Total Hip Replacement (THR) is a surgical procedure that relieves pain from most kinds of hip arthritis, thus helping to improve the quality of life for the majority of the patients that undergo the operation. Arthritis simply means "inflammation of a joint." Arthritis can occur in any joint in the body. The main symptom of arthritis is pain which usually worsens with activity and weight bearing. This pain may be relieved most of the time through rest. There are over 100 types of arthritis but less than a handful account for over than 95 percent of the hip replacements that are performed. Some of these include Osteoarthritis (causes deterioration of the cartilage and the growth of bone spurs), Rheumatoid arthritis and Osteonecrosis of the femoral head. Doctors suggest that before considering hip replacement surgery for arthritis that the patient tries a number of non-operative interventions. Your doctor may have you consider little things such as weight loss (most arthritis is caused due to the weight bearing on a joint), activity modification or even the use of a cane. Patients should consider THR when daily living activities become harder to accomplish due to the pain. These activities would include walking, climbing stairs or other moderate pastimes. Anti-inflammatory medications which will help reduce the inflammation from the arthritis and reduce your pain may also be prescribed by the patient's doctor.
The modern total hip replacement was invented in 1962 by Sir John Charnley. Sir Charnley was an orthopedic surgeon who worked for a small hospital in England. The total hip replacement is considered by many to be the most important operation developed in the 20th century, solely based on the fact that it helps to relieve human suffering. Total hip replacement was first performed in the United States around 1969. Since then there have been more then a hundred of thousands of replacements performed in the United States. One of the first surgeons to perform this surgery was Charles O. Bechtol. In 1969, while he was a professor at UCLA, Bechtol started a total hip replacement program. The artificial hip joint is considered a prosthesis. There are two major types of artificial hip joints, cemented prosthesis and uncemented prosthesis. The type of prosthesis that will be used on the individual patient is decided by the surgeon depending on the patient's age, lifestyle and the experience that the surgeon has with a particular one.

I carried out this case study on Mrs. Casey (Pseudonym), any 86 year old woman who underwent an elective left total hip replacement (THR). After the OT student studied Mrs. Casey's past medical history in her medical chart, it was noted that she had previously undergone a right THR in 2011, which had been successful and free from complications. Ms. Casey had no other significant past medical history and had been an independent and active woman before the progression of her arthritis. Ms. Casey was required to have total hip replacements carried out on both hip joints as a result of severe Osteoarthritis (OA), which lead to stiffness, pain, and an eventual decrease in mobility, affecting her quality of life and involvement in meaningful occupations.
With improvement in post-operative quality of life, activity level and function, hip resurfacing may influence mortality. Secondly, studies suggest that high levels of cobalt and chromium in circulation contribute to increased risk of cancers (19). Mortality and cause of death following total hip arthroplasty has been reported low as compared to general population. In a study by the Finnish joint registry (20), the overall SMR was 0.69(CI 0.67 -0.70) in a cohort of 24,638 patients undergoing total hip arthroplasty. The cancer mortality was also low with SMR of 0.54(CI 0.50 – 0.57).
In orthopedic surgery, iliac bone graft which is commonly used for treatment nonunion or spinal fusion is contained multipotential stem cells or mesenchymal progenitor cells. These cells are found approximately 1 in 10,000 nucleated cells in the bone marrow. Friedenstein et al. (1970) reported that a rare population of cells was found by plastic-adherent ability (Friedenstein et al., 1970). These cells are capable of proliferation and mesenchymal lineages differentiation (Pittenger et al., 1999, Caplan, 1991). Unique antigens of MSCs (hMSCs) are controversy and the limited popularity of sorting cells techniques are not as widely used as gradient media in MSC isolation. Most MSC populations have been isolated by plastic adherent ability which is similar to that originally used by Friedenstein. Direct BM plating method is commonly used for cells from small animal (Lennon and Caplan, 2006, Nadri et al., 2007) but in human bone marrow, Density gradient centrifugation is the most commonly used method for isolating MSCs.

