a young mother grieving the loss of her baby with nobody else to turn to. This nurse accompanied the 19 year old mother to the cemetery the day after the baby’s death, New Year’s Eve, to bury her baby that had been born at just 22 weeks gestation. She bought her a book on grieving and loss and supported the woman during that difficult time. This nurse did not have to do that—she did not have to go above and beyond for this stranger. But she did. She felt empathy and compassion for this woman and she acted on that. That is what nursing is about, going above and beyond, making emotional investments into the lives of other people. The significance of the actions of this nurse cannot be overstated. Every person’s needs must be recognized, respected, and filled if he or she must attain wholeness. The environment must attuned to that wholeness for healing to occur. Healing must be total or holistic if health must be restored or maintained. And a nurse-patient relationship is the very foundation of nursing (Conway et al 2011; Johnson, 2011). The Theory recognizes a person’s needs above all. It sets up the conducive environment to healing. It addresses and works on the restoration and maintenance of total health rather than only specific parts or aspect of the patient’s body or personality. And these are possible only through a positive healing relationship between the patient and the nurse (Conway et al, Johnson).
In contemporary nursing practice, nurses need to integrate scientific knowledge and nursing theories prior to providing optimal health care. Nursing theories guide nurses to treat clients in a supportive and dignified manner through client centred approaches. However, it is challenge for nurses to practice client centred care in daily realities due to heavy workloads. In order to assist nurses to decrease the gap between ideal and real practice, Registered Nurses Association of Ontario (RNAO) develops Best Practice Guideline of Client-centred-care (Neligan, Grinspun, JonasSimpson, McConnell, Peter, Pilkington, et al., 2002). This guideline offers values and beliefs as foundation of client-centred care, and the core processes of client-centred care can facilitate provision of optimal nursing care. These four core processes of client-centred care include identifying concerns, making decisions, caring and service, and evaluating outcomes. According to RNAO (2006), ongoing dialogue with clients and self-reflection are essential for nurses to develop their nursing skills and knowledge on client-centred care. As a nursing student, I reflected on written transcripts of interactions between patients and me, so that I could gain insights into client-centred care for further improvement. Therefore, the purpose of this paper is to discuss importance of the core processes of client-centred care in nursing practice through identifying and critiquing blocks to conversation. Based on the guideline of RNAO (2006), respect, human dignity, clients are experts for their own lives, responsiveness and universal access will be elaborated in each core process of client-centre care as reflecting on three dialogues with patients.

Within the nursing situation, the shared lived experience of caring enhances personhood (Alligood 2014). Boykin and Schoenhofer’s meaning of caring is being lived out moment to moment; and it is in the intimacy of caring that respect for self and respect for others are the values that confirm personhood (Alligood 2014).
This paper explores the personal philosophy I have as a nursing student and what I intend to convey throughout my nursing career. A philosophy is “an analysis of the grounds and concepts expressing fundamental beliefs (Merriam-Webster’s online dictionary, n.d.). Before entering into any profession it is important to evaluate your personal philosophy about the profession, as it pertains to values and principles in which believe in to guide your practice. The field of nursing is more than treating a physiological ailment, but it involves providing quality care for the individualized needs of each patient, hence being client centered. My philosophy of nursing integrates the importance of knowledge base practice of medicine, combined with addressing holistic needs of the patient and family, including the physical, psychological, cognitive, emotional, spiritual and social care (Scottish Intercollegiate Guidelines Network, 2010). Additionally, a vital aspect of nursing is effective interpersonal relationships with other healthcare professionals to promote quality patient care. Moreover, my philosophy includes the importance the client-nurse relationship to aid in health promotion to prevent illness and increase the level of health of clients.
A transpersonal caring relationship is a where humans are connected with another with regard for the whole person (Watson, 2012, p. 75). Watson (2012, p. 75) further describes transpersonal caring when the nurse can sense the spirit or soul of the one being cared for, feels the condition of the spirit, and then responds in a way that the one being cared for can release subjective feelings and thoughts. With the release of this type of thoughts, more harmonious energy enters the mind through other thoughts and feelings, which places the patient an improved position for inner healing. Transpersonal nursing relationships also express the nurses’ commitment to protecting the dignity, providing holistic care, and encouraging the patient to achieve harmony, which goes beyond the technical aspects of providing patient care.
In 2005 Fawcett stated “the metaparadigm of professional nursing incorporates four concepts: human beings, environment, health, and nursing” (as sited in Kearney, 2012, p. 4). This paper discusses my philosophy of nursing by stating my own personal definitions, values, and assumptions regarding each of the above mentioned concepts. My paper concludes with an exemplar from my own nursing practice and how I integrated my nursing philosophy into that particular clinical situation.
I am grateful to be at an institution of higher learning that understands and respects faith. My philosophy of nursing cannot be explained without God and Jesus. They are a part of each one of us. The reason that someone goes into a healthcare professional is usually to favorably treat a patient’s health. Spirituality of mind and body cannot be separated completely in my nursing philosophy. In this vein, nursing cannot be wholly separated from faith.
The concept of person needs to be explored to go into further depth with the remaining concepts of the metaparadigm of nursing. Person refers to the person undergoing nursing care which includes individuals, families, groups, and communities (MacIntyre & McDonald, 2014, p.63). It is evident that each person may be unique with different biological, psychological, social, and spiritual depth (Thorne, 2010, p.66). Therefore, it is necessary for nurses to realize that each person at the centre of any nursing care will experience different feelings in regards to their body as a whole. The theorist, Parse, defines the concept of person as being “linked to an unfolding process, the relating of value priorities, meaning, and quality of life” (Wu, 2008, p.6). Nurses have our patients trust with their lives every day. These patients have needs that must be understood and met, whether; physical, psychological, or emotional. According to Kristen Swanson’s theory of caring, caring consists of Knowing, Being with, Doing for, Enabling, and maintaining belief. She believes that the environment and what is in the environment can affect people, either positively or negatively. As a nursing student, I possess the qualities of enabling, doing for, and being with. These qualities are implanted in me via my upbringing, culture, religious belief, and life experience/encounters(my environment). “Enabling” is the nurses’ responsibility to help the make a transition into the unknown.
