Elena maintains confidentiality on all reports, and will only share information with the appropriate people during the course of the investigation. Elena is very good at identifying the PS need in a case, because of her background and experience; she can also pick up on underlying issues that may not be present in the initial report. Elena has never had to complete any actions pertaining to in-home services, but she is familiar with the programs that the department offers. She has in this past year, helped many clients complete Medicaid applications and referred them to the assessment team for potential in-home services. If for some reason she cannot locate a participant or make contact with a reporter, she will let me know right away. She is good about sending letters and making the appropriate amount of attempts to locate clients that are hard to reach. Elena continues to show improvement with getting plans of action submitted on time. Elena’s plan of action are very well written, she takes time to plan out in steps how she is going to follow up on a case after her initial visit. Elena’s recordings are very detailed, and it is very easy to understand the steps she has taken in a case. In addition to great detail, Elena always submits recordings in a very prompt manner. Elena still struggles to complete cases within 60 days, on a consistent basis, but has utilized the extension code when appropriate on cases. Elena received a total of 206 cases. Elena had 20 past due cases using the extension code in January, 16 extension code cases, and 7 past due cases in February. In early April Elena completed a PIP and was current on her cases by May until August when she had 4 past due cases, 2 that were PS cases. She had 3 past due cases in October, 2 exception code cases and 10 past due cases in November, and 12 past due cases in December. In the video, Whose Records, the client demonstrated frustration with her current counselor and made a request for her medical records in order to transfer to another counselor. During the conversation, the counselor remained calm and respectful to the client regardless of the clients’ disrespectful approach. I do not agree with the challenge approach made by the counselor regarding the client seeing a different counselor every three to four weeks. That seemed to increase the level of tension on behalf of the client. I feel a better approach would have been to allow the client to express her concerns regarding her current treatment. After which, the counselor could explain the process of obtaining her medical records along with requesting the client to sign the appropriate release of information to acquire her records.
The family is scared as to what might happen with the heart surgery. Justine’s mother is also afraid that her family might blame her for going ahead with the heart surgery if anything bad were to happen to Justine in the hospital during the surgery. The family is hoping that the healing or the praying ceremony scheduled at the temple might work and cure Justine, and hopefully surgery might not be needed after all.
I will be evaluating the case of Angela and Adam. Angela is a white 17 year old female and Adam is her son who is 11 months old (Broderick, P., & Blewitt, P., 2015). According to Broderick, P., & Blewitt, P., (2015) Angela and her baby live with her mother, Sarah, in a small rental house in a semirural community in the Midwest. Adam’s father, Wayne, is estranged from the family due to Sarah refusing to allow him in the house however, Angela continues to see him without her mother’s permission which is very upsetting for Sarah. Angela dropped out of high school and struggles raising her son (Broderick, P., & Blewitt, P., 2015). The primary goal of case management is to deliver quality care to patients in the most cost effective approach by managing human and material resources. The focus of this paper is on the concept of case management and how it developed historically, the definition of case management, the components of case management, and how it relates to other nursing care delivery models.
