PER REPORTER: On Monday, January 9th, Aayalliah reported to therapist (Courtney) that two different staff (Kenisha & Christy) smushed her behind her bedroom door. She said she was behind the door trying to get a toy and they smushed her by pushing the door against her. She said she told them "I'm behind here" and they said they didn't care. Aayalliah said she came from behind the door and they put her in a restraint on the floor until the nurse (Gloria) arrived. She said the nurse witnessed her being smushed behind the door then Aayalliah changed it and said Gloria witnessed her being put in a restraint. Per reporter, Janice (nurse) assessed the child to check her for injuries. There were bruises on her right forearm, inner right elbow,and
Cynthia Adae was taken to Clinton Memorial Hospital on June 28, 2006. She was taken to the hospital with back and chest pain. A doctor concluded that she was at high risk for acute coronary syndrome. She was transferred to the Clinton Memorial hospital emergency room. She reported to have pain for two or three weeks and that the pain started in her back or her chest. The pain sometimes increased with heavy breathing and sometimes radiated down her left arm. Cynthia said she had a high fever of 103 to 104 degrees. At approximately 0230 hours on February 16, 2016, a male subject was struck on the left side of his face by a pistol. Rashaun Grant, victim of the assault, was struck by the suspect, Rashaun Grant, after an argument occurred. Rashaun was transported to Hampton Regional Medical Center by his mother. The suspect fled the scene before Law Enforcement arrival.
At admission, Mollie’s main complaint was right hip pain. She was not oriented to person, place or time, responding with “I don’t know” to questions asked. While the emergency department nurse completed a physical assessment, Molly’s hospital record was retrieved. Molly was discharged from the hospital two weeks ago, having been admitted for dehydration. Her health history was significant for hypertension and diabetes. Her primary care provider and home health care information were included in Mollie’s hospital record, as was her daughter’s contact information. The emergency department performed an x ray to evaluate Mollie’s right hip pain and there was no evidence of a fracture. Per MD order, labs and samples were collected and processed: CBC-diff, CRP, hyperal, blood culture, prealbumin level and urinalysis. R/s Ja’Mya was telling other students her dad hit her in the head with a belt. R/s Ja’Mya stated that her dad was mad because she pushed her sister down, so her dad spanked her and hit her in the head with the belt. R/s she asked to nurse to look at the child’s head and the nurse said there is slight abuse near the hair line. R/s Ja’Mya told the nurse the bruise was from when she was spinning around and she hit the wall.

To begin with, there is Autonomy with Lora wanting to make the choice on not having the abuse she encountered be reported. Secondly, Beneficence is present with the nurse wanting to do good by doing what is best for the patient, preventing further abuse, and getting Lora out of the environment she is in. Morally, the nurse wants to keep Lora’s abuse private and confidential per Lora’s wishes, but legally the nurse is required by law to report the abuse no matter what age the child is. Nonmaleficence is present in that the nurse must consider the pain in suffering the patient and family will encounter when this is reported resulting in Lora being removed from everything that is familiar to her with the end result of getting Lora out of the abuse and into a healthy environment. The nurse has to be truthful to Lora and explain why the nurse will report the abuse and what the process will be which pertains to the principle of Veracity. The nurse must keep the information that was given about the abuse confidential by only telling the appropriate agency or those in a need to know basis that will deal with the abuse. The question presented is whether the broad language of § 1512(c)(2) suggests that an "official proceeding" encompasses a joint-task force investigation, pursuant to Congressional intent.
The reporting party (RP) stated on 4/22/16 she arrived at the facility to visit her mother, resident Maria Reeves DOB: 3/17/25. The RP observed the resident in bed and when the RP approached the bed to sit, the resident winced in pain. The RP disclosed when she sat on the resident's bed she yelled out in pain. The RP revealed the resident was unable to sit up in bed due to pain. The RP stated when the first medication technician entered the resident's room she was informed the resident was experiencing pain to her left hip. The RP stated the medication technician (Resha) made no effort to exam the resident for possible signs of injury. Isaiah Sais and Shawna Adams both admit that they had been verbally and physically abused; Shawna was even sexually abused. NBC Universal conducted an investigation and fount that there were more than 127 recorded abuse cases in the state of Florida within a two-month period. There are also several children that spoke to reporters about the abuse they were exposed to. Stephanie says that her mother signed her into foster care because Stephanie and the mother’s boyfriend were always fighting. The Calizaire sisters told about two instances they were abused. Sophia Calizaire, referring to her foster mother, said, “She took a belt, she started beating me with the belt, picked up a hanger, she started beating me with the hanger, picked up a heel, and started beating me with the heel.” She also says that one foster parent made the children eat dog food. Sophia tells about one incident with that foster mother and says, “She told me to eat in front of my sister while she watched and my sister was hungry.” That same foster mother also punished her by holding her head underwater until she almost drowned. Apparently, harsh punishments are quite common. Alexandria Hill was also punished for “misbehaving”. She was found in a corner of a dark room, and was not allowed to speak to anyone. She was being punished for waking up earlier than her foster mother and getting food and water from the kitchen. Not only are the parents abusive, but the other children can be extremely abusive as well. Princess Calizaire says that she once stayed in a foster home where one of the boys used to try to rape her every night before she would go to sleep, which actually made her terrified to go to sleep at night. “In the front seat was Gregg, driving, Sarah, in the middle, and Robyn, on the passenger side. In the rear seat was Jeff, behind the driver, Haley, in the middle, and Rachel, on the passenger side. EVERYONE was wearing their SEAT BELTS, as is our family habit. EVERYONE walked away from this accident with only bruises. Some barriers to evidence-based practice include fixed tradition, with an inability to adapt; time constraints, inadequate education, resistance from nurses and doctors (Melnyk et al., 2014). In looking at the evidence that informed my nursing practice: this doctor was not trusting my judgment, even though I have been a nurse for 28 years, almost as many years as this doctor has been alive. She was resistant to teamwork, she placed staff at risk, and did not do what was best for the patient; Further, 30%-76% of psychiatric staff is assaulted by a patient at least once in
Le Mien Culture states that this is justifiable. Dr. Leigh seems apprehensive about this situation as in the Le Mien culture this medical practice is a norm, and conflicts with western medical practices as the child is being physically harmed. Even though it is not considered child abuse in Ms.S culture, it is in the western society where the issue is occurring. Is it honest to not tell authorities, and to be empathetic to Marie. As a doctor she should report it as a duty.