Osteoarthritis is a degenerative joint condition that primarily affects the hands, spine and the ankles and hips. It is known to be associated with aging, and is concerned with the cartilage that protects the joints (the meeting place of two bones). Normal cartilage allows for bones to slide over each other uniformly, acting as a shock absorber to any damage. However cartilage in osteoarthritis, located at the ends of the bones erodes and deteriorates, causing friction and hence pain, swelling and restricted movement. Therefore in osteoarthritic both the composition and appearance of the cartilage alters, while the body attempts to repair this damaged cartilage is broken down faster than it can be built up. A defective repair process can cause bone overgrowth, bone spur/osteocytes to form which can be seen at the ends of the joints. In osteoarthritis the bones come in close contact with each other, and wear away leaving the bone exposed and unprotected.

Although the cause is unknown, it is believed that the increasing age of the population, prevalence of obesity and injuries add to the progression of the condition. Osteoarthritis can affect any joint in the body; however, those of the hand, hip, and knee are often the most common. This condition may be asymptomatic, or may present symptoms including soreness, stiffness and pain. The symptoms are more common in the older population, those with limited activity levels, and those who are obese. Joint cartilage thins over time, causing an increased risk for symptoms in the elderly, and obesity puts extra pressure on the joints during activity. Osteoarthritis is commonly diagnosed with the use of a physical assessment along with results of radiology testing such as X-Ray and MRI.

Every disease is known to have its own cycle or process; well as for Osteoarthritis its process is pretty complex. In osteoarthritis (OA), at the end of the bone where they meet to form a joint, allowing movement, the cartilage covering the bone begins something called breakdown.

One of the major causes of chronic debilitation in industrialized nations is Osteoarthritis which results from damage to the joints, which may be due to trauma, infection, or age-related wear. Rheumatoid arthritis is encountered less frequently than osteoarthritis and is estimated to affect around one per cent of the world’s population. Amongst patients of Rheumatoid arthritis, women are three times more likely to be affected by this condition than men. This condition is caused by an inflammatory process where the body starts attacking itself. Rheumatoid arthritis also affects several joints, with inflammation sometimes seen in and around the lungs, the heart, the eyes and the skin. The most commonly reported complaint by arthritis patients is pain. The pain might be from the joint itself and be a result of inflammation, damage from the disease, or through daily wear and tear. Muscle pain is also common and is caused by having to force movements against stiff and painful joints. Although range of movement in the affected joint may be limited and uncomfortable, physical exercise has been shown to benefit those with arthritis. Physical therapy has been shown to significantly improve function, decrease pain in the long term and delay the need for surgery in advanced cases. The most common use of biomechanics is in the development of prosthetic limbs used for the handicapped. Most work on prosthetics is done in laboratories where scientists use calibrated machines to test stress and wear of artificial limbs. These days, prosthetics, are made of titanium and lightweight fiberglass to make a near perfect match with most people. The most common prosthesis is the replacement in a below the knee amputation. The American Society of Biomechanics (ASB) held a meeting at Clemson University of 1997 in order to develop a sports prosthesis that would stand up to every day flexing of the knee for performance in sports.

What is Arthritis? Arthritis is inflammation of a joint bringing such symptoms as chronic joint pain, stiffness, and swelling. The Arthritis Society states that approximately 4.6 million Canadians are currently living with arthritis; by 2036 this number will increase to 7.5 million (1 in 5). Health care costs and loss of productivity amount to $33 billion, by the year 2031 this number is expected to double. (The Arthritis Society, 2014). According to Aging in Contemporary Canada, arthritis and rheumatism is the most common chronic health problem of Canadian seniors affecting 47%. (Chappell, McDonald, & Stones, 2008, p. 221). With over 100 types of arthritis varying from mild to severe the most common age related form is osteoarthritis (OA) affecting 1 in 10 Canadians. Approximately 13% of Canadians suffer with OA. Joint damage from OA accounts for 80% of hip replacement surgery and over 90% of knee replacement surgeries. (The Arthritis Society, 2014). Severe cases of OA can restrict the ability to participate in activities and consequently affect a person’s quality of life. At this time there is no cure for OA without a joint replacement. Although OA can affect all ages the purpose of this paper is to focus on the impact this disorder has on the daily activities and functioning on the aging population. Discussion will also highlight the origin and manifestations of this disorder and will examine current and future treatment options available.
