On 11/18/2016 at approximately 2231 hours, I was notified by Corporal L. Rivera #143648 of a traffic crash with injuries on Sligh Avenue East, east of 56th Street North, involving a Hillsborough County Sheriff's Office unmarked Dodge Charger patrol vehicle and a Honda Accord. Upon arrival, I observed the vehicles at final rest in the roadway with the Honda on its roof and the Dodge facing to the east in painted center median. The driver and passenger of the Honda were transported to Tampa General Hospital at the request of the driver for precautionary reasons. The driver and passenger of the Dodge were on scene and had no injuries.
I observed the Honda at final rest in the westbound lane, facing in the east direction. The vehicle rolled a half turn and landing on the roof. The damage to the vehicle was concentrated to the front passenger corner, the driver side and the roof. I observed multiple other areas that had sustained older damage from previous impacts. I observed the front driver and passenger airbags deployed and the seat belts unlocked and in the retracted position. I observed no blood on the deployed airbags in the Honda or any other obvious areas. I observed the headlight switch to be in the on position and the headlights were operational during the investigation.

On the above listed date and time, deputies arrived at Hampton County Medical Center, and made contact with the complainant regarding the above listed incident type. Deputies gathered the pertinent information needed to complete this report. The complainant and his mother wrote a voluntary statement. Photos were taken while at the Emergency Room. The complainant was issued a Victim’s Right’s Form along with a case number attached. Deputies attempted to make contact with the subject, no contact was made. Deputies cleared the call and resumed normal patrol duties.
On 01-01-17 at 0023 hours I was monitoring the radio and heard that Officer Harrell #3441 and Officer Thebeau #8402 were involved in a vehicle pursuit in the area of Fair Oaks Avenue and Corson Street. I responded to the above location to assist. They advised responding units that the suspect was involved in a traffic collision on the eastbound 210 Freeway Fair Oaks Avenue off-ramp.
On May 22, 1990 two Sacramento County sheriff’s deputies were responding to a call to break up a fight. While returning to their cars one of the officers, Murray Stapp noticed a motorcycle moving at rather high speeds. The motorcycle was being operated by 18-year-old Brian Willard and carrying a 16-year-old passenger by the name of Phillip Lewis. Neither the operator nor the passenger of the motorcycle had anything to do with the fight being responded to.
This incident occurred while the vehicle was parked in the west parking lot located at 120 W. Bellevue Drive.
At about 1940 HRS I arrived at Deputy Melchers location and made contact with Deputy Melcher and Gary Jacobs the Reporting Person/ Victim. During a high speed pursuit at speeds of over 100 mph involving an officer and the driver of the sports car the officer lost control and hit and killed a pedestrian on the sidewalk. When this occurred the sports cars driver turned to look back, crashed the sports car and was killed.

In the month of October 2011, two of my friends were exiting Fort Carson, Colorado, and waited at a red light. The light turned to a green arrow and they proceeded to the intersection. The driver did not see a large dump truck travelling approximately 50 miles per hour toward them until it was too late. The dump truck failed to adhere to a red light and struck my friend’s truck. The truck spun around several times and stopped in the median.
