The activities and events that I participated in during this INT D 410 course in the last year have been a dramatic eye opening and humbling experience. Process Excellence in the emergency department is a team collaboration that has a focus of interest for improving quality of care for patients. Team collaboration in health care is recognized as a group of health care workers from different disciplines working together on a common goal. This particular “multidisciplinary” (Finkelman, 2012, p. 336) team meeting was a collaboration of team members that included: the Emergency Room (ER) Director, ER physicians, and ER nurses, ER Head Health Unit Coordinator, ER Business Manager, Senior Process Excellence Coordinator, Director of Information Management, and the Senior Marketing Specialist. This team’s purpose aims to organize a team approach to care for patients treated in the emergency department and focuses on the care approach that provides continuity of care to patients. This focus on the patient is aimed to provide not only a higher level of patient satisfaction, but also to improve professional satisfaction by developing approach by emergency room staff to provide care as team collaboration. This process excellence team has been meeting for over two years in hopes of this goal being reached. This paper aims to help the reader gain a better understanding of this specific team collaboration, the roles of its members, and the communication methods utilized.

The demands on health care providers to provide the best quality care for patients is increasing. With added responsibilities and demands on our health care workers, it is hard not become overwhelmed and forget the reason and purpose of our profession. However, there is a way where all professionals can meet and come together for a common cause, which is the patient. A new approach in patient care is coming of age. This approach allows all health care professionals to collaborate and explore the roles of other professions in the hope of creating a successful health care team. However there is still a massive gap in multi-agency work for improvement which needs to be looked within a few areas. As we can still see that each agency involved are not receiving the correct training to be able to work well and integrate with other professionals to create an overall better service and care. We don’t know how the training is received and the true outcomes of it and how it will cooperate with other professionals.
At the beginning of the semester, I did not know what the "Service Learning and Social work Practice" course mean. However, through the process, with the accompaniment of Professor Preeti Charania, I understood the course and noticed a lot of transformation in me through the semester. The following are some of the changes and growth I have seen in me from class readings, discussions, group works, and working with the Church World Service (CWS) agency.

It was unquestionably the exemplary teamwork and communication, that were so fundamental in providing the holistic care that Mrs X needed. The responsibility and roles of the multi-disciplinary team were varied and often overlapped within the theatre suite. The team members had differing and varying levels of experience and expertise, but combined these when working together to care for Mrs X.
This project used a four person team to help manage the care of veterans on a more intimate level, such as making phone calls to their homes to find out how their day to day health is being maintained and if they have any current needs that the health care provider can help them obtain. As a result of reading their case study, I believe that having a healthcare group for each individual within a practice can also help address health care concerns of today. According to Porter and Lee (2013), this type of care would be called an integrated practice unit (IPU). In an IPU, a dedicated team made up of both clinical and nonclinical personnel provides the full care cycle for the patient’s condition. IPUs treat not only a disease but also the related conditions, complications, and circumstances that commonly occur along with it. IPUs not only provide treatment but also assume responsibility for engaging patients and their families in care by providing education and counseling, encouraging adherence to treatment and prevention protocols, and supporting needed behavioral change (p.11). By utilizing this type of group, it can provide better care for patients. This group would be made up of a medical doctor who will be the primary care physician and head of the group. A nurse would help follow up with the patient and continuing to help
The practice of using inter-professional teams in delivering care is not a new concept but current health policy requires professionals work within a multidisciplinary team Department of Health (2001) and entrenched in the Nursing and Midwifery Council (2008) Code. The principle focus of this essay is to discuss the importance of inter-professional collaboration in delivering effective health care and what challenges and constraints exist. The integration of a case study will give an insight into inter-professional collaboration in practice.
Interprofessional collaboration has been shown to increase access to healthcare, improve outcomes for patients with chronic disease, reduce medical errors, reduce tension and conflict among caregivers, improve the use of clinical resources, and lower rates of staff turnover (Lemieux-Charles & McGuire, 2006). IPE is a means to promote collaboration between health care professions. In a study designed to assess the effectiveness of IPE interventions compared to separate, profession-specific education interventions, 4 of 6 studies found that IPE improved patient satisfaction, collaborative team behaviour, and decreased clinical error in emergency departments (Reeves et al., 2008).

The purpose of the Inter-professional Event was to develop team skills among various health professionals. Thus, the three main concepts that I found was useful in the session were; first is the teamwork because by working as a team in healthcare will help us keep our patient safe; the second is communication because it will help us to minimize our mistakes; and the third is pre-briefing because it will keep the team members on a same page so that everyone will have the same goals. 2. Observing the teacher/student interaction, the students are highly eager to get involved in discussion. This shows that they understand and appreciate the material that is given to them. This caused me to reflect back on my sophmore year in English we didn 't have much to read that was actually enjoyable. Sure we talked about ethnicity but we didn 't get to embrace the meaning of diversity as much as he does in this class. He has two groups, the class as a whole, and the four students whom make up the honors group. I got to spend time with the honors group and you can tell the different roles and the different qualities that students have that compliment each other.
