The client is a 16-year-old female that presents as talkative, insightful, and intelligent. Client is involved in dance, gymnastics, and plays the piano. Client is the third oldest of four siblings. Client identified that she has strong faith and a connection with her Spanish and Turkish cultures. Client was five when her parents divorced and they now share custody. Client was referred to services by her mother after the client’s recent (3 weeks ago) discharge from an in-patient facility for teens. Client stayed in-patient for 5 days. Client had no prior knowledge of the counseling appointment, but she reports a desire to engage in individual therapy. Client reports a desire “to be less unhappy and angry” with her family and the world in
Client takes a medication to prevent the migraines. Client reports stress in her life increases the frequency and severity of her migraines. Client reports her maternal aunt and younger sister also experience migraines. Client reports an in-patient hospitalization 3 weeks ago, due to weight loss, food restriction, and depression. Client reports no food restriction since her hospital discharge. Client appears thin but does not present as underweight. Her family’s mental health history was not discussed. This information should be obtained at the next session. Client appears to have relationship challenges with her mother and father. Client reports getting along “okay” with her mother, which should be explored further in a future session. Client appears to have a strained and distant relationship with her father. Client reports that she has not seen or communicated with her father for approximately two months. Client reports the last contact with her father was during a domestic violence scene involving the client’s 14-year-old sister. Client past relationship with her father was not explored and should be addressed in the next
