I believe that this situation arose because of a lack of communication, as well as struggles for control. The understanding of where one person’s profession and responsibility lies compared to other professions sounds to be a common situation that arises in the realms of therapy. However, I think that this situation is not only applicable to other environments of occupational therapy, but all professions in general. I believe that this situation was a prime example of Schell’s Ecological Model of Professional Reasoning. At the end of the previous school year, Carol, and her daughter’s teachers noticed that Carol’s daughter was not progressing in her studies, and an evaluation conducted by the child study team revealed that she had a specific learning disability. An IEP meeting was scheduled, and conducted. The determination was made to place Carol’s daughter in the same resource room as Carol’s client Jody, for half of the day. Carol instantly realized that this decision put her in a predicament where a dual relationship would be created. She would then cross a clear boundary, and become a parent of a classmate, as well as the professional she has already established herself as to Jody and her parents. This would undoubtedly lead to instances in which both her client, and the client’s family would be interacting with her at both school functions and on class trips. She knew that allowing this to happen was against the BACB Guidelines for Responsible Conduct for Behavior Analysts. Carol voiced her opposition and explained the ethical quandary to those attending the IEP meeting, and what would occur if her daughter was placed in the same room with her client. Carol stated that she would speak with her supervisor, but it was probable that he would determine that she would not be able to continue working with Jody. In this article, the American Physical Therapy Association (2005) discusses the ethical conduct standards that a physical therapist assistant should follow. Within the preamble it states, “All physical therapist assistants are responsible for maintaining high standards of conduct while assisting physical therapists. The physical therapist assistant shall act in the best interest of the patient/client.” These standards of conduct must be followed by all physical therapist assistants. Occupational therapy was based off of psychology; we evolved from treating mentally ill patients with isolation as an efficient treatment plan. We must never forget we are known to be “the art and science of helping people do the day-to-day activities that are important and meaningful to their health and well being through engagement in valued occupations” (Crepeau, Cohn, & Schell, 2008). To other professional disciplines this article explains the difference between each of us, yet can also express our relation to one another. The basic goal of all therapeutic disciplines is to better our clients life, through physical, speech or occupational therapy. Every discipline targets different goals, may it be body mechanics, reducing a stutter or buttoning a shirt, at the end of the day our clients well being may it be through science, art or both is all that matters. After this incident I spoke to my team leader and we both agreed I needed to report this situation to higher management. I documented the occurrence under the Incident Report file and filled out an online incident report for the doctor due to his unacceptable behavior, unsafe practices and professional misconduct. Within one week, our department’s management contacted me, the team leader, and the resident doctor that was involved. They spoke to all of us about how to avoid scenarios like this in the future, they recommended that we look at each other’s role on the health care team as equal not above or below one another, and that we share power and control in our patient’s plan of care. They also reiterated that if any order or intervention is unclear that it is better to seek clarification rather than have any errors occur. Every Wednesday I was assigned a patient to attend to by a specific nurse who was also my supervisor in a hospital setting. My instructor ensured that this nurse followed my progress in respect to the way I was to handle the patient through written report. While taking care of the patient, I recorded various changes, he/she indicates towards recovery. I reported these changes to the nurse. While participating in this activity, I was expected to follow the strict guidelines by the instructor and nurse; as required by the rules and regulations, and code of conduct in nursing.

Averill, DeWitt, and Zimmer (1978), stated a person is commonly considered less responsible if their actions are interpreted as emotional rather than deliberate. While working for a previous employer as a Case Manager for persons receiving case benefits from social services, a popular and well-liked supervisor failed to provide consistent leadership for those in her unit. Her mood of the day, rather than policy dictated how circumstances were to be handled. Although most situations should be treated on a case-by-case basis, there should and must be a certain set of criteria followed to maintain program integrity. Based on this, as case managers, we assisted our clients based upon past practices; however, we were sometimes called-in to explain our actions. While we were working within the rules, if she felt our actions were not in compliance, we had to defend our decisions. To bolster the direction I took with my own cases, I would often use the supervisors past case actions as a precedent. Without a fair and equitable way to determine who received what, our program tended to rely on subjectivity rather than objectivity.
