Due to America’s immigration patterns as of roughly the 1990s, there has been a spike in diverse population residing in the United States. In order to deliver professional standards of care, cultural competence is important. Why? Clients could file claims for health care provider’s failure to successfully diagnose and treat a certain disease or symptom due to the provider’s lack of knowledge of the patient’s health belief, cultural beliefs, traditions part of religion…etc. Due to the rise in immigration, we have such disparities in the country when it comes to health care services, if health care practitioners can treat every patient or client in the most cultural competent manner then that is when we, as a nation, can reduce this disparity.
Douglas, Rosenkoetter, Pacquiao, Callister, Hattar-Pollara, Lauderdale, Milstead, Nardi, & Purnell (2014) outline ten guidelines for implementing culturally competent care; knowledge of cultures, education and training in culturally competent care, critical reflection, cross-cultural communication, culturally competent practice, cultural competence in health care systems and organizations, patient advocacy and empowerment, multicultural workforce, cross-cultural leadership, and evidence-based practice and research. One specific suggestion I will incorporate is to engage in critical reflection. This is mentioned both by Douglas, et al. (2014) and Trentham, et al. (2007) as an important part of cultural competency. I will do this by looking at my own culture, beliefs, and values and examining how they affect my actions. I will use this information to better inform my day to day practice when working with patients with a different culture than my

The United States’ population is currently rising exponentially and with growth comes demographic shifts. Some of the demographics shifts include the population growth of Hispanics, increase in senior citizens especially minority elderly, increase in number of residents who do not speak English, increase in foreign-born residents, population trends of people from different sexual orientation, and trends of people with disabilities (Perez & Luquis, 2009). As a public health practitioner, the only way to effectively eliminate health disparities among Americans, one must explore and embrace the demographic shifts of the United States population because differences exist among ethnic groups (Perez, 2009). We must be cognizant of the adverse health conditions for each population and the types of socioeconomic factors that affect them. Culture helps shape an individual’s health related beliefs, values, and behaviors. It is more than ethnicity and race; culture involves economic, political, religious, psychological, and biological aspects (Kleinman & Benson, 2006). All of these conditions take on an emotional tone and moral meaning for participants (Kleinman & Benson, 2006). Going to a different country or area of the world can open up anybody’s eyes to see that culture makes a huge impact on the understanding and practices of healthcare that seem to be so common to other areas of the world. When a person lives in one country their whole life, that person may not realize how different the life they live is from someone in a foreign country. If a person is going to receive treatment from someone with a different cultural background, they should be expected to get treatment to respects their own culture. Massachusetts College of Pharmacy and Health Sciences having such a diverse variety of students has their own cultural competency definition that states “effectively and comfortably communicate across cultures with patients of differing backgrounds, taking into account aspects of trust in order to adopt mutually acceptable objectives and measures”. In the book Dancing Skeletons: Life and Death in West Africa by Katherine Dettwyler, the issue of culture and healthcare are greatly prevalent. Katherine Dettwyler herself goes to West Africa as an anthropologist and her horizons are broadened when during her research she comes in contact with how much culture has an impact on healthcare and everyday life.
Cultural Competence is important for many reasons. First, it can help develop culturally sensitive practices which can in turn help reduce barriers that affect treatment in health care settings. Second, it can help build understanding, which is critical in competence, in order wards knowing whom the person recognizes as a health care professional and whom they views as traditional healer, can aid the development of trust and improve the individual’s investment and participation in treatment. Minnesota is a state with a rich, diverse, and evolving cultural landscape. In the past two decades, Minnesota has seen a significant increase in the number of children and families from culturally and linguistically diverse communities. Each of these communities is unique in their culture, values, and perceptions of child development and disabilities. A growing body of research reveals that significant health disparities exist across racial groups in early screening, identification, and diagnosis of developmental delays (CDC, 2014; Mandell et al, 2002; Shattuck et al, 2009; Zuckerman et al., 2014).
Cultural competence can be defined as using the ability of one’s awareness, attitude, knowledge and skill to effectively interact with a patient’s many cultural differences. Madeline Leininger, a pioneer on transcultural nursing describes it this way; “a formal area of study and practice focused on comparative human-care differences and similarities of the beliefs, values and patterned lifeways of cultures to provide culturally congruent, meaningful, and beneficial health care to people” (Barker, 2009, p. 498). The importance of cultural diversity in healthcare allows for the delivery of appropriate cultural autonomy. Showing respect for others will lead to trust between nurse and patient which in turn improves healing and health.

Racial and ethnic disparities in behavior health care are not a new phenomenon. According to researchers’ disparities in health services use and outcomes have contributed to difference in access to care. Evidence showed that compare to the majority, African American and the Latino they have the lowest income, less education, lower rates of private insurance coverage. There are possibilities that explain this phenomenon: racial bias on the part of the door, patient preferences, and poor communication.
There are times when our values conflict with our professional roles. Religious values, cultural, social values might affect the way service is being rendered in the health care industry. There are challenging events happening every single day in the daily activity setting of nursing. I value respecting one another. I believe in diplomacy than violence. I place my faith at the highest level. Like most of us do, I have found it hard in my life to make peace with people who disrespect my dignity and people who lack respect for their wives and children’s. I believe, assisting in ending life, facing verbally abusive behavior, aggression, violence from a patient or circumstances involving these, and prejudiced patients would be challenging for me from a variety of value standpoint.
As a nurse strive to provide culturally sensitive care, they must recognize how their client's and their perceptions are similiar as well as different. Nurse enhance their ability to provide client-centered care by reflecting on how their beliefs and values impact the nurse-patient relationship. To provide appropriate patient care, the nurse must understand her/his culture and that of the nurse profession. Cultural biases can be particularly difficult to identify when the nurse and client are of a similar cultural backgroup. When we recognize and know a culture, we will know what is right for our patient, and thus may impose our own values on the client by assuming our values are their values. Recognizing differences a present an opportunity not only to know the other, but also to help gain a greater sense of self.
