1) For this activity, I have chosen to create concepts maps to meet my learning goal because according to Bes Piá, Blasco-Tamari, and Muñoz-Portero (2011), an important factor that enhances learning is using previous knowledge and linking it to new knowledge. Concept maps are a good learning tool that allows an individual to identify relationships between ideas and or concepts, promotes reflection, ties in previous knowledge, analysis, and creativity (Bes Piá et al., 2011). As a nursing student who is in fourth year, with much yet to learn, I believe that having visual and written representations and practical tools would benefit myself and many others. With not many opportunities to do such, I have decided to incorporate what has worked for me in the past into my learning plan activity. Pilcher (2011) explains that using a concept map can allow one to view the different aspects of care such as assessments, interventions, and outcomes. Also, it allows to identify gaps in knowledge. While creating my learning plan, I was able to acknowledge what I did not know in risk factors, interventions, causes, and different categories of the diagnoses. As a visual learner, creating concept maps enhances my understanding, and memory as I am able to not only read the differences and similarities among the diagnoses, prevention, treatment, risk factors, but also able visualize it. For this reason, I am able to retain the information in a much more effective manner, within a shorter period. This activity has contributed to my ongoing competence as I have increased my understanding of the diagnoses that are prevalent within my placement setting, as well as improved assessment skills in cognitively impaired residents. 3) Throughout my placement this semester, I have encountered many residents on my unit whom I am able to assess through observation and interactions over the span of the semester.
My activity has met my sub goal through increasing my ability to assess risk factors for diagnoses, recognize signs and symptoms of residents, and be able to identify the interventions for the diagnoses in my practice. The activity links to my overarching goal as I have mentioned that I wanted to promote positive outcomes in residents with dementia, by focusing on having a greater insight into the diagnoses, assessments, and interventions of dementia, delirium, and
depression.
Due to an ageing population , The Francis Report recommends the introduction of a new status of nurse, the “registered older persons nurse”. One of the illnesses linked in with this is dementia, and multiple factors relating to dementia are having an impact on how nurses are trained and their deliverance of services. In 2013, the Royal College of Nursing (RCN) began a new development program to transform dementia care for hospitals. There aim is to develop skills and knowledge related to dementia, the roles of all those who are involved, understanding the development of action plans that identify key changes.
I worked with Dementia and Alzheimer patients as a Certified Nursing Assistant for almost three years. Working with the elderly has been one of my greatest achievements. I assisted my residents with bathing, grooming and making them feel comfortable. I was able to create a favorable environment for my residents while working with them. I had the opportunity to see patients go from early stage to their last stage of dementia. This gave me an opportunity to want to do more for people who are in need of my care. The College of Nurses of Ontario (CNO) is the governing body of all registered nurses in Ontario and is regulated. The CNO provides expectations and guidelines to follow, which need to be met by each Registered Practical Nurse (RPN) individually. As a nursing student, I am taught about the CNO and the importance of referring back to the guidelines while caring for patients. While gaining experiencing in the nursing field through my clinical settings, I have realized as a nursing student there are areas I need further development in. Relate personal learning style to preparation for the NCLEX-RN: Critical thinking and being able to work together with the healthcare team is a key to be a successful nurse and providing the best care to the patients. A nurse should understand the disease process to fully understand how to care and organize a plan to care for assigned patient. This concept will also help me learn and understand better and eventually apply this knowledge toward NCLEX as well. My goal is to exhibit the use of my critical thinking skills throughout assessing, diagnosing, planning, implementing and evaluating nursing care (ADPIE). Much of the nursing school courses specially medical- Surgery centered around evidenced based which now I can see how to relate them into capstone course and eventually helping me to pass NCLEX.

Dementia is a significant health issue in Australia (Australian Institute of Health and Welfare 2012) (AIHW 2012). Whilst Dementia primarily affects older members of the community, it can also affect young people and has a significant influence on overall health and quality of life (AIHW 2012). The type of Dementia is a determinant in the severity and development of symptoms in individuals (Department of Health 2013) (DoH, 2013). The gradual, progressive and irreversible nature of Dementia has a considerable social and physical impact not only on the individual, but also on family and friends.
Dementia is common among a large population of elderly people. The disease affects not only the individual diagnosed, but also the caregivers that work towards making their life comfortable in the end. Understanding and learning about the disease is crucial in helping those that experience or live with someone who has dementia. The services and support that are currently in affect for elderly people with dementia and the caregivers is poor, and ineffective because of the lack of research and information on the topic.

A concept is a general idea, plan or understanding of something. The concept of adaptation is something that affects every living being, from a one celled organism all the way up to human beings. We all must adjust and acclimate in order to survive. I will explore what adaptation is, as well as how it applies to our nursing careers. Aim is an important part of the teaching process because it will assist nurses in designing the teaching plan content and will help the patient a clearer understanding of what they will expect to learn after the teaching session (Jame, 2015). To be able to become effective in educating the patient, nurses should have an understanding the characteristics of their patient.
