Community is an essential part of the existence of human being. The term community may be describe in so many forms. However, Yiu (2012) define community as a group of people, who live, learn, work, and play in an environment at a given time; they have unique characteristics and interest; function in a social system that meets their needs, such as an organization, a region, a province or a nation (p. 213). Elderly resident living in Leisure World Long term care center Brampton, Wood is the community I will be assessing. The facility was established in early 2003, it is located at the intersection of Humber west road and Gore way drive. The home consists of private and shared rooms, accommodates 160 residents with cultural diversity. Residents on wing 2 East at Leisure world Brampton, Wood was the target community of interest that was assessed. My aggregate are elderly resident age 80 – 90 years living with social isolation in wing 2 East at Leisure World long term care center, Brampton, Wood. The assessment was focused on the causes of social isolation; observed among the resident during windshield survey and interview with key informants. Social isolation and loneliness are described as feelings that are often experience by Older adults (Lucie,Gavin, Gosselin &Laforest 2009). Method used in collecting information includes qualitative and quantitative data. Qualitative data is used to determine the history of the community; quantitative data such as windshield survey, focus group and one on one interview were also included because both sources were important for the past and current information of the community (Stamler & Yiu, 2012, p.221).
