Many pivotal events over the last century have brought our healthcare system to where it is today. Some were indirect, such as World War II (and how it led to direct events such as medical advances that shifted focus from critical care and managing contagion to preventive medicine and health insurance as an employee benefit) and the internet (which has provided a wealth of tools and resources that were once only available to healthcare providers and has served to foster technological advancements such as Electronic Health Records and telemedicine). Others were targeted interventions, such as the Hill-Burton Act, which was enacted in 1946 and provided infrastructure dollars to healthcare facilities that agreed to provide a significant volume of free or reduced cost services to those with limited ability to pay (HRSA, 2014). Perhaps the most influential targeted event was the passage of Medicare and Medicaid programs, which was the point at which the government became the administrator for insurance programs for the poor, creating a system that would continuously grow and impact service delivery through regulatory control.

Health Maintenance Organizations, or HMO’s, are a very important part of the American health care system. Also referred to as managed care programs, HMO's are combinations of doctors and insurance companies that are formed into one organization. This organization provides treatment to its members at fixed costs and decides on what treatment, if any, will be given based on the patient's or doctor's current health plan. Sometimes, no treatment is given at all. HMO's main concerns are to control costs and supposedly provide the best possible treatment to their patients. But it seems to the naked eye that instead their main goal is to get more people enrolled so that they can maintain or raise current premiums paid by consumers using their service. For HMO's, profit comes first- not patients' lives.

Zappos is an online clothing and shoe shop in Las Vegas, which has become one of the world’s largest online stores since its inception in 1999.The case discusses in detail the reason that a small entrepreneur became a world class entrepreneur to establish such a successful business model. However, according to Sicko most the bourgeois politicians are trying to fix the system by finding new ways to extend the disaster of health insurance industries. For example, the Republican Nixon administration supported HMOs in the early 1970s, and Clinton administration ruined the health care reform in the first two years. The main merit of Sicko is that it moves the debate beyond this framework. To strengthen the argument, Michael Moore provides references to direct speech of reputable people in politics, healthcare and economics. The initial purpose of the discussion is to find the causes of healthcare failure and to provide recommendations in changing the current situation in dysfunctional health care system. As part of the applied framework, Moore incorporates statistics and visual presentation of comparison between healthcare in the USA and universal health care. Another strategy used by Moore in explaining the issue of broken health care system is inclusion of case studies of many Americans
Over the past 30 years Wal-Mart has grown into the world leader in retail and supply chain management. However, the company has faced and continues to face challenges with its stakeholders and other parties who are very critical of their business practices and ethics. Wal-Mart has established a reputation of squeezing the little guy to acquire the lowest possible price for the most earnings at the expense of some of its stakeholders as addressed in the 2005 documentary Wal-Mart: The High Cost of Low Price by director Robert Greenwald. Various parties including community groups, religious organizations, grassroots organizations, environmental groups and Wal-Mart customers as well as current and former associates have accused Wal-Mart of engaging in unethical business practices for the Every Day Low Price (EDLP) strategy.
