On 5/16/18 and 5/22/18 I spoke with and ordered wound care supplies for Mr. Elliott. We have been trying to coordinate meeting at an appointment with Dr. Raythatha. Since there has been a conflict in my schedule I will meet him at a physical therapy appointment. On 5/30/18 I spoke with Ortho-Tec regarding the new arm. They have had delays due to some parts that were on back order. He will be fitted for his arm on 5/31/18. On 5/31/18 I met Mr. Elliott at the physical therapy location. He walks still in a toe touch position with the left foot, he is very tight due to scar tissue to the ankle causing him difficulty getting the foot on the floor when walking or standing. His physical therapist said he is doing a great job. He attends as directed

Once school was out last year, I had done something to my foot. I don’t know what happened to it, but I know a general time frame it happened in. Over the course of two semesters I shadowed Dr. Robert Marsh as he interacted with countless and diverse patients seeking orthopedic treatment. Through my observation, I had the opportunity to experience the doctor patient relationship and the great responsibility that relationship entailed. His passion and commitment to every patient he helped, taught me that to care for and respect the patient is just as important as treating their injuries. This experience taught me that as a physician you need to work with the patient as well as those around you (residents/physicians/nur sing staff) in order to be a reliable source of information and have the insight for the patient’s well-being.
He said that Mr. Rasak is a high-risk surgery candidate. He fears that with his heart, edema to the extremity that he is at high risk for a surgery not healing and risking an amputation. He would not recommend any surgery. He said that he has another injection that also included Toradol that may help to give him more relief for a longer period of time. He can have the injections every 3 to 4 months. He also told Mr. Rasak that he is leaving the practice moving to another State so another physician in the practice who specializes in ankles will take over his care. We also discussed the possibility that he may be helped with the pain with some other medications such as Mobic. There are an estimated 6.5 million stroke survivors in the country. Twenty percent of stroke survivors suffer from foot drop, the inability to raise the foot (WLS-TV Chicago, 2010). Lower limb hemiparesis and foot drop affects gait, which can have an impact on normal daily activities. One of the main goals of patients with hemiparesis is to be able to walk independently and return to their normal activities. Physical therapists know that poor gait from ankle and /or knee joint problems can affect the pelvis, lower back, spine and neck as the body tries to compensate for the defective gait. Using electrical stimulation to treat hemiparesis and foot drop can help a patient achieve functional gait and return to their normal activities.
Mr. McDonald has started to treat with a Chiropractor. He reports that has helped with his low back pain. He said the Chiropractor doesn’t want him to start working with a personal trainer until he has finished with his physical therapy and acupuncture. The acupuncture has just started and he reports feeling relief just after one session. It took several calls to Dr. Shah’s office to obtain all of the orders he wanted for Mr. McDonald to have.
